    Compassionate staff, dangerous management problems

I appreciated many kind, helpful nurses and CNAs, strong therapy, a clean, welcoming facility with good activities and roomy accommodations. Unfortunately, after a change in ownership care deteriorated - chronic understaffing, poor supervision, medication errors, late or removed meals, outbreaks, ignored calls, and my loved one declined and died alone without family being contacted. It's a mix of compassionate staff and dangerous management problems; I cannot recommend it until leadership fixes safety, staffing, and accountability.

    Summary review

Overall sentiment for AdviniaCare Wilmington is highly polarized, with reviews ranging from glowing endorsements to severe allegations of neglect and unsafe conditions. Multiple reviewers praise the facility for exceptional therapy, compassionate caregivers, clean and attractive common areas, and a welcoming, hotel-like atmosphere. These positive accounts emphasize friendly, attentive staff who learn residentss names, effective and encouraging rehabilitation services, a variety of activities (bingo, bowling, arts and crafts, music), accommodating meal services for special diets, and positive experiences with admission and care coordination.

Contrasting sharply with those positive reports are numerous, detailed negative accounts describing systemic problems. The most prominent concerns center on staffing and supervision: reviewers report chronic understaffing, high staff turnover, and CNAs who are inattentive (on phones or observed napping). These staffing issues are tied in multiple reviews to delayed or missed care — hour-long waits for assistance, delayed cough medicine, delays in medication administration, and meals served late then removed. Several reviewers specifically allege serious medication management failures, including incorrect medications being given without authorization and medications left on the floor.

Safety and infection control are recurring and serious themes among the negative reviews. There are multiple mentions of scabies outbreaks and unexplained rashes, along with hygiene problems such as smells of urine and feces and dirty floors. Several accounts claim residents were left unattended during transfers, exposed to choking risks, or otherwise neglected, and some families report deaths that they characterize as occurring alone or with inadequate communication from the facility. Photographic evidence and direct allegations of these problems are reported by reviewers, which intensifies their concerns.

The reviews also show a split in perceptions of management and organizational culture. Some reviewers attribute a decline in care quality to a post-takeover change in ownership and leadership, alleging a shift toward profit-driven decisions, cost-cutting measures, and hiring cheaper labor. These reviewers report that care ‘‘under former owners was above reproach but that it deteriorated afterward. Others, however, praise the administration and executive staff as kind, responsive, and supportive. Communication with families is another divided area: while some describe prompt, attentive communication and weekly menu reviews, others complain about unanswered calls and emails and a lack of notification when care deteriorates.

Facilities and amenities receive mixed but generally positive remarks from many reviewers: the building and common areas are often described as clean, modern, and comfortable with welcoming features (fireplace, piano, bright rooms), and several families specifically note tasteful décor and family-friendly visiting including pets. Therapy spaces and rehabilitation services receive frequent praise as "rehab-first" with demonstrable progress for residents. At the same time, some reviewers say memory care areas feel older and note backlog issues with services such as the hair salon.

Dining and activities present another split: several reviewers commend the dining experience, special-diet accommodations, and active engagement programming, while others describe late or removed meals, poor food quality (cold meals, sparse sides), and a lack of dining-room supervision. These divergent reports suggest variability across shifts, units, or time periods.

Taken together, the review set paints a facility with significant strengths — notably rehabilitative therapy, many compassionate and engaged staff members, a pleasant physical environment, and diverse activities — but also serious, recurring concerns about staffing, supervision, infection control, medication safety, and inconsistent management. The pattern is one of high variability in resident experiences: some families report excellent, respectful care and positive outcomes, while others report neglect, safety incidents, and deeply troubling lapses. Families considering or monitoring care at this facility should be aware of this polarization, seek up-to-date information about current staffing levels, infection-control practices, medication administration procedures, and leadership stability, and consider speaking directly with current families and reviewing recent inspection or incident reports to assess whether the specific issues raised in negative reviews have been addressed.
