    Filthy facility, neglectful staff, unsafe

    I was forced to place my loved one here in an emergency and it's been a nightmare. The facility is dirty (roach and strong urine odor), residents have matted hair and stained clothes, towels torn, and rooms smelled awful. Staff are avoidant, rude and unengaged - no eye contact during care, phones at the desk, frequent agency nurses, and often no one when help is needed; I've seen missed baths, suspected bruises, dehydration risks and bedsores. Management was hostile and unresponsive when I complained; I reported it to DPH. Do not send your family here - this place should be shut down immediately.
