    Cozy compassionate rehab; exceptional care

I had rehab here and was impressed - a cozy, home-like facility with friendly, compassionate staff who truly go above and beyond. Therapy and the rehab-to-home program were phenomenal; I improved physically and emotionally, and nursing care and food were excellent. Small, family-oriented community with flexible stay options and welcoming management (special thanks to Jerilane and Kathy Digs). I'd highly recommend it and would return for future procedures.

    Summary of reviews

The reviews indicate a facility with clear strengths in interpersonal care and rehabilitative services alongside recurring operational concerns. Many families and visitors describe a warm, home-like atmosphere supported by a small, community-oriented setting. Staff members โ€” including nursing assistants, social services, and office personnel โ€” are frequently characterized as welcoming and willing to go above and beyond. The therapy department and the rehab-to-home program are consistently highlighted as standout services, with several reviewers reporting positive recovery outcomes and successful transitions back to home.

Clinical care quality appears mixed. Positive accounts emphasize effective PT, outpatient therapy availability, and successful short-term rehabilitation. Conversely, there are recurring mentions of delayed responses to resident call lights, late medication administration, and gaps in timely incontinence care; these point to operational weaknesses that can affect day-to-day resident experience. Prospective residents should ask specific questions about medication-administration protocols, call-response targets, and how incontinence care is scheduled and monitored.

Staffing and management impressions are similarly polarized. Multiple reviewers praise an inviting administrator and a helpful office team, and specific staff members received named gratitude for their efforts. At the same time, others noted instances of unresponsiveness or unprofessional communication. This suggests variability in staff conduct and leadership follow-through; it may reflect staffing-level fluctuations, shift-to-shift differences, or uneven training/supervision. Visitors should inquire about staffing ratios, staff training programs, and leadership oversight during a tour.

Dining, activities, and the physical environment are generally seen as positives. Several reviewers complimented the food, the friendly atmosphere at meals, and the facilityโ€™s cozy, homelike character. The small size is frequently cited as a benefit for social feel and personalized attention, and opportunities to volunteer or work at the site were noted as community-strengthening features.

Safety and facility maintenance merit targeted questions. Reviewers mentioned malfunctioning room alarms and other equipment-maintenance issues, which, combined with responsiveness complaints, indicate areas where operational reliability could be improved. Cleanliness and general upkeep were not a dominant theme in either direction, but alarm/equipment maintenance and response procedures are specific operational areas to verify.

Overall pattern: strong rehabilitative services, a warm community atmosphere, and many staff who are described as compassionate and effective, contrasted with intermittent operational weaknesses in responsiveness, medication timeliness, incontinence-care scheduling, and equipment maintenance. For families prioritizing therapy outcomes and a homelike setting, this facility may be a good fit provided they confirm current staffing levels, response-time protocols, medication administration practices, and alarm/equipment maintenance during a visit.

    About McPherson Care Center

McPherson Care Center, also called McPherson Operator, sits in McPherson and serves as a senior living community offering assisted living, memory care, nursing home, independent living, skilled nursing, and home care. The nursing home has 50 certified beds, usually holding about 43 residents, and has an occupancy rate of 86%. It holds a 2-star rating from the Centers for Medicare & Medicaid Services and an average score of 8.1 out of 10, making it the 4th highest-rated community in McPherson. Residents find both studio and semi-private or companion rooms, with amenities such as a dining room, WiFi, a hair salon/barber, outdoor living spaces, and several areas for socializing. The community offers nutritious meals made with good ingredients that support health and has special activities that aim to keep residents engaged physically, mentally, and socially.

The care team provides round-the-clock assistance, including nursing care, skilled nursing, respite care, rehabilitation, and both inpatient and outpatient rehab. Short-term, long-term, transitional, palliative, and comfort care are part of the services, and there's a focus on rehabilitation for those who need it, like speech, occupational, cardiac, orthopedic, pulmonary, physical, and respiratory therapy. Residents dealing with pressure sores, moderate to severe pain, and infections can receive help, and the staff works to avoid pressure ulcers and major falls. Vaccinations are available, with high percentages for both flu and pneumonia shots among residents. Trained staff manage complex needs like wound care, tracheotomy, IV antibiotics, diabetes, CPAP/BiPAP, PEG tubes, CVA management, life vest care, and sepsis care, and they've got resources for stroke recovery and dialysis too.

Staffing at McPherson Care Center stands at about 0.51 RN hours, 0.83 LPN hours, and 1.83 CNA hours per resident, each day, which adds up to about 0.4 hours of nursing care per resident, daily, and the nurse quality grade is A-minus with a facility grade of B+. Though there isn't a Resident Council or Family Council, the staff aims to foster a nurturing and secure environment, making room for personalized care plans shaped with help from residents and families. They review residents' health regularly after big health events or when new people arrive. There are some health and environmental safety deficiencies on record, but the facility is fully sprinklered and not marked as a special focus or hospital-based place.

McPherson Care Center is a for-profit, corporation-owned facility that accepts Medicare and Medicaid, offering help with eligibility, long-term care planning, and cost comparison tools for senior care. The facility provides educational programs for caregivers, courses on dementia and Alzheimer's, certification training, and job resources. Community advisors can help families find care options that fit. Staff at McPherson Care Center are said to be kind and joyful, working to make life easier for both residents and visitors, and the place supports a sense of belonging and peace for seniors who live there, whether they need short-term help or long-term care.
