    Clean facility with caring staff

I moved my loved one here and I'm very pleased - the facility is exceptionally clean and well-maintained with warm, inviting decor, a porch, pavilion and lakefront outdoor space that make it feel like a second home. The staff are friendly, courteous and knowledgeable; caregivers go above and beyond, coordinate home health care and provide high-quality medical support. Residents seem happy, food is good, it's wheelchair accessible with studio units, a salon and large sitting areas - I recommend it.

    Summary of reviews

Overall impression: Reviewers portray Country Gardens Assisted as a generally clean, well-maintained facility with a warm, homelike aesthetic in many common areas and a range of supportive services. Positive remarks center on staff demeanor and clinical capability, pleasant outdoor amenities, and the availability of private studio units. At the same time, reviewers identify several operational and physical limitations that prospective residents and families should consider.

Care and staff: Reviews emphasize courteous, knowledgeable caregivers who are described as respectful of residents' dignity and willing to go beyond routine duties. The facility appears to have adequate staffing levels and access to specialized medical care, and it coordinates with home-health providers. However, there are recurring mentions of lapses that impact safety and family experience — specifically, occasional resident-safety incidents and inconsistent communication processes (for example, staff unawareness of visitor arrivals). These points suggest strengths in day-to-day caregiving and clinical coordination alongside opportunities to tighten safety monitoring and front-desk/communication protocols.

Dining and activities: Dining receives favorable comments, ranging from good to excellent, indicating generally reliable meal quality. Social programming includes music and devotional groups and other communal activities, and residents benefit from large sitting areas and a porch. Activity offerings have been interrupted at times by infection-control events (notably during COVID), which reviewers say reduced programming temporarily. Prospective residents who prioritize robust, uninterrupted activities should ask how the facility presently structures programming and contingency plans for outbreaks.

Facilities and amenities: The campus offers accessible design elements, wheelchair access, a pavilion, lakefront/outdoor space, and on-site salon services, contributing to an overall pleasant atmosphere described by some as a "second-home" feel. Cleanliness is repeatedly noted as a strength. Conversely, private living spaces are described as small, and portions of the facility give an institutional impression to some observers; these physical attributes may influence quality-of-life for residents who value larger private quarters or a distinctly residential assisted-living environment. Additionally, the rural location provides a tranquil outdoor setting but may limit access to nearby medical specialists, shopping, and public transportation.

Management and operational patterns: Management appears effective in maintaining facility cleanliness and coordinating clinical services, while some operational gaps remain. Notable patterns include family responsibilities for tasks such as laundry, which may indicate limited on-site housekeeping or service expectations that families should clarify. Communication and visitor coordination are other areas for management attention. Finally, reviewers praise staff compassion and clinical capability but also note intermittent safety and monitoring concerns; families should ask about incident-prevention protocols, staffing patterns on different shifts, and how the facility manages and communicates clinical events.

Recommendation guidance: Country Gardens Assisted presents as a well-kept, medically capable facility with a strong caregiving culture and attractive outdoor/common spaces. Prospective residents and families should weigh these strengths against smaller private rooms, pockets of institutional character, the rural location, and the facility's past experience with activity interruptions and certain operational gaps. Direct questions to leadership about current safety protocols, visitor check-in procedures, laundry and housekeeping services, and the present status of activity programming will help determine fit for individual needs.

    About Country Gardens Assisted

Country Gardens Assisted is a small assisted living facility licensed for up to 6 residents, and some people might call it an Adult Foster Home, Personal Care Home, or Assisted Living Home, depending on where they live, and this place offers a home-like setting with both private suites and shared rooms, all furnished and with private bathrooms and kitchenettes, and the building comes with air conditioning, cable TV, phone, and internet to help folks stay comfortable and connected. Staff stays awake 24 hours a day to help with anything from bathing, dressing, or moving about, including help for people who need a wheelchair or two-person transfers, and they'll monitor things like insulin for diabetes and handle medication management or administration when needed, which is important for folks who need more than just a watchful eye. Housekeeping, linen service, and laundry or dry cleaning come standard, so residents do not have to worry about cleaning up after themselves, and meals get made three times a day, always with a focus on good nutrition and usually using organic ingredients, and the dining room lets people have meals restaurant-style throughout the day, and the kitchen staff makes special meals for people with conditions like diabetes or high blood pressure and takes allergies seriously.

Daily activities, group exercises, fitness programs, and arts or crafts events keep everyone engaged, and there are regular movie nights, music therapy, and pet therapy as well as spiritual or devotional gatherings on site or outside, and the staff can help get residents to doctor's appointments, errands, or places of worship. There's a library, book room, arts room, and recreation rooms, along with comfortable community areas set up for conversation or relaxing, and there are walking paths and garden areas for spending time outdoors. Memory care services are available for people with Alzheimer's or other forms of dementia, and the environment tries to help prevent wandering and reduce confusion for those who need it, and extra assistance is available for those who need respite or hospice care. Safety features include a 24-hour call system and an emergency alert system, and the building is wheelchair accessible and set up for non-ambulatory care and incontinence care. The on-site barber and salon or mobile hairdresser visits help everyone look and feel their best, and all the while, the staff gets noticed for helpful, joyful, and kind service, which helps create a friendly and supportive place to call home. Reviewers have given Country Gardens Assisted a score of 9.4 out of 20 reviews, pointing to good meals and atmosphere, and the people who live there can count on plenty of support for daily needs as well as activities meant to promote social, physical, mental, and emotional well-being, and renovations are ongoing to keep things as comfortable and safe as possible.
