    Warm attentive staff; excellent rehab

I'm very happy with the care here - warm, attentive staff who go above and beyond, excellent rehab therapy, good meals, and a clean, home-like atmosphere. Social services and nursing communicate well, and I'd recommend it for short-term rehab or longer stays.

    Summary of reviews

River's Edge Rehabilitation & Living Center elicits strongly mixed but specific impressions. Many families and residents praise the bedside staff and therapy teams: nursing personnel, CNAs, and therapists are frequently described as compassionate, attentive, and focused on resident dignity. Rehabilitation services (physical and occupational therapy) are a clear strength; multiple accounts highlight successful functional recoveries and staff who go "above and beyond." Social-work and case-management resources are another positive area, with staff helping to coordinate discharges, navigate benefits, and secure alternative coverage when needed.

Dining, activities, and communal life receive largely favorable comments. Reviewers note good food quality and meal variety, steady programming and activities, and a warm, home-like culture. The facility's grounds, patios, and landscaping contribute to a pleasant environment for residents and visitors. Housekeeping and maintenance are often described as thorough, and leadership presence (including an accessible director of nursing and involved administrators) is identified as contributing to an overall welcoming atmosphere.

At the same time, there are recurring operational concerns that create variability in resident experience. Staffing consistency is a primary theme: periods of understaffing and delayed response to basic needs are described, which reviewers link to issues such as inconsistent bathing and assistance with toileting, and delays in attending to residents. Several accounts raise concerns about medication-administration practices and documentation, and there are references to sanitation or odor concerns in particular areas. Safety-process gaps are also noted, including inconsistent post-fall response and incomplete family notification procedures. The physical plant is described as generally well-kept but aging in places; some rooms are small and lack certain safety features.

Management-related patterns are mixed. Multiple commenters commend individual leaders and frontline staff, and some note organizational improvements under newer ownership. At the same time, administrative responsiveness (front desk and billing/insurance navigation) can be variable, and families sometimes needed to advocate to resolve coverage or placement issues. Taken together, the pattern suggests a facility with strong relational strengths — committed caregivers, effective therapists, and active case management — whose operational reliability varies depending on staffing and unit-level conditions.

For prospective residents and families, the important considerations are the facility's clear rehabilitative and relational strengths balanced against variability in day-to-day operations. Suggested areas to clarify during a tour include current staffing levels and shift coverage, medication-administration and incident-notification processes, incontinence- and bathing-care schedules, and specific room safety features. Asking to meet the case manager and therapy team can help confirm the supports that reviewers most frequently praised.

About River's Edge Rehabilitation & Living Center

River's Edge Rehabilitation & Living Center stands at 714 N Butte Ave and serves people who need rehabilitation or long-term skilled nursing care, and they take time to create custom care plans that fit each resident's needs, whether someone's healing from surgery, managing a condition like diabetes, or needing help with wound care, and the place is known for its focus on comfort, with therapy suites that have their own bathrooms, cable TV, and showers, so folks can feel more at home. The team here offers occupational, physical, respiratory, and speech therapies, and they go between high-tech therapy gadgets to old-fashioned strength and balance training, all to help residents get stronger and keep their independence as much as possible, and for people moving in, there are studio and companion apartments available, with private or semi-private choices that are cleaned regularly since laundry and housekeeping are always handled.

You'll find daily help with bathing, dressing, and getting meds, plus in-house therapy that lets residents work towards recovery right where they live, and meals are nutritious and prepared with care-so people get what they need to stay comfortable while still enjoying their food. River's Edge doesn't skip the social side either, with a monthly activity calendar filled with art programs, games, movies, worship services, group outings, and cards, and everyone can go outside to the landscaped grounds or enjoy planned excursions. There's also an adult day care program, help with transportation, and hospice care for those who want to stay in familiar surroundings. The physical therapy department has more than five therapy suites, and staff provide expert help for stroke recovery, postsurgical care, medication monitoring, and diabetes management.

If someone's family wants to learn more about caring for their loved one, the center runs regular seminars and workshops about long-term care and wellness, and they open their doors for community events and outreach, aiming to keep strong ties to the area. Meals, social events, and worship services help folks stay connected, and custom therapy plans are set up between the staff, the resident, their family, and healthcare providers. The center's reputation for kindness and a sense of home shows in its high reviews, and everything from laundry to nutritious menus, from active outings to quiet movie nights, points to a simple goal-helping people heal, live with dignity, and stay as independent as they can while they're here.
