    Clean, welcoming facility; attentive staff

I had a very positive experience. The facility is clean, well-maintained and smells fresh, with a welcoming atmosphere and secure, efficient check-in. Staff were consistently friendly, attentive and helpful-from front desk to nurses and rehab-and my mom settled in quickly, enjoyed meals and activities, and felt well cared for.

    Summary of reviews

The reviews for Town & Country Health & Rehab show a pronounced split between strong, resident-centered experiences and serious operational concerns. Many families praised compassionate, attentive nursing assistants and nurses, named individual staff members who provided exemplary communication, and highlighted an effective rehabilitation program. The facility's front desk procedures, secure screening, well-kept rooms, and an attractive, homelike interior were frequently described as strengths, and multiple reviewers noted engaging activities and special events that support socialization.

Clinical care is characterized by two contrasting patterns. On one hand, therapists and CNAs receive repeated positive mention for responsiveness, regular room checks, and successful rehabilitation outcomes. On the other hand, there are persistent concerns about medication management and timely symptom control, including delays in delivering pain medication. There are also isolated, serious allegations regarding medication administration and post-acute clinical follow-up, including an allegation of a lifeโ€‘threatening infection; these accounts point to the need for careful clinical oversight and clear escalation pathways.

Dining and dietary services are similarly mixed. Several families described enjoyable event meals and a generally pleasant dining atmosphere, while others raised issues with meal temperature and inconsistent meal-service practices. Some reviewers experienced limitations in dietary accommodations, with concerns that cost constraints affected the ability to meet specific nutritional needs. Activity programming is a clear strength overall: the facility runs a range of social, spiritual, and recreational offerings that many residents find engaging and beneficial.

Facility appearance, infection-control screening, and environmental services receive frequent positive feedback: many reviewers described the building as clean, comfortable, and well maintained, and maintenance staff were often singled out for helpfulness. At the same time, a subset of reviews documented sanitation and incontinence-care concerns in particular wings, indicating uneven performance across units or shifts.

Management and communication emerge as areas with notable variability. Several families commended individualized, family-focused staff and smooth admissions interactions, yet others described abrupt policy changes, inconsistent enforcement of visitation rules, difficulties obtaining explanations about specific incidents, and dissatisfaction with how complaints or discharge decisions were handled. Reports of inconsistent professionalism, occasional dismissive attitudes, and workplace interpersonal problems suggest variability in staff conduct that can affect family confidence.

In summary, Town & Country displays several operational strengthsโ€”compassionate caregivers, strong therapy services, a clean and secure campus, and active programmingโ€”alongside recurring operational weaknesses that include inconsistencies in clinical practice, medication handling, sanitation in some areas, dietary accommodation, and communication. Prospective families should weigh the facilityโ€™s therapeutic and social offerings against the need to confirm clinical and operational consistency: ask about staffing patterns, medication-administration protocols, dining accommodations, unit-level cleanliness audits, and the facilityโ€™s process for handling clinical concerns and family complaints before making placement decisions.
