    Clean caring comforting well-run facility

I'm very pleased with this small, very clean facility - the staff are caring, attentive to detail, and make residents feel valued. I also appreciate the regular church services and warm fellowship; overall a comforting, well-run place I'd recommend.

    Summary of reviews

Overall impression: Reviews show a mix of strengths and serious operational weaknesses. Positive comments highlight a small, personable setting with attentive caregivers, clean spaces, and onsite spiritual activities. At the same time, multiple reviews describe recurring gaps in clinical responsiveness, safety, and administrative systems that prospective residents and families should weigh carefully.

Care quality and clinical monitoring: Several reviewers described delays in staff response to call lights and extended waits for basic care, which contributed to concerns about timely monitoring and intervention. There are multiple accounts of infections requiring hospitalization, pressure-related skin issues, and at least one fall with head injury; these items suggest gaps in routine clinical surveillance, infection-prevention practices, and wound/pressure-injury prevention. Some families also noted a lack of consistent physician rounds or regular clinical oversight.

Staff and conduct: Many reviews praise individual caregivers as compassionate and detail-oriented, and the facility's small size is often cited as supporting personalized attention. However, other comments indicate inconsistent staff performance, with concerns about delegation and medication-handling practices. There are mentions of staff roles and responsibilities not being consistently followed and of inadequate dementia-focused skills among some team members, indicating a need for targeted training and clearer accountability structures.

Dining and nutrition: Reports include meal errors such as incorrect diets being served and unresolved dietary issues. These point to weaknesses in meal verification and communication between dietary and nursing staff. Families considering this facility should confirm how dietary needs, special textures, and allergy/health restrictions are documented and checked at mealtimes.

Facilities and maintenance: The physical environment is described as clean by some reviewers but also as dated in furnishings and dรฉcor by others. Specific maintenance concerns include mattress surface integrity and moisture control, bed hardware problems, and equipment height/fit issues that can affect resident comfort and safety. Sanitation concerns in certain areas and inconsistent bathing schedules were noted, which raises questions about routine housekeeping and personal-care protocols.

Management and systems: Multiple comments point to outdated clinical documentation and medication-passing (MedPass) processes, along with perceived limited managerial oversight and slow resolution of family-raised issues. These systemic weaknesses align with the operational patterns above (delayed responsiveness, medication controls, and dietary errors) and suggest opportunities for improvements in training, supervision, and administrative modernization.

Notable patterns and considerations: The facility appears capable of providing attentive, compassionate care in many cases, particularly for residents who benefit from a smaller setting and available spiritual activities. However, recurring themes of delayed response times, medication and dietary inconsistencies, maintenance and mattress concerns, and insufficient dementia training suggest uneven performance across core operational areas. Prospective residents and family members should consider asking for recent audit results, infection-control protocols, staffing levels and turnover, dementia-care training plans, and examples of corrective actions taken for any prior incidents before making a placement decision.

    About Legendary Nursing & Rehabilitation

Legendary Nursing & Rehabilitation sits in Marshall, Missouri, and offers skilled nursing services and premier rehabilitation programs with care available around the clock, so if someone needs help at any hour, someone's always there. The staff uses an interdisciplinary approach, meaning nurses, aides, therapists, and other support work together to help each resident reach their best level of independence and comfort, and nobody's left without attention to their personal needs or health goals. They accept Medicaid and Medicare, making payment options a bit wider for many families. Residents get support that can include help with bathing, dressing, managing medicine, and more complex care if needed, and anyone who can't move easily gets help with transfers and walking, so moving around is safer and more comfortable. There are long-term care choices, respite support for regular caregivers, and even specialized services such as infusion therapy, hospice care, palliative care, and post-surgical care for those who need constant or end-of-life attention.

Physical, occupational, outpatient, and speech therapy are all there, designed to help people recover skills or keep their abilities as strong as possible, and daily medical checks help catch health concerns early. For everyday living, the rooms come furnished, and there's Wi-Fi, cable TV, private bathrooms, kitchenettes, and air conditioning, so things feel more comfortable and home-like. The activities and amenities spread out across a game room, activity room, arts spaces, a music program, outdoor common areas, and even a movie theater and a fitness room, so there's usually something to do alone or in groups. People enjoy all-day restaurant-style dining with meal prep, special diet options, housekeeping, laundry, and dry cleaning, and if families need support or information, staff's there to help educate them about care and services.

There's a concierge service, a community library to read or relax in, and scheduled activities sponsored by the community, plus resident-run groups and outdoor programs. Residents needing help get 24-hour supervision, an emergency alert system, and a 24-hour call system in case something happens, so someone's close by to assist. Transportation and parking services mean getting to appointments or visits is easier, and move-in coordination helps new residents settle in without trouble. With attention to each resident's health, safety, and goals, Legendary Nursing & Rehabilitation gives a wide range of care, making the daily routines smoother for residents and families.
