    Neglectful care, dirty, unsafe facility

My mom lived here and my experience was very disappointing. Staff were frequently unavailable - phone calls went unanswered or rang forever, alarms/oxygen beeps ignored, and there was no clear person in charge. Cleanliness was poor: dirty trash cans, rusted beds, holes in linens, bugs and a strong odor. I witnessed neglectful care - residents left in urine/feces for hours, bed sores, a UTI that required hospital transfer, and inadequate bathroom help. Meals didn't match the menu and were unappealing; parking and the building showed visible neglect. A few staff were kind (Clematine stood out) and the new administrator seems to be making progress, but overall safety, hygiene, and care quality left me unwilling to recommend this facility.

    Summary review

Overall sentiment from the reviews is mixed-to-negative with recurring and serious concerns about cleanliness, safety, and consistent quality of care, offset by intermittent positive experiences tied to specific staff and recent administrative changes. Multiple reviewers describe the facility as old and neglected, citing concrete housekeeping failures such as dirty trash cans, unclean bedside tables, rough towels, holes in linens and furniture, rusted bed frames, strong odors (including urine), and even bugs in rooms. While a few reviewers reported that parts of the facility were very clean or that visiting areas were comfortable, the dominant theme is inadequate sanitation and infection-control concern; reviewers explicitly mentioned bacteria worries, bedsores, urinary tract infections, and transfers to hospitals following deterioration in condition.

Care quality and safety are central issues in the reviews. Numerous accounts describe delayed or non-existent responses to call bells, ignored alarms (including an oxygen machine beeping), and staff who are unavailable or overworked. Several reviews give specific examples of neglect: residents left in urine and feces (one report specified 3.5 hours), poor bathroom assistance that caused humiliation, bed sores, and reported falls that were not promptly reported or not properly assessed. Reviewers reported that these failures sometimes led to hospital transfers and formal complaints. Rehabilitation services also received criticism: some family members felt the facility misrepresented its PT/rehab capabilities and did not deliver the promised rehabilitative care.

Staff behavior and communication present a mixed picture. Many reviewers describe nurses and other staff as rude, unprofessional, or ignorant, with poor bedside manner and a lack of respect for patients. Phone communication problems were noted—long unanswered rings and calls not being transferred to rooms—creating additional stress for families trying to reach the facility. At the same time, several reviews singled out individual staff as positive influences (for example, a CNA named Clematine), and some reviewers explicitly called staff kind or praised the DON as knowledgeable. A recurring comment is that staff appear overworked and that there is no clear person in charge, although one review mentioned a new administrator who is making visible improvements.

Dining and amenities received criticism for inconsistency and low quality. Some reviews said meals initially looked above average but the actual served meals did not match the posted menu or expectations and were unappealing. Food quality was repeatedly described as awful by several reviewers. Facility infrastructure issues were also noted: parking lot disrepair with faded lines, lack of a covered loading area, old rooms, and worn furniture. These physical problems reinforce perceptions of neglect and contribute to overall dissatisfaction.

Management and responsiveness to concerns are mixed but lean toward inadequate oversight in the past, with a few signs of improvement. Several reviewers explicitly mentioned formal complaints and concerns about regulation or the facility’s open status, indicating families escalated issues. Conversely, a couple of reviews praised the new administrator’s progress, and some reviewers reported positive experiences with particular staff members and the DON. This pattern suggests variability in management effectiveness across time or among different shifts/staff teams.

In summary, the dominant themes across the reviews are significant and recurring problems with cleanliness, hygiene, and safety; inconsistent and sometimes neglectful caregiving; poor food service; and infrastructure neglect. These issues have led to adverse outcomes for some residents (infections, bedsores, hospital transfers) and to formal complaints by families. Offsetting these concerns are scattered but meaningful positive observations: individual caregivers who are kind and competent, a knowledgeable DON, a new administrator reportedly implementing improvements, and isolated reports of residents enjoying life and maintaining mobility. The reviews indicate a facility with serious operational and quality challenges but with pockets of good care and potential for improvement if staffing, sanitation, responsiveness, and management oversight are addressed consistently.
