    Compassionate attentive team providing rehabilitation

I was very pleased with Autumn Lakes - a wonderful head nurse and a compassionate, attentive team providing 24/7 care. My loved one was well looked after: clean, groomed, comfortable room, personalized rehab and frequent PT helped recovery. Admission was smooth, the social worker and front desk were welcoming, and activities (Bebe) kept residents engaged. Staff answered questions promptly and treated us with warmth and professionalism; I would recommend this community for rehab and long-term care.

    Summary of reviews

Autumn Lake Healthcare at Silver Spring elicits a strongly mixed set of impressions. Many families and patients praised the facility’s rehabilitation services, noting frequent, effective physical and occupational therapy that produced measurable progress for some residents. The activities department — frequently singled out by name — is widely regarded as a strength: programs are creative, socially engaging, and perceived as beneficial to resident morale. Several reviewers also highlighted dedicated social-work support and individual staff members who provided clear communication, personalized attention, and helped families navigate care transitions.

At the same time, persistent operational issues are evident across multiple accounts. Staffing levels and consistency were continual concerns: reviewers described delays in nurse response, long waits for basic assistance, and high staff workload. Those staffing constraints appear to be associated with delays in medication timing, uneven attention to incontinence care, and slower-than-expected responses to call lights. Communication gaps between clinical staff, administrative offices, and families emerged repeatedly, including missed phone calls, inconsistent reporting of vital signs, and variable follow-up after clinical events.

Sanitation and facilities maintenance are recurrent themes. Several accounts referred to uneven housekeeping performance, odor concerns in some areas, pest-control issues, and aging physical plant elements that would benefit from refresh and repair. Dining quality and meal-service consistency were also variable: some families praised the food and weight gain, while others described marginal meal quality and limited therapeutic or diabetic meal options. Supply-chain issues — including shortages of pull-ups, blankets, or pressure-relief equipment — were reported as operational weaknesses that affect resident comfort and safety.

Safety, security, and conduct concerns appear intermittently but are notable. Reports include allegations of missing personal items and at least one serious staff-related safety claim; these raise questions about property-control procedures and incident reporting. Several reviewers raised concerns about transfer and mobility assistance practices and about the overall reliability of clinical documentation and handoffs. Conversely, other families described the staff as caring, professional, and instrumental in successful recoveries, creating a clear division between positive care experiences and critical operational failures.

Management impressions are mixed: some families described proactive, communicative leadership that resolved issues quickly, while others experienced ineffective follow-up and inconsistent enforcement of policies. For prospective residents and families, the pattern suggests that individual experiences can vary substantially depending on unit, shift, and staffing. Important pre-admission questions include current staffing ratios, infection-control and housekeeping protocols, medication administration timing, pest-control measures, property-theft prevention practices, and the facility’s plan for addressing maintenance needs. A focused, in-person tour and direct discussions with therapy, nursing leadership, and social work are advisable to assess how current operations align with a prospective resident’s clinical and comfort needs.

About Autumn Lake Healthcare at Silver Spring

Autumn Lake Healthcare at Silver Spring sits on quiet and lush, parklike grounds, where the air feels peaceful and the setting encourages people to rest and recover. The facility runs both short-term and long-term care, so some stay for rehabilitation like after surgery or illness while others call it home for much longer. Board-certified doctors, nurses, and therapists watch over everyone, and the staff really makes you feel like you matter, because you see them trying hard to keep things warm and friendly every day, helping each resident stay as active as they can with things like activities and social events. They have speech, occupational, and physical therapy, plus care for people who need trach management, IV therapy, or special respiratory help, and there's a closed dementia unit with its own trained team for people who'd do better with extra support and memory care.

People who are facing tough or complex medical issues can get sub-acute rehab or around-the-clock skilled nursing there, and the team pulls together using hands-on rehab to help people regain strength or independence, working from plans that adjust as someone gets better or if needs change. The building itself feels up to date and even kind of fancy, and they make sure people can easily move around, since everything is ADA accessible for those with disabilities, and there are places both indoors and outside to gather, sit, or join in with others. Memory care and hospice services are available, and the atmosphere feels like it's meant to help everyone stay calm and relaxed, whether they're spending time in the gym with top equipment or just sitting outside among the trees and greenery.

The nursing care, memory support, long-term options, and hospice care are all licensed services, provided by Autumn Lake Healthcare at Silver Spring alone, since the center is independently owned and part of a wider group of centers but acts on its own. There are no age limits for residents or visitors either. That all means people get advanced medical attention with some luxury touches, and that's why the center's earned a reputation for good nursing and quality rehab care in a truly peaceful setting.
