    Compassionate responsive care; highly recommended

I felt truly supported from the start - nurses and CNAs were attentive, compassionate, and even attended his funeral, while the administrator and DON were responsive and gracious. Hospice and skilled nursing care were peaceful and effective, therapy improved his condition, and staff made us feel at home with clear communication and quick resolution of any issues. The facility is warm, well-maintained, active in the community (students and literacy outreach), and I highly recommend it.

    Summary of reviews

Overall impression: Reviewers convey a mixed but instructive picture. Many families and residents praise the facility for its compassionate clinical teams, rehabilitative therapy results, active programming and strong community ties. At the same time, several operational weaknesses were described that may affect everyday experience and risk perception.

Care and clinical services: Positive comments focus on effective therapy/rehabilitation, attentive nursing interactions, and supportive hospice transitions. Families described meaningful clinical interventions that improved residents' conditions, and clinicians who respond to changes in status. However, patterns of inconsistent daily-care delivery were also raised, most notably irregular bathing and personal-care schedules and variable attention to routine clinical needs. These inconsistencies suggest gaps in day-to-day operational reliability rather than wholesale clinical failure.

Staff, communication and family involvement: Many reviewers highlighted compassionate, helpful CNAs and nurses, administrators who listen, and inclusive care-plan meetings. Several families noted that staff went beyond clinical duties to support residents and relatives, including active outreach and respectful end-of-life care. Conversely, there are repeated accounts of variable staff professionalism and delayed responsiveness; some families felt communication tone and timeliness were uneven. These mixed accounts point to a facility with committed staff but uneven consistency in conduct and communication.

Activities and community engagement: The facility receives strong praise for its activities program and social environment. Reports emphasize a welcoming, home-like atmosphere, frequent programming, and visible community engagement — including educational donations and partnerships — that contribute positively to residents’ quality of life.

Facility condition and sanitation: Many reviewers describe a comfortable, well-maintained interior and larger resident rooms, but there are also operational concerns tied to environmental maintenance. Specific issues include dim lighting in certain rooms or common areas and pest-control concerns. Together these indicate that while the physical environment is generally pleasant, maintenance and sanitation practices may require targeted improvement in areas.

Management and notable patterns: Management responsiveness appears mixed: some reviewers describe prompt action and resolution when issues are raised, while others perceive management as prioritizing financial considerations over care decisions. There are also concerns about initial room setup and fall-risk mitigation at move-in, suggesting opportunities to strengthen admission protocols and safety checks. A small number of complaints rise to the level of regulatory concern for families; those instances underline the importance of direct verification (visits, care-plan meetings, and regulator records) when evaluating placement.

Bottom line: Green Acres Health & Rehabilitation demonstrates clear strengths in therapeutic outcomes, compassionate caregiving, community engagement and programming. Prospective residents and families should weigh those strengths against reported inconsistencies in daily personal care, staff conduct variability, and environmental maintenance issues. Recommended due diligence includes touring during different times of day, asking to review recent inspection records, meeting the nursing leadership and therapy team, and clarifying admission room-setup and fall-prevention procedures prior to placement.

About Green Acres Health & Rehabilitation

Green Acres Health & Rehabilitation is a non-profit skilled nursing facility located at 313 Allen Memorial Dr. SW in Milledgeville, GA, and it's known to some as Green Acres Nursing Home, which is how it's listed on the BBB website where they've got an A+ rating but haven't received BBB accreditation, and this place has been managed by Clinical Services Inc since 2003 and Brandon Poole since 2021, with the whole ownership belonging to Health Scholarships Inc and the place being part of the Ethica Health network. The facility is licensed and regulated under CMS guidelines, gets regular inspection reports, and, like most places, has had some deficiencies noted, though staff have a reputation for being joyful, helpful, and kind to residents and visitors, which helps set a friendly atmosphere.

Green Acres Health & Rehabilitation has a capacity for 98 certified beds and sees about 86 residents each day, so it's usually about 87% full, and that means it provides both short-term rehab for folks who need some extra help after being in the hospital and long-term nursing care for seniors with health needs that require daily assistance. Residents can choose between furnished private or semi-private rooms that have technological connections-there's all kinds of comfort mixed in with practical touches for safety and convenience. The facility provides 24-hour skilled nursing, long-term support, rehabilitation, pain management, and hospice care, with a focus on health and rehabilitative care tailored to each person's needs. There's physical, mental, social, and emotional activities planned by staff who've even received awards for their programs, and you'll see things like scheduled activities, games, and holiday parties on the calendar to help people stay active and interested.

There are clean landscaped outdoor areas where folks can take a breath of fresh air, along with a beauty shop for anyone wanting a haircut or spruce-up. Meals are cooked by chefs and meal planners who pay attention to nutrition, making sure every meal's balanced, and these are served daily in a common dining area. If anyone feels hungry between meals, there's a snack cart available through the day. Housekeeping and laundry services help keep things neat, which makes for less worry about chores. You'll see therapy services on offer to help residents improve or keep their mobility and range of motion, with care teams providing nursing and rehabilitation support, always trying to match care to every resident's health needs. The place is committed to helping each resident and their family by offering support, specialized care programs, and a team dedicated to improving quality of care, so, although it may have the expected ups and downs of any busy facility, Green Acres Health & Rehabilitation gives a steady, neighborly place for seniors who need skilled nursing, rehab, and everyday help.
