    Dirty overcrowded negligent unsafe facility

I placed a family member here and I'm horrified. The facility is dirty and overcrowded (three men per room, six sharing a tiny bathroom), with urine/feces, bugs, and a scabies outbreak the facility knew about - a resident even died. A few staff (Jason, Ms. Kimmie, Ms. Tonya) were compassionate, but too many nurses were rude, negligent and short-staffed: meds gone, charts not checked, showers skipped, privacy/HIPAA breaches, missing discharge papers, and awful communication. It costs about $7k/month, yet management asks for donations and ignores safety - I will never recommend this place and it should be shut down.

    Summary review

Overall sentiment in these reviews is sharply polarized, with many families and callers reporting heartfelt gratitude for individual caregivers and certain clinical services, while an equally large body of reviews alleges serious systemic failures, neglect, and unsafe conditions. There are repeated, specific praises for compassionate nurses, excellent wound care, effective physical and occupational therapy, and a supportive admissions process that can make transitions from hospital to facility smooth. Several reviewers explicitly describe a family-like atmosphere, consistent team-oriented care, holiday events, music programs, and named staff (for example, Jason, Ms. Kimmie, and Ms. Tonya) who provided memorable, dignified care. Positive accounts frequently highlight successful rehab outcomes, hospice teams who provided respectful end-of-life care, engaged activity programming, and volunteers who enhance resident life.

Counterbalancing those positive reports are numerous, detailed complaints alleging neglect, abuse, and severe hygiene and safety problems. Multiple reviewers report residents being left in urine or feces for extended periods, clothes and linens unchanged for days, and refusal or failure to provide showers or basic personal care. There are explicit accounts of feces and urine in public areas, offensive odors, and claims of filth and pest problems. Outbreaks such as scabies were raised by multiple reviewers and described as mishandled. These sanitation and infection-control concerns are among the most serious themes because they directly affect resident safety and health.

Staffing and staff behavior emerge as a central mixed theme. Many reviews praise individual caregivers and teams for compassion, devotion, and good clinical skill; others describe the same workforce as inconsistent, short-handed, or poorly supervised. Several comments attribute lapses to being understaffed and overworked, with staff spending time at nurses' stations instead of assisting residents. There are specific allegations of rude, uncaring, or unprofessional employees, abusive language, and even laughing at incidents—contrasting with other reports that the staff are loving and treat residents like family. This variability suggests uneven training, uneven leadership oversight, or significant staff turnover influencing resident experience from day to day.

Clinical safety, documentation, and privacy are prominent concerns. Reviewers described medication errors and even reported medications being stolen, charts not being checked, failure to follow aftercare or physician instructions, and an attempted unsafe removal of a medical device. Several families allege privacy breaches, HIPAA violations, and missing discharge paperwork, as well as poor communication about transfers. There are reports of ambulances called for deteriorations, near-death ER transfers, and even deaths that some reviewers directly tie to inadequate treatment. These reports have driven calls for regulatory investigation, license removal, and legal action in multiple posts.

Management, billing, and institutional culture are recurring topics. Some reviews commend supportive upper management when present, but many characterize administration as profit-focused or unresponsive. There are multiple complaints about billing disputes, refund problems, and a perception that the facility prioritizes its bottom line. Requests for donations to staff and high stated monthly costs (roughly $7,000 per person in at least one review) raised concerns about value given the variability of care. Families also noted poor follow-through from care teams, care-planning meetings that felt perfunctory, and a general lack of transparency.

Activities and amenities receive generally favorable mentions where present—music, themed events, holiday parties, therapy programming, and occasional special offerings (Taco Tuesdays) were described positively by several reviewers. However, dining and nutrition did also receive criticism in places: reviewers noted unbalanced meals, lack of snacks, and minimal feeding in some instances.

Notable patterns and acute allegations that recur in many reviews include: (1) reports of individual staff heroes providing excellent care amid systemic problems, (2) repeated hygiene and infection control failures including scabies and pests, (3) serious documentation and privacy violations, and (4) impassioned calls from families demanding immediate oversight or closure of the facility. Several reviews reference a resident named 'Andy' and activism or hashtags calling for his release, indicating at least one high-profile, emotionally charged case alleging illegal confinement or mistreatment. There are also named deaths and calls for accountability connected to specific cases.

What emerges is a facility that appears capable of delivering high-quality, compassionate care in many individual cases, particularly in rehabilitation, wound care, and hospice. At the same time, there are repeated, specific, and severe allegations of neglect, poor hygiene, clinical and documentation errors, and management failures that pose safety risks. Families considering this facility should weigh these polarized accounts carefully: ask for current inspection reports, inquire specifically about staffing ratios and infection-control practices, request to observe rooms and bathrooms, verify how medication administration and charting are handled, and confirm how the facility communicates with families. Regulatory bodies and oversight agencies may find the volume and severity of complaints here warranting review; the reviews point to both commendable caregiving by certain staff and urgent, systemic issues that require corrective action.

    About PruittHealth - Shepherd Hills

PruittHealth - Shepherd Hills sits at 800 Patterson Road in Lafayette, Georgia, and looks a lot like an apartment building, though it's really made for older adults and people needing ongoing health care or help getting better after an illness or injury. The staff here knows how to care for people with many different needs and tries to do their work with warmth and a personal touch, making sure people feel safe and looked after and aren't left alone if they don't want to be. The place offers physical, occupational, and speech therapy right onsite, and has memory care for people dealing with conditions like dementia.

People living here can come for short-term stays to recover or stay longer if that's what's needed, because the building handles both. There's a focus on cardiac and pulmonary care and palliative care, and the staff keeps up-to-date with new technology and treatments. Skilled nurses and expert rehab staff try to guide residents every step of the way, making big changes a bit easier to manage, and they'll do what they can to support families, too, with things like a payment portal for bills and letting loved ones visit or check in on progress from afar.

The building itself feels homey and comfortable, though there's not much known about special features or outdoor spaces. People here can also get extra help, like pharmacy and infusion services, or help managing medicines and medical supplies. There's no pool or gym, but the menu of healthcare services is pretty wide, covering home health, hospice and palliative care, senior and assisted living, and personal care-though what's on offer at any one time might change, and the details about specific rooms or amenities aren't noted. At its core, PruittHealth - Shepherd Hills tries to give high-quality and compassionate care with skilled, attentive staff and makes sure families know what's happening every step along the way.
