    Caring staff, but unsafe, unclean

    I appreciated the caring, attentive nurses and aides, excellent PT/OT, engaging activities, good food, and veteran involvement - staff often felt responsive and well-staffed. However, I experienced serious safety and cleanliness problems: foul odors, rooms not as advertised, broken toilets, unsupervised residents, and an incident that led to an ER return - care felt inconsistent and sometimes negligent. Because of those safety/maintenance issues, I can't fully recommend this facility unless they fix them.
