    Compassionate care but cleanliness issues

I placed my husband here and overall we're happy - staff are warm, professional and attentive (Amanda RN and Iris stood out), therapies are very good, the updated bright rooms, outdoor deck, activities and tasty homemade meals give us real peace of mind and make visiting from the Keys convenient. Staffing feels strong and personalized care is common, but I've also seen troubling issues: ants in a room, urine odor, soiled clothing/diapers not always returned, a rude social worker, awkward tech-only check-in, and a concerning aide incident in the shower that should be investigated. In short, excellent, compassionate care in many ways, but insist on close oversight of cleanliness and staff practices before committing.

    Summary review

Overall sentiment: The reviews for Oasis at the Keys Nursing & Rehab are mixed but tilt strongly positive in frequency. A majority of reviewers praise the clinical staff, therapy programs, facility updates, dining, and activity offerings; many families report peace of mind and recommend the community. However, a smaller number of reviewers report serious concerns about hygiene, management response, and at least one allegation of abuse. These contrasting experiences create a polarized impression: for many residents and families the community functions as a compassionate, modern skilled nursing and rehab option, while for others the facility fell short in basic cleanliness, administrative responsiveness, and safety oversight.

Care quality and clinical services: Several reviewers highlight attentive, compassionate, and professional caregivers and nursing staff. Specific compliments include quick medical responses, proactive monitoring by nursing leadership, and strong therapy services (both physical and speech therapy described as "very good" and "effective"). Named staff (for example, an Amanda RN and a staff member named Iris) are singled out for going above and beyond and providing warm reception and detailed attention. These reports suggest a robust clinical core that delivers individualized care and helps residents progress through therapy plans.

Staff, culture, and responsiveness: A common theme is a family-like atmosphere with staff who learn residents' names, encourage participation in activities, and are described as welcoming and compassionate. Reviewers consistently note responsive maintenance and helpful admissions assistance, cheerful laundry staff, an on-site beautician, and staff-to-resident ratios that support attentive care. This positive culture is reinforced by mentions of holiday celebrations, outings, and staff who assist families through admission processes.

Facility, cleanliness, and environment: Multiple reviewers praise the physical plant: recent renovations, bright and clean spaces, rooms with large window views, outdoor decks and seating, and pleasant grounds. These features contribute to an attractive, modern environment that facilitates visits and resident engagement. However, there are notable negative reports around hygiene: several reviews reference a strong urine odor, residents found in soiled diapers, dirty clothing not returned, and at least one report of ants in a room. One reviewer even stated the facility was "not cleaned when needed" and another reported no front desk staff. These issues suggest inconsistencies in housekeeping and environmental services that can undermine the otherwise positive descriptions of the facility.

Dining and activities: Dining is frequently praised for thoughtful menus, well-presented meals, varied selections, and homemade soups. Dietary needs such as allergies are reported to be accommodated. Activity programming is described as active and engaging, with daily activities, social groups, field trips to local restaurants and shopping, and holiday events — all of which contribute to resident engagement and a sense of community.

Management and administrative concerns: While many reviewers report a responsive and helpful administrative team, several critiques point to troubling administrative lapses. One of the most serious concerns is an allegation of abuse by an aide in a shower and uncertainty about whether the aide was fired or appropriately disciplined; this raises questions about incident handling and transparency. Other administrative issues reported include a rude social worker, permitting sick visitors, inconsistent front-desk coverage, and a digital check-in process that can be difficult for non-tech-savvy visitors (including loss of a prior paper sign-in option). There are also brief but strong negative statements from some reviewers labeling the facility "bad" or "not recommended," indicating that a subset of experiences were severe enough to produce outright rejection.

Notable patterns and takeaways: The dominant pattern is of many positive experiences related to clinical care, therapy, social programming, and a modern facility — balanced against a smaller number of serious negative reports focused on hygiene, pest control, incident response, and administrative consistency. The presence of both glowing testimonials (including named staff who receive praise) and serious allegations (abuse, soiled residents, odors, pests) suggests variability in performance that may be linked to staffing shifts, specific units, or episodic lapses rather than uniform facility-wide conditions.

Bottom line: Reviews indicate that Oasis at the Keys offers many strengths — compassionate staff, effective therapy, engaging activities, good dining, and an updated facility — which make it an attractive option for many families. However, the serious nature of some complaints (abuse allegation, hygiene and pest issues, and perceived management shortcomings) are significant red flags that prospective residents and families should investigate further. If considering this community, it would be prudent to ask management about recent corrective actions for hygiene and pest control, incident reporting and staff discipline procedures, front-desk and visitor sign-in policies, and how the facility ensures consistent housekeeping and infection control across shifts and units.

About Oasis at the Keys Nursing & Rehab

Oasis at the Keys Nursing & Rehab sits in Tavernier, Florida, right up in the upper Florida Keys, and it's a 120-bed skilled nursing center where folks can get a range of care services with as much help as needed, whether it's for a short stay or something more long term. The staff does handle round-the-clock skilled nursing, and they're there to provide help with everything from daily activities, medication, and meals to more complex care like wound treatment, cardiac care, post-hospital recovery, stroke and neurological support, and orthopedic care. For people who need it, the community offers special memory care programs and 24-hour support, along with memory-boosting activities and the exclusive "Speak Out" program for folks with dementia or Parkinson's, which isn't something you see at every place, nor is electro-stimulation or ultrasound therapy, both available to help with pain, strength, and recovery.

If someone's only staying a short while or if a family caregiver needs a break, respite care is available, and there's also post-acute care, outpatient therapy, and restorative care, all managed by a team that includes physicians, therapists, and support staff who seem to work pretty closely with families and the local community. There's a focus on hands-on rehabilitation, with in-house physical, occupational, and speech therapy programs that include state-of-the-art equipment, so those who need help regaining abilities or managing pain can do so without leaving the building.

In terms of living spaces, long-term care residents stay in rooms with electric beds, flat-screen TVs, and phones, and everybody's got access to meals cooked fresh on site and a beauty and barber shop for grooming, which does make life a bit more comfortable. The shared areas are set up to encourage socializing and participation, and the facility has regular outings planned every month, so there's a chance to get out and about safely. Nutrition services, pulmonary care, and advanced wound care add more ways to meet the needs of those staying short term or moving in long term. New admissions happen every day of the week, so the support's always available, and with both inpatient and outpatient therapy, along with long-term custodial care, Oasis at the Keys has most bases covered for those needing nursing, rehab, memory care, or daily help in a supportive environment.
