    Good therapy, but alarming neglect

I have mixed feelings. Many therapists and aides were warm, caring and went above and beyond - good rehab, activities and a family-like feel - but I also saw alarming issues: heavy sedation/medication, missed basic care, hostile staff, loud paging/TVs, hot/cramped rooms and pest problems. I would not place a loved one here until management fixes staffing, medication practices, cleanliness and noise.

    Summary review

Overall sentiment about Oak Creek Rehabilitation is highly mixed and polarized. A substantial portion of reviewers praise the facility for its strong short-term rehabilitation services (physical, occupational, and speech therapy), a caring and family-like staff, and active social programming. Multiple reviewers emphasize that staff members are compassionate, attentive, and willing to go above and beyond; some describe the facility as a second home where residents receive one-on-one attention and emotional encouragement to recover. Several positive comments specifically mention active activity and social service departments, monthly holiday events and entertainment, and targeted programs for behaviorally challenged adults. There are also reviewers who noted responsive new management and helpful communication from medical staff.

Contrasting sharply with the positive reports, there are serious and repeated negative allegations that raise significant concerns. Some reviewers report neglect of basic personal care such as failure to brush teeth and provide showers, as well as hostile or dismissive interactions with staff (one reviewer named a case worker). Multiple reviewers describe poor facility conditions—reports include roaches/bugs, cramped rooms, and generally disgusting conditions—and there are specific safety-related allegations including an escaped/missing patient. HVAC problems are mentioned (rooms being hot while only one air conditioner worked properly), and pervasive noise complaints (loud paging, alarms, and televisions) appear across reviews. There are also troubling reports about medication causing excessive sedation for some residents, which in at least one case led to removal; combined with reports of staffing shortages and high turnover, these items contribute to a picture of variable and at times inadequate care.

Key patterns and inconsistencies emerge from the reviews. Many of the positive comments highlight individual employees or teams who are dedicated and create a family atmosphere, suggesting that staff behavior can be excellent depending on who is on duty. Conversely, the most serious negative reports tend to focus on systemic issues—cleanliness, safety, medication practices, and management/staffing levels—that may reflect shifts in administrative oversight or resource constraints. Several reviewers explicitly state that care has declined under current administration or that CNAs and nurses are leaving, while other reviews indicate recent managerial improvements and responsiveness. This divergence suggests uneven performance over time or across units/shifts: some residents and families experience very attentive care and effective rehab, while others experienced neglect, hostility, or hazardous conditions.

Specific areas a prospective resident or family should scrutinize based on these reviews include: infection and pest control procedures (given roach/bug reports), HVAC and room comfort, safety measures to prevent elopement, medication review and monitoring policies (especially regarding sedating medications), staffing ratios and turnover rates, how intake and visitation are handled, and noise-control practices. Activities and social engagement are a clear strength but were limited during COVID according to multiple reviewers; prospective families should ask what current programming looks like post-COVID. Management appears to be a central factor in reported variability: some reviewers cite a decline under certain administrators, while others note improvements under new management.

In summary, Oak Creek Rehabilitation receives both strong praise for rehabilitation services, compassionate individual caregivers, and engaging activities, and severe criticism for basic care failures, facility cleanliness and safety, staff hostility, medication sedation concerns, and inconsistent management. The mixed nature of the reviews indicates that experiences may vary widely; therefore, anyone considering this facility should perform a careful, in-person assessment, ask detailed questions about the negatives noted here, and verify current conditions, staffing, and quality-of-care safeguards before making a placement decision.

    About Oak Creek Rehabilitation

Oak Creek Rehabilitation sits as a facility that offers nursing care and a variety of rehabilitation services like physical, occupational, and speech therapy, and you'll find the staff helps with medication management, wound care, IV therapy, diabetic support, and even specialized cardiac care for conditions such as CHF, COPD, and recovery after surgeries like knee or hip replacements, and then there's help for memory care, behavioral care needs, adult day services, home health care, respite, and hospice care. The building holds 57 certified beds and most days about 39 residents stay there, and they've got both private and semi-private rooms with standard amenities along with communal areas for dining and social activities, and the meals aim to be nutritious and pleasant. The nursing staff provides 24/7 care under physician direction, but the nursing hours per resident per day are 3.01, which falls below the state average of 4.1, so while staff works hard, there may be less support at times than elsewhere, and nurse turnover is higher than average, with a rate of 72.9% compared to the 46.4% state rate, which might mean you see new faces more often than not. Inspections have found 10 deficiencies, including one related to infections, and there have been documented failures concerning resident protection from abuse, neglect, and actual harm, plus a couple of issues around not reporting suspected abuse quickly, so while they try to serve people's health needs, these are important things families might want to know about. The community accepts Medicare, Medicaid, and private insurance, and while their scores for staffing and inspection reports are lower than many other places, their quality measures rate much above average, and the overall CMS rating sits just slightly below average-not the highest, but not the lowest either. You'll find therapists on site who work to help folks recover and get back home when possible, and Oak Creek Rehabilitation organizes community activities, offers both short and long-term care, and keeps an eye on the basics like meals and room options, providing services in a home-like setting without any unnecessary frills.
