    Compassionate staff and excellent therapy

I've been very pleased with Liberty Care - the place is clean and home-like, the activities and rehab/therapy team are excellent, and nurses, therapists, aides and support staff (Nurse Emily especially) are kind, attentive and genuinely caring. Residents are treated like family, greeted by name, and staff go above and beyond to make you feel welcome. I'm grateful for the compassionate, community-oriented care and would recommend it.

    Summary of reviews

Feedback about Liberty Care and Rehabilitation Center is strongly polarized: many family members and former residents describe high-quality, compassionate care and excellent therapy outcomes, while other accounts raise significant operational and clinical concerns. Positive comments consistently highlight a warm, family‑like atmosphere, friendly individualized interactions, and a rehabilitation department that produced measurable functional gains for some residents. Housekeeping and dietary services are frequently noted as strengths, and the building’s interior is described as clean and well maintained.

Care quality and clinical issues are an area of divergence. Several families praised nursing staff for attentiveness and quick escalation to emergency care when needed; conversely, other reports describe delays in care responsiveness, medication-administration problems, and communication gaps when incidents occurred. These contrasting accounts suggest variability in clinical consistency across shifts or units rather than a single uniform level of care.

Staffing and workforce patterns are an important theme. Reviewers identify a dedicated core of long‑term caregivers and therapists who foster a supportive, personable environment. At the same time, families describe frequent use of agency nurses, short staffing at times, and uneven performance among departments. These staffing fluctuations appear tied to inconsistencies in service delivery and timeliness of routine care tasks.

Dining, activities, and rehabilitation receive mixed but generally positive feedback. Many reviewers value the activity program and social opportunities that contribute to a homelike feel; therapy services are repeatedly singled out for producing positive outcomes. However, meal satisfaction and consistency were raised as concerns by some residents and families, indicating that food quality and preference accommodation may be variable.

Facility and logistics observations are likewise mixed. The physical environment is often praised for cleanliness and pleasant common areas, but reviewers also note small room sizes and a prevalence of semi‑private rooms. Some families raised sanitation and hygiene concerns in specific situations, suggesting variance in execution of housekeeping and personal‑care protocols.

Management and governance is another recurring subject. Positive comments reference a supportive community and staff camaraderie, but there are also comments about leadership communication issues, perceived rudeness in some department heads, and serious allegations related to billing or insurance practices. Those financial concerns are significant and merit direct inquiry by prospective residents and families.

Overall, Liberty Care appears to offer strong rehabilitative services, engaging activities, and compassionate caregivers in many cases, producing positive outcomes for many residents. At the same time, inconsistent staffing, medication and incident‑response processes, variable dining and hygiene execution, and questions about management transparency are clear patterns that prospective residents and families should investigate. Recommended next steps for decision‑making include asking facility leadership about staffing ratios and agency nurse use, reviewing medication‑administration and incident‑notification policies, touring both private and shared rooms, and requesting documentation or clarification about billing and insurance practices.

About Liberty Care and Rehabilitation Center

Liberty Care and Rehabilitation Center offers assisted living and Alzheimer's care, with skilled nursing, speech therapy, and occupational therapy available, and they do both long-term and short-term care, so people can get help for a while or stay if they need it, and the care plans get made by a group of health professionals within seven days after assessment, so residents' needs are looked at pretty soon after moving in. This place has 97 certified beds and usually has about 81 residents each day, and the nurse staffing level comes out to about 3.60 hours per resident a day, but the nurse turnover rate's pretty high at 63.4%, which means many nurses leave during the year. Liberty Care and Rehabilitation Center works with groups like the Alzheimer's Association, American Lung Association, and American Heart Association, and it's part of Kindred Healthcare, though Signature Healthcare LLC has run things since August 2013, and it's now in Signature HealthCARE's Hometown network that tries to bring care and rehab together in a bigger picture.

The building has parking for customers and onsite services to help with daily needs, plus different amenities aimed at matching what seniors like or need, and they have quality of life and spirituality programs for all backgrounds. They've even joined the Eden Registry for person-directed care and talk about dignity a lot in their mission, working towards treating everyone as an individual and focusing on personalized spirituality and care. You'll find clinical therapy programs that are tailored for each resident, and they try to make activities and care fit different cultures and religions. It's been cited for one infection-related deficiency and a total of 16 deficiencies on inspection reports, so that's something to keep in mind, but they do have safety measures in place for accident hazards and supervision, wanting to avoid accidents as much as possible. The center focuses mainly on nursing home services and rehabilitation, so most folks come here for steady care when they can't live at home safely anymore, and the environment tries to keep things resident-centered so that dignity and comfort are part of daily life.
