    Excellent compassionate care during rehab

I stayed for rehab and had a very positive experience - the beautifully renovated, light and open facility is clean, tidy, and well-situated with many activities (karaoke Fridays) and good meals. The therapy team, nurses, CNAs, onsite midlevel providers and housekeeping were skilled, compassionate, and communicative - concerns were always handled respectfully (special thanks to Lashetia). I'd highly recommend this caring, well-maintained place for short-term rehab or longer stays.

    Summary of reviews

The reviews of Pilgrim Manor Skilled Nursing and Rehabilitation present a mixed picture with clear strengths in rehabilitation and notable operational concerns. The facility is frequently praised for its therapy services: many families described effective short-term rehabilitation, competent physical and occupational therapists, and measurable functional gains such as improved mobility and weight stabilization during recovery stays. Several accounts also highlight attentive individual caregivers, a capable clinical team for wound care, and onsite medical providers that support clinical decision-making for complex patients.

Staffing and staff conduct emerge as a key axis of variability. Numerous reviewers commended specific nurses, aides, therapists, and administrative personnel for being warm, communicative, and helpful; those individuals were credited with positive outcomes and an improved family experience. Conversely, other reviewers described inconsistent responsiveness, lapses in monitoring, and concerns about tone and professionalism from certain staff members, including communication gaps on night shifts. These conflicting impressions suggest uneven staff performance and possible staffing or supervision issues that affect resident experience from shift to shift.

Clinical safety and operational processes are another area of concern. Reviews referenced delays in attending to residents, gaps in medication and treatment monitoring, problems with transfer handling, and instances that families felt indicated insufficient clinical follow-up. These items cluster into broader issues such as inconsistent incident-response processes and family communication following clinical events. Reviewers also raised sanitation and housekeeping variability, incontinence-care delays, and pest-control or environmental maintenance shortcomings that suggest inconsistent housekeeping protocols in some units or rooms.

Dining, activities, and facility environment were commonly cited as positives. Many families described enjoyable meals, a strong dietary staff, and activities that engage residents (examples cited include social events and occasional pet or community programs). The facility itself has been noted as recently renovated in parts, with light, open common areas, a tidy entrance, and generally pleasant communal spaces; however, room-level upkeep was reported as inconsistent across different stays.

Management and administrative practices show mixed performance. Several reviewers praised responsive administration, helpful admission staff, and clear communication from office personnel. Others reported unresolved complaints, missing personal items, and concerns about billing or financial processes — including at least one serious allegation of unauthorized charges. These items point to weaknesses in accountability, complaint-resolution procedures, and financial oversight that families should inquire about directly.

Notable patterns for prospective families: Pilgrim Manor appears to offer strong rehabilitation resources and several highly regarded staff members, making it a reasonable option for short-term rehab needs. At the same time, the facility shows variability in day-to-day caregiving, housekeeping, and administrative follow-through. Families considering the facility should prioritize a detailed tour, ask about staff-to-resident ratios and night-shift coverage, request documentation on incident-response and infection/pest-control protocols, clarify billing and consent processes, and seek references regarding dementia-care expertise if applicable. Doing so will help match the facility’s therapy strengths to individual needs while assessing operational consistency and accountability.

About Pilgrim Manor Skilled Nursing and Rehabilitation

Pilgrim Manor Skilled Nursing and Rehabilitation has been open since July 1, 1995, sitting at 1524 Doctors Dr in Bossier City, Louisiana, offering both short-term rehabilitation and long-term nursing care for adults who need help after illness, injury, or surgery, and for those who require ongoing support. The facility has 155 certified beds-36 of which were available as of June 2025-and accepts Medicare and Medicaid insurance. With skilled nurses and licensed professional staff on hand, residents can get 24/7 supervision and personalized care, whether they need assistance with daily activities like bathing, dressing, medication, or more involved care such as respiratory support, dementia care, or hospice services. Management comes from Priority Management Group, with James Almond as the Director, and Sdb Holdings with William Bauder, Douglas Boulware, and Steven Boulware as indirect owners. Average nurse hours per resident per day is 3.29, and the facility has a nurse turnover rate of about 61.9%.

The place operates as a for-profit limited liability company, with no recent change in ownership, and is organized within a Continuing Care Retirement Community, which means residents can move between levels of care as needed. Residents find a comfortable environment, with furnished rooms that offer flat-screen TVs with cable, kitchenettes, air conditioning, Wi-Fi, and private or shared settings. There's a dining room with all-day and restaurant-style meal service, a professional chef, and options for dietary needs like diabetes and allergies. The kitchen follows food safety standards, and management sources food from approved suppliers.

Pilgrim Manor has documented some deficiencies, including not always meeting care quality, respiratory care, or food handling requirements, so families should look into recent inspection results if concerned about health and safety standards. Still, the staff provides rehabilitation services, including outpatient therapy and occupational therapy, using a fully equipped area for therapy and exercise. Residents can enjoy game rooms, a movie theater, arts and crafts, a library, outdoor paths and gardens, and relaxed spaces for social time, along with a beauty shop, spa, and family support groups. There's laundry service, housekeeping, transportation, and organized activities, plus emergency alert systems in rooms.

The facility creates custom care plans with the help of expert nurses and therapists, focused on keeping each resident safe and comfortable, whether for a short stay or longer care. Respite care and palliative support are also available when families need extra help. With a focus on compassionate treatment and a resident council that brings up concerns, Pilgrim Manor tries to keep care personal and respectful. While the BBB gives it an A+ rating, some care issues have happened, so anyone looking at Pilgrim Manor should look at up-to-date reports to decide if this community fits their needs.
