    Caring professional team provided peace

I was very pleased with my loved one's stay - the staff (nurses, CNAs, rehab and therapy) were kind, attentive and genuinely compassionate; Crystal and the customer service manager kept us updated daily. The building is clean, well-maintained and welcoming, with good meals, activities, snacks for picky eaters and thoughtful end-of-life support. I felt peace of mind and would recommend this caring, professional team for rehab or long-term care.

    Summary of reviews

Reviews present a mixed but focused picture of care at Citadel of Bourbonnais. Many families praised the caregiving teams for compassion, warmth, and personalized attention: nurses, CNAs, aides, therapists and nutrition staff are frequently described as attentive and invested in residents' comfort. Several accounts highlight strong hospice and end-of-life support, including spiritual accompaniment and family-oriented comfort measures. Rehabilitation and therapy services are noted positively in multiple reviews, and specific clinical aspects such as wound care and diligent nursing are called out as strengths.

Facility environment and programming are also commonly cited as positives. The building and units are described as clean and well-maintained, with a home-like atmosphere. Activity programming, group outings, hair salon access and transportation are emphasized as meaningful quality-of-life features. Dining receives favorable mentions for variety and an ability to accommodate preferences and snacks. Proactive family outreach from certain administrative staff and a dedicated customer-service liaison contributed to peace of mind for many families.

However, a consistent theme in critical feedback is uneven operational performance. Communication and notification procedures are a recurrent concern: families cite delays or lack of notice regarding hospital transfers, unclear responses to medical information requests, and inconsistencies in how visitation or POA access is handled. These issues are tied to broader administrative clarity gaps, including processes for authorizations and documentation. Several reviews describe incidents that indicate inconsistent enforcement of visitation policies and variable access for authorized representatives.

Clinical-operations weaknesses appear in patterns as well. Reviewers describe variability in staff responsiveness to urgent clinical requests, delayed or inconsistent medication and IV administration, and gaps in controls over medications and personal items. Personal-care scheduling (for example bathing and hygiene assistance) and therapy availability are sometimes inconsistent from day to day. A few accounts referenced safety concerns that prompted formal complaints or external welfare checks; these suggest the need for clearer incident-response and de-escalation protocols.

In summary, Citadel of Bourbonnais shows notable strengths in compassionate caregiving, hospice support, rehabilitation services, facility upkeep, and activity programming. Those strengths are tempered by operational inconsistencies in family communication, visitation policies, medication and personal-item management, and occasional lapses in staff responsiveness and conduct. Prospective residents and families will likely find strong interpersonal care and programming there, but should ask specific questions about communication channels, visitation/access procedures, medication controls, and contingency protocols to assess fit and to mitigate the operational concerns described by some families.

    About Citadel of Bourbonnais

Citadel of Bourbonnais sits at 20 Briarcliff Lane in Bourbonnais, IL, in a single-story building designed for comfort and easy access, and offers long-term care, skilled nursing, assisted living, rehabilitation, and specialized memory care for seniors who need help with daily tasks or more complex nursing support, and there's this Hartwell House assisted living residence if someone needs some help but doesn't require full-time nursing, whereas people with memory loss, Alzheimer's, or dementia can receive thoughtful care, and hospice or palliative care is also an option for those who need it. Private and semi-private suites are available, all with individually controlled heating and air conditioning, private telephones, mailboxes, voice mail, cable TV, and regular weekly housekeeping and linen services, so residents don't have to worry about chores, and the staff take care of food services with meals and snacks that are tailored to preferences, plus full-service dining options so eating feels good and nutritious.

The center runs 24/7 which lets residents get round-the-clock care and support, and nurses, CNAs, and activity aides watch over everyone, making sure health checks, medication management, personal assistance, and help with daily activities are always taken care of, and if someone needs to take a temporary break from being a full-time caregiver, respite care gives them support and lets their loved one stay for a short stay. Therapy and rehabilitation services include physical and occupational therapy, stroke rehabilitation, and wound care, and people who need complex medical care, like end-stage kidney disease, can get dialysis on site in a quiet room just for dialysis, which is convenient since it means no one has to transfer to another clinic for treatment.

Citadel of Bourbonnais has both Medicaid acceptance for payment and a customer service manager who keeps in regular contact with family members, so people know what's going on with their loved ones, and worship services and pastoral care are available for spiritual needs. Housekeeping and laundry are included, and office hours run Monday to Friday, 8 AM to 5 PM, which is when most business gets done. There's no telehealth service, but there are a variety of home care and day care programs for different needs, so people can stay connected with their community if that's what they're looking for. All in all, Citadel of Bourbonnais provides personalized attention in a calm and home-like environment, with suites that allow for treatments in familiar surroundings, aiming to support healing while maintaining respectful, individualized care for each resident or patient.
