    Friendly staff clean active community

I live here and am very pleased. The staff are friendly, caring and helpful, with nurses on duty and excellent personal/bathing care. My spacious, clean room and the well-kept grounds feel comfortable. The dining room serves great food, and there are tons of activities - movie theater, salon, activity rooms, game nights/bingo and church visits - so it's an active, comfortable place with private or shared room options.

    Summary of reviews

The reviews present a mixed picture with distinct strengths in hospitality, amenities, and social programming alongside operational and clinical concerns. Many comments emphasize friendly, helpful caregivers, an active calendar of events (including bingo, game nights, and church visits), and a range of on-site amenities such as a movie theater, salon, and activity rooms. The facility’s dining experience and dining room atmosphere receive positive remarks, and reviewers frequently note clean, well-kept grounds and spacious resident rooms with private-room options.

Care quality and staffing are the primary areas of divergence. Positive impressions of compassionate individual staff members and visible nursing presence coexist with reports of delayed responses to call lights, inconsistent bathing and personal-care routines, and broader perceptions of inadequate staffing levels. Several reviews describe consequential clinical problems that align with operational patterns — for example, concerns about pressure-injury prevention and gaps in ongoing skin care. There are also mentions of care-continuity issues associated with hospital transfers and readmissions, which families should consider when evaluating clinical handoffs and care plans.

Dining and daily life are generally highlighted as strengths: reviewers praise food quality and the social dining environment, and activities are described as plentiful and engaging. However, there are notes of inconsistency in meal delivery and follow-through in some situations, which suggests variability in day-to-day service execution despite an overall appealing dining program.

Facilities and amenities are consistently seen as positive contributors to resident quality of life. The building, common spaces, and grounds are described as clean and well-maintained, and on-site features such as the salon and movie theater support social engagement and recreational variety. Private and spacious rooms are an additional advantage cited by families.

Management and administration receive mixed feedback. Positive caregiver interactions contrast with comments about weak administrative responsiveness and operational shortcomings. Cost is another recurring theme: several reviewers perceive the monthly fees as high relative to the value delivered, given the operational inconsistencies they observed.

Overall, this facility appears to offer strong hospitality, social programming, and physical amenities, but prospective residents and families should probe staffing levels, call-light response times, personal-care schedules, skin-care and pressure-injury protocols, and care-continuity practices during visits. Observing the floor during peak times, asking for staffing ratios and recent incident/transfer histories, and reviewing individualized care plans can help clarify whether the facility’s strengths will meet a specific resident’s clinical and daily-care needs.

About LeGrand Health Care & Rehabilitation Center

LeGrand Health Care & Rehabilitation Center sits in Bastrop, Louisiana and has offered senior care since 1972 under the guidance of Donnice Reynolds, with Doris Johnston and Tammie Sistrunk included in ownership. The center runs with 125 certified beds and stays open around the clock. The facility has 16 total deficiencies, including some in areas like keeping residents mobile, caring for incontinence needs, and making sure nurses and aides have the right skills; these deficiencies have shown no actual harm, but there's a risk for more than a little harm if not fixed. Nurse turnover runs at 41.3%, a bit less than the state average. Staff nurses give 4.04 hours of care per resident each day, which is higher than what most state facilities provide.

LeGrand Health Care & Rehabilitation Center has a homelike setting with rooms including private suites, couple's rooms, and semi-private options, all wired for phone and internet access. The center serves three home-cooked meals a day, cooked by a registered dietitian, and residents can eat in an elegant dining area or in private dining rooms, especially in the secured Alzheimer's unit which also has its own dayroom, a special bathing area, and a fenced patio for walking.

The community runs as both a skilled nursing and assisted living facility, offering full nursing care, long-term and short-term rehab, respite stays, and high-acuity medical support, with a focus on helping residents regain movement and manage pain through therapy gyms and specialized in-house therapists for physical, occupational, and speech therapy. There's memory care for those with Alzheimer's or dementia, with spaces for both privacy and activities, while social services and a Certified Activity Director keep programs running for emotional and social enrichment. The property holds a library, an on-site chapel, a salon and barber shop at no extra charge, an activity room, a cinema theater for movies and popcorn, a courtyard for fresh air, and free cable TV and Wi-Fi.

LeGrand Health Care & Rehabilitation Center supports its residents' needs for both medical and emotional care, making space for wellness and community events. It operates under Paramount Healthcare Consultants, carries several accreditations for care, and tries to give a warm environment where residents feel part of the community. The center keeps its social and health care simple yet focused, though it continues to address areas where it hasn't met standards in care planning and staff training.
