    Therapy strong, clinical care unsafe

I had a mixed experience. The therapy teams and many nurses/aides were exceptional - kind, motivating, and instrumental in good rehab progress - and the facility is clean and pleasant at times. But staffing and clinical consistency were alarming: missed meds and vitals (including an unchecked blood sugar of 468), slow or rough responses, poor communication from administration, room moves and refund delays, and even an emergency that resulted in my mother's death. Food, activities and cleanliness were hit-or-miss. I'd consider this place for short-term rehab if family can be present and vigilant, but I would not trust it with a medically fragile loved one without constant oversight.

You've got options to set up tours to meet the staff and see what the place is like before making any decisions. The care team stores all medicines in locked compartments and follows strict guidelines for labeling drugs, so safety standards are clear. This place handles post-op physical and occupational therapy, plus offers support for end-of-life care and care transitions between different levels of service, which can mean a lot when health changes. The aides have a reputation for being attentive and kind, and families have noticed that the staff really does its best to support both the people living there and their families, allowing visitors and encouraging family presence. The environment's geared toward residents' safety, comfort, and quality of life, and you'll find amenities for daily living and programs made for each person's own health needs. CareOne at Wilmington runs as a for-profit limited liability company and is affiliated with CareOne, which is known for its experience in clinical practice and management, and it's received high marks-like a CMS 5-star rating and a track record that shows JC Accreditation plus several Deficiency Free Surveys from the Department of Public Health in 2014, 2015, 2019, and 2022-so people do tend to see it as a premier healthcare community while still getting that important day-to-day support.
