    Family-like care and excellent rehab

I placed my mom here and I'm very pleased - the staff truly treat residents like family and always keep me updated. Rehab (PT/OT), skilled nursing and on-site services are excellent, admissions was a breeze, and transportation is convenient. The facility is clean, meals are good, and there are plenty of activities to keep her engaged. I would recommend Courtney Manor and would choose them again.

    Summary of reviews

Courtney Manor Nursing & Rehabilitation is consistently described as a facility with a strong, family-oriented caregiving culture and a particularly well-regarded rehabilitation program. Reviewers frequently praise attentive CNAs and nurses, hands-on physical and occupational therapists, and staff who provide regular updates and personalized attention. Multiple accounts emphasize successful post-operative recovery and return-home outcomes, and specific departments (admissions, dietary, housekeeping, PT/OT) receive repeated positive mentions.

Clinical care and staffing are clear strengths: reviewers highlight compassionate interactions, timely assistance from nursing aides, and skilled therapy staff who drive rehabilitation progress. Staff members are also credited with good communication and coordination of on-site services such as x-rays and skilled nursing. At the same time, there are operational concerns related to continuity of care after discharge and occasional breakdowns in follow-up communication; prospective families should confirm the facility’s discharge-planning and aftercare protocols before placement. There is at least one allegation regarding management empathy and staff conduct following a resident death; families may wish to ask facility leadership about staffing oversight and complaint-resolution procedures.

Dining, housekeeping, and amenities are commonly described positively. Meals are characterized as varied and accommodating (including diabetic meal options), rooms are kept clean, and the facility offers added services such as hair and nail care that contribute to a spa-like atmosphere. Housekeeping and maintenance are cited as contributing to a pleasant environment, and many reviewers mention scenic views and a homelike feel.

Activity programming and resident engagement are noted as generally energetic and meaningful, with daily programs and cognitive activities available. Reviewers also note that some activities were curtailed during infection-control periods; this appears to reflect temporary operational restrictions rather than a lack of programming intent. Families should ask about current activity schedules and infection-control policies to understand resident daily life at the time of placement.

Facility infrastructure and administrative controls present the clearest operational risks mentioned. A safety inspection identified deficiencies in sprinkler and smoke-alarm installation and noted the absence of a documented backup emergency plan; these are facility-level issues that bear direct inquiry and verification of remediation. Separately, there are recurring remarks about occasional lapses in responsiveness (missed callbacks) and instances of persistent unsolicited outreach or contact-data errors, suggesting opportunities to tighten communication workflows and data-handling practices.

In summary, Courtney Manor appears to deliver strong rehabilitative care, attentive front-line staff, and a clean, comfortable environment that many families describe as family-like. However, prospective residents and their families should verify current status on fire-safety and emergency-preparedness corrections, ask about discharge and aftercare coordination, and review the facility’s communication and privacy practices before placement. These targeted questions will help align expectations and address the primary operational concerns reflected in the reviews.

About Courtney Manor Nursing & Rehabilitation

Courtney Manor Nursing & Rehabilitation sits in Bad Axe, MI as a skilled nursing and rehab facility with 99 beds, and they do quite a bit for seniors needing 24-hour nursing or rehabilitation care and folks who have physical or mental health conditions, so when you walk in the place you've got registered staff and a care team who work steady hours each week-this includes licensed administrators, directors of nursing, physicians on call all day and night, nurses, certified nursing assistants, social workers, therapists, wound care team, and folks who cover special services like dental, vision, and psychiatry, plus a full dietary team led by a registered dietitian, and they keep family in the loop with both a resident council and a family council. The building itself is non-smoking, modern, and aims for a friendly, helpful atmosphere where the staff is known for being joyful and kind, making the environment feel welcoming, and folks who live there can choose between private and semi-private rooms, with in-room TVs, Wi-Fi, and visiting hours any time you feel like dropping by, and for those who want a little pampering there's a full-service salon and beautician and barber services right on site. Meals are a real focus here, and they go for quality in the kitchen with restaurant-style dining, nutritious foods, and sometimes a bit of fun like hand-dipped ice cream, fountain pop, and happy hour every weekday, so meals are sociable and the dining areas are pleasant, with 24-hour room service if you want to eat in your room.

They don't just deliver standard care but cover a wide range of services that include skilled nursing, respite care, hospice, inpatient rehab, and personal care for a variety of needs, whether you're there for short-term recovery or long-term support, and they're part of Ciena Healthcare. The therapy team works six days a week providing physical, occupational, and speech therapy, and for those with special medical needs they offer memory care, wound care (including Wound VAC), dialysis support, tracheostomy care, tube feeding, sepsis management, substance abuse management, IV therapy, total parenteral nutrition, and pain relief, and all this is supported by their own pharmacy, plus on-site lab and X-ray services. They've got transportation and medical transport if you need to get to appointments and have access to telemedicine, and they're good about discharge planning and helping arrange for the next step if you're only staying for rehab. For the daily basics, the nursing staff helps with personal care and activities of daily living, with restorative nursing programs so folks can keep as much independence as possible.

They accept both Medicaid and Medicare, so there's a wider range of folks who can use their care, and they have special programs for dementia and cognitive deficit management, as well as recreational therapy and lots of activities aimed at keeping residents social, active, and engaged, and they've even gotten awards for their activity programs. Awards from the AHCA-Bronze and Silver-and the HCAM "Facility of Excellence" say that the place is doing quite a few things right, with a facility inspection rating of B+ that means things run well and the standards are being met. Families can use a private lounge for gatherings, and everyone gets access to social services if something extra comes up. The campus is comfy, there's a sense of hospitality, and the staff tries to build real relationships with the people who live there, and while it's a for-profit community run individually, they've carved out a place that runs steady, offers a lot under one roof, and tries to be a good choice for both short stays and long stays, with a real focus on kindness and care.
