    Beautiful clean facility, excellent care

I visited this beautiful, well-kept facility and was very pleased. The grounds and rooms are clean and welcoming, staff (Dawn, Tiffany and Ashley stood out) are warm, professional and proactive - friendly sign-in, snacks, quick responses and attentive nurses who treat residents with dignity. Rehab/physical therapy was excellent, activities are varied, and the atmosphere felt home-like and secure. I'd recommend it for rehab or longer stays and plan to visit again.

    Summary of reviews

Reviews of Southern Hills Healthcare and Rehabilitation show a strongly polarized pattern: many families and visitors describe compassionate direct-care staff, effective rehabilitation services, and a pleasant facility environment, while a subset of accounts raises significant operational concerns that merit careful inquiry.

Care and staff: Numerous reviewers praised individual nurses, CNAs, and therapists as kind, attentive, and effective — particularly the physical-therapy program, which several families singled out as delivering measurable improvement. Admissions and social-services staff were also frequently described as helpful in care planning and transition. Conversely, other accounts describe inconsistent staffing levels, slow response times to call lights or after falls, and lapses in medication administration and monitoring. These divergent experiences point to a facility where frontline commitment is evident for many residents but where staffing consistency and clinical process controls appear uneven.

Dining and activities: The facility receives credit for an active, home-like activities program and occasional positive comments about dining. Some families appreciated the ability to bring in outside food and the facility’s accommodation of visitors and pets. At the same time, reviewers described variability in meal accuracy and dietary accommodations, indicating that food-service consistency can be unpredictable for residents with specific diet needs.

Facilities and maintenance: Many reviewers noted a clean, well-kept building, attractive grounds, and comfortable single rooms. However, there are recurring concerns about sanitation practices and maintenance: inconsistent housekeeping in some areas, isolated pest-control concerns, and equipment or fixture issues that suggest maintenance workflows may be uneven. Safety and supervision of residents with behavioral or cognitive challenges were also raised as a concern in several accounts.

Management and communication: Several reviews compliment leadership for warmth and professionalism, while others describe gaps in administrative oversight — particularly during shift changes and weekends — and frustrating communication with families. Multiple accounts referenced formal complaints or intent to file complaints; a small number of allegations of serious staff conduct have also been reported and have prompted family action. Overall, the pattern suggests variability in management follow-through and in staff training or culture across shifts.

Notable patterns and guidance: The body of feedback indicates two contrasting experiences at Southern Hills: many families experience attentive, rehabilitative, and home-like care, while others encounter lapses tied to staffing, medication processes, sanitation, and incident response. Prospective residents and families would be well served to tour the facility, ask about current staffing ratios and weekend/shift supervision, review medication-administration policies and incident-reporting procedures, inquire about housekeeping and pest-control routines, and observe mealtime and activity periods to assess consistency. These steps can help clarify whether the facility’s strengths align with an individual resident’s clinical and safety needs.
