    Summary of reviews

Overall impression The reviews portray Clearwater Health and Rehabilitation of Cascadia as a facility with a strong emphasis on relational care and rehabilitation. Reviewers consistently highlight a family-style atmosphere, close relationships among residents, families, and staff, and CNAs who provide attentive, compassionate day-to-day assistance. Rehabilitation services are described as active and mobility-focused, with therapy staff working to get and keep residents moving within their abilities. Leadership and administration receive positive remarks for being supportive and visible. Care quality and staff Clinical and direct-care strengths center on hands-on caregiving and therapy. CNAs are specifically praised for their attentiveness, and therapy services are characterized as effective in maintaining resident mobility. That said, there are concerns about inconsistent staff conduct and communication tone; reviewers used terms that indicate occasional rude or unprofessional interactions. This suggests variability in staff training or supervisory reinforcement of communication standards rather than an across-the-board culture issue. Emergency response, clinical oversight, and billing One serious individual allegation described an emergency birth and subsequent billing for services not performed. While this appears to be an isolated, severe event in the sample provided, it raises two operational concerns: potential gaps in emergency clinical-response and oversight, and weaknesses in billing/documentation controls. Families evaluating the facility should ask about emergency protocols, staff training for atypical acute events, and the facility's auditing procedures for clinical documentation and billing accuracy. Activities, dining, and facilities The reviewed comments emphasize social warmth and community engagement, which indicate an active social environment and likely regular activities that help foster a family-like atmosphere. There is limited information in the supplied summaries about dining quality, menus, or the physical plant and cleanliness; reviewers did not provide substantive detail on food service or facility maintenance in these excerpts. Prospective families should request current menus, sample activity calendars, and an in-person tour to assess dining and facility conditions. Management and notable patterns Management and administration are highlighted positively, with references to approachable leadership and a responsive administrator. That leadership appears to support strong relationships with families and community partners. The notable pattern in the feedback is a contrast between generally high marks for daily caregiving and rehabilitation versus isolated but serious concerns about staff professionalism in communication and operational controls (emergency response and billing). These patterns suggest the facility's strengths are interpersonal care and therapy, while targeted improvements could be made in emergency preparedness, staff conduct consistency, and administrative controls. Recommendations for families For families considering this facility, recommended follow-ups include asking for: (1) descriptions of emergency-response protocols and staff training records, (2) examples of billing and documentation audit practices, (3) observation of CNA–resident interactions during a visit, and (4) current activity schedules and dining information. These targeted inquiries will help confirm the generally positive caregiving and rehab practices while clarifying how the facility addresses the operational gaps suggested by the reviews.

About Clearwater Health and Rehabilitation of Cascadia

Clearwater Health and Rehabilitation of Cascadia sits at 1204 Shriver Rd in Orofino, ID 83544, and you'll find that the staff take care to put together treatment plans that fit each person's needs, whether someone's staying short-term to recover or for the long-term with more help needed every day, and the facility runs with 24-hour skilled nursing care along with patient-centered rehabilitation programs. The place has 60 certified beds and usually about 39 residents each day, with rooms that can be private or semi-private, which gives people some choices about their space. There's a range of services here, including nursing care, short-term rehabilitation, transitional care, medication management, specialized help for those with dementia or Alzheimer's, and palliative or hospice care when needed, plus residents have support from social and recreational activities to help keep up community life and stay engaged. Clearwater's team includes nurses and therapists trained to provide physical, occupational, and speech therapies, and they focus a lot on helping each person work toward their own health goals. The facility's pretty close to the hospital, making it easier to get to in emergencies or after surgeries or illnesses. People can come in for tours to meet the staff and see how the place feels before making a decision. There's management from Cascadia Healthcare, and since January 2017, Warren Bodily has held managerial control, with direct ownership from Cascadia Idaho Operations LLC and indirect stakes held by Cascadia Healthcare LLC, Warren Bodily, Owen Hammond, Nathan Hosac, and Matthew Smith. The building has to follow federal rules about infection prevention and resident care, and while the staff work hard for patient safety, recent inspections, like one on April 17, 2025, reported 17 deficiency citations, with some about infection control, resident assessment, care planning, and a few involving nutrition and protection of residents' property, so some areas have room for improvement. Nurse turnover stands at 36.4%, with nursing care hours averaging 3.29 per resident per day, which can affect consistency of care at times. Clearwater's a for-profit limited liability company, and while they serve mainly in English, some staff may speak other languages. They're not currently taking new patients but can let folks know when a spot opens up. The environment aims to be supportive and welcoming, with comfort-focused amenities and a code of conduct to protect residents' privacy and rights. The staff try to help each person live as well as possible day to day, adapting care plans as situations change.
