    Warm staff and homelike atmosphere

I had a very pleasant stay - the staff were warm, caring, and professional, with experienced, informed nursing and loving, involved management. Meals felt homemade and activities were enjoyable (bingo with nice prizes). The small rural setting is quiet and comforting; it was the best place for my mom - she didn't want to leave. I would consider this facility for a loved one.

    Summary of reviews

Reviews convey a mixed picture of care quality. Several long-term accounts describe attentive, professional nursing and caregiving over extended stays, while other accounts describe a recent downturn tied to staffing shortfalls. Reviewers linked staffing inconsistencies to delays in care, increased hospital transfers, falls, and other serious clinical outcomes; there were also explicit concerns following a resident's death. Overall, care quality appears uneven with meaningful differences between periods of stable staffing and periods when personnel levels were reported as inadequate.

Staffing and staffing-related issues are a central theme. Positive comments emphasize warm, caring, and experienced nurses and aides who create a pleasant, supportive environment. Conversely, other comments identify inconsistent staffing, communication breakdowns when families attempt to reach staff by phone, and concerns about staff conduct and equitable treatment of residents. These patterns suggest that day-to-day resident experience is highly dependent on current staffing and supervisory practices.

Dining and activities receive both praise and criticism. Several reviewers praised homemade-style meals and a comforting, small-facility dining environment. At the same time, others raised concerns about meal continuity and nutrition oversight. The activity program is generally described as active and engaging, with organized events such as bingo; however, there are also comments about uneven oversight, questionable programming choices, and dissatisfaction with incentives and event management.

Facility condition and infection-controlโ€“adjacent matters are also raised. Positive descriptions highlight the facility's rural, quiet character and a close-knit atmosphere. Negative observations include sanitation and pest-control concerns and issues with waste management and cleanliness in some areas. These facility-level concerns are operational in nature and point to a need for improved environmental-maintenance and pest-control protocols.

Management impressions are mixed. Some reviewers characterized leadership as caring and accessible, and noted hands-on involvement in resident life. Others described barriers to effective communication, difficulty escalating concerns, formal complaints, and mention of potential media attention. Taken together, the pattern indicates variability in management responsiveness and in how concerns are addressed.

Notable patterns for prospective residents and families: the facility can offer compassionate, personalized care in a small, rural setting and has strengths in nursing experience and activity programming. However, there are recurring operational weaknesses โ€” particularly inconsistent staffing, gaps in communication, fall-prevention and safety practices, inconsistent meal and activity oversight, and sanitation/pest-control issues โ€” that should be evaluated during tours and conversations with leadership. Families considering this facility should ask about current staffing ratios, fall-prevention protocols, communication procedures for families, recent inspection outcomes or corrective actions related to environmental concerns, and how the facility addresses complaints and escalation.

About Chariton Park Health Care Center

Chariton Park Health Care Center sits in Salisbury, Missouri, and runs under Reliant Care Management Company, LLC, offering care since December 1994, holding 120 certified beds with about 113 residents on most days, focusing on skilled nursing, memory care, assisted living, and independent living for the elderly or people over 18 with mental illness and medical needs, while also helping folks with dementia. The place offers long-term care, short-term care like respite and rehabilitation, hospice services, wound care, medication help, insulin injections, dialysis support, and post-surgical care, plus professional nursing care around the clock, even though the staffing ratio is 1.65 nurse hours per resident per day, lower than the state average of 3.4, but nurse turnover is at 45.2%, better than the state's average of 57.9%. Residents can find social services, a barber and beauty shop, transportation, therapy services like physical, restorative, and speech therapy, special diets, and activities, with insurance resources available for Medicare, Medicaid, private pay, and hospice arrangements. The facility's approach centers on staff working as a team, treating every resident with dignity and compassion, and open communication for families and employees, describing itself as an equal opportunity employer. However, inspection reports in the past 36 months recorded 59 deficiencies, two of them infection-related, and included concerns like safety hazards (F0689) and care planning (F0658) with the risk for more than minimal harm, meaning there have been some issues with supervision and assessment. Chariton Park Health Care Center stays licensed and meets health and safety checks, with programs and amenities aimed at making life comfortable, while they provide information about services and insurance on their Medicare page, so families will find resources to help choose the care their loved one might need.
