    Warm caring therapy community cat

I'm a returning resident and truly thankful - the staff deliver supreme, caring support and the therapy department is amazing. The place is warm and welcoming (not the fanciest), and Sadie the security cat greets me at the door and parking lot and even acts like porch-therapy - everyone loves her. I feel safe, cared for, and glad to live here.

    Summary of reviews

Reviewers convey a mixed picture of care at Ringgold Nurse Care Center. The facility's therapy department and CNAs receive consistent praise: families note strong rehabilitation programming, attentive therapy staff, and compassionate hands-on care from certified nursing assistants. Several accounts describe a welcoming, home-like atmosphere augmented by an animal presence (a facility cat) that visitors and residents find comforting. The center's location and accessibility for family visits is also viewed positively.

Clinical-safety and care-continuity issues emerge as a key concern. Multiple comments point to medication administration errors and lapses in timely assistance following resident incidents, which suggest gaps in medication-management controls and fall-response procedures. There are also mentions of questionable assessment and documentation practices. These safety-related themes indicate the need for clearer clinical protocols, stronger incident-response procedures, and closer oversight of medication administration.

Staff culture and leadership are frequently highlighted as areas needing improvement. Reviewers describe problems with teamwork, staff gossip, inaction by some supervisors, and instances of insensitive or discriminatory conduct, which together contribute to a perception of weak professionalism. Many attribute staffing instability to compensation and workplace-culture issues; high turnover is noted as a factor that likely undermines continuity of care and institutional knowledge.

Dining and amenities are described as functional but inconsistent: food and meal service quality were criticized by several families, and the facility is not positioned as an upscale environment. Activity offerings beyond therapy are less frequently described, though the animal presence and the therapy program provide meaningful engagement for some residents.

Taken together, the pattern is one of strong rehabilitation capability and committed direct-care staff (particularly CNAs and therapists) alongside operational weaknesses in leadership, staffing stability, clinical-safety protocols, and dining/service consistency. Prospective families should weigh the facility's rehabilitation strengths and accessible, home-like atmosphere against reported concerns about clinical controls, staff culture, and management oversight. For current residents and families, focusing on clarifying medication and incident-response protocols with management, and confirming staffing patterns during visits, may help address the most critical operational risks noted in these reviews.

    About Ringgold Nurse Care Center

Ringgold Nurse Care Center sits on Kenneth Street in Ringgold, Louisiana, and folks tend to talk about the place when they're looking for senior living that covers quite a few bases, since the facility's got 112 certified beds and usually cares for about 91 residents a day, and the building's a one-story setup where people can pick a private or semi-private room, each with space for personal things, cable, and a phone line to stay in touch. The center provides a lot of levels of care-nursing home care, assisted living, and memory care-so whether someone needs a little help with dressing, bathing, or medications, or needs round-the-clock skilled nursing, this place tries to cover that, and they've also got a Special Care Unit with a 40-bed capacity that's designed for people diagnosed with mental illness or behavior disorders, with 24-hour medical and psychiatric care, regular medication checks, and staff trained for complex needs, plus a secure courtyard attached for fresh air and safety.

Ringgold Nurse Care Center gets inspected by federal agencies like CMS and is known to be affiliated with D&N, LLC as part of its for-profit structure, and when you look back through public records, the place has had at least two complaint reports in the years 2024 and 2025 and some deficiencies found in recent inspections, especially on resident rights and abuse prevention, so people keep a close eye on the rules and quality reports. The ownership's split, with D & N, LLC and DTD HC, LLC each holding half, and there are indirect owners listed in records as Donald T Denz Irrv Tr and Norbert A Bennett Irrv Tr Fbo Children and Grandchildren, so it's got some layers when it comes to who runs things.

The nurse turnover rate is 52.4%, which is higher than state average, and each resident gets about 3.51 nurse hours a day measured out, so sometimes folks look at those numbers to get a sense of staffing consistency. The therapy side's pretty extensive here, with physical, occupational, and speech therapy available five days a week, whether you need rehab after a surgery, care for stroke recovery, pain management, or training for balance and falls, plus they offer orthopedic, neurological, and pediatric therapies, as well as help for conditions like Parkinson's, MS, and cerebral palsy. There's outpatient services too, for when someone doesn't need to stay overnight.

Rehab Suites are set up for short-term and long-term stays, focusing on complex cases post-hospital, like orthopedic injuries, cardiac illnesses, pulmonary issues, using modern therapy tools and plans made for each person, and a big part of the program's working on recovery from surgeries like hip or knee replacements, and a lot of folks use the gym, activity room, and outdoor courtyard while they're here. The staff takes on wound care, IV therapy, contracture management, urinary incontinence care, and managing tubes like tracheostomies, so people facing medical challenges usually get an individualized plan.

Meals on Wheels runs through the center, and there's a big list of support services-caregiver respite, personal care, homemaker program, recreation like singing or themed events throughout the week, and outings for holidays or special days, plus a beauty shop, housekeeping, laundry, and wheelchair-accessible transportation, so day-to-day life stays busy. They try to keep residents' rights in focus, too, with an Ombudsman program that's part of their process, and people can look at facility history, mission statements, council and board lists, and sometimes join the mailing list or volunteer.

Ringgold Nurse Care Center is listed as a nursing home in inspectable databases and directory categories, so you can look up their inspection results and see the past reports online, and if someone wants to see the place, there are tour options to meet the staff, see the facilities, and decide if it's a good fit. Even though there have been notes about deficiencies and turnover, the team keeps working on adapting care plans, offering therapies, and supporting people with medical and personal needs in a setting that feels homey but also structured for the level of help that's required.
