    Warm staff, effective rehab, recommended

I'm very pleased overall - the staff were warm, attentive and compassionate, admissions and management were responsive, the building was clean and comfortable, and rehab/therapy (PT) helped my mom recover and return home. Meals, activities and family support were excellent and made visits easy. My only caveats were occasional staffing/communication inconsistencies (slow night response, rehab/discharge info and billing follow-up) and a few dated rooms/small TVs, but I would still recommend this facility.

    About The Oaks

The Oaks is a senior care facility run by Life Care Centers of America, and it's got skilled nursing, rehabilitation, Alzheimer's care, and regular senior living services, so folks can get long-term or short-term help depending on what they need, and the place uses a team-based approach, which means case managers work with people in their homes, at the hospital, during rehab, and then when they're back home again, offering in-home private geriatric case management services and helping with healthcare decisions. People living there have support from nurses, doctors, and therapists around the clock, and there's a focus on physical therapy, occupational therapy, and speech therapy for things like moving better or swallowing, plus they've got some specific programs like end of life care, falls prevention, and there's even a Care Transitions program to help people move from hospital to home. The Oaks offers amenities you might not expect, like cafes, coffeehouses, bubble tea stores, tea rooms, and cupcake bakeries, plus a mix of restaurants-Asian, Southern, Soul Food, Vietnamese, and more-so residents and their families have plenty of dining options. For fun and everyday needs, there are shops, a library, hairdresser, and beauty salon, and you can even find amenities like an ice cream shop, bright common areas, landscaped courtyards, and pet visits, while regular activities are planned by a full-time activity director and there are religious services for spiritual needs. Rooms can be private or semi-private with their own bathrooms, and everyone gets flat-screen TVs, phone, and cable hookups, as well as internet access, while the grounds stay safe with 24-hour security and fire systems. The staff covers housekeeping, laundry, and meals with a fine dining program, plus there's transportation, a family council, and both inpatient and outpatient rehab services, including state-of-the-art equipment like the VitalStim system for swallowing, and Closed Pulse Irrigation wound care. People get individualized care plans that cover daily help, medical needs, medication, IV therapy, infection control, and recovery after surgery, all in a bright and homelike setting where they've got 24-hour skilled nursing, a certified dietitian, wellness programs, and ongoing advocacy for caregivers. The team tries to improve safety with things like the Fight the Flu Campaign, Sepsis Smart, care transition support, and efforts to reduce off-label antipsychotic use, and there's plenty of focus on education for both residents and their families. Families can participate in councils, and ombudsman services are available for extra support, so between the rehab, elder care, amenities, and programs, The Oaks aims to give folks a comfortable place to recover or live while making sure they have the help and resources they need.
