    Kind staff, recommend with caution

I felt genuinely cared for by kind, attentive staff - Cindy in social work and Tia in billing were outstanding, PT and nurses were collaborative, activities and pet visits helped me settle, and many staff went above and beyond so my transition felt effortless. That said, I encountered serious issues at times - long medication delays after transfer, understaffing, poor cleaning (I was once left with wet bedding), cold room/no water, safety concerns and billing/administration confusion - so stay vigilant. Overall I was mostly happy with the quality of care and would recommend, but ask about discharge/billing and watch for staffing or safety problems.

    Summary review

Overall sentiment across the reviews is mixed but leans positive for frontline caregiving and therapy services while revealing recurrent administrative, safety, and consistency concerns. A substantial number of reviewers emphasize that daily caregivers, nurses, and therapy staff are caring, attentive, and often go above and beyond: many callers single out the physical therapy program as excellent, praise a collaborative PT department, and name social work and billing contacts (for example, Cindy and Tia) as particularly helpful. Multiple accounts describe comfortable rooms, nice facilities, an on-site hair salon, pet visits being allowed, many in-house activities, family-welcoming policies, and a service staff rated highly — all factors that produce strong recommendations and gratitude from family members and residents who report feeling safe and well treated. Several reviewers reported smooth admissions/registration experiences and effective discharge assistance in individual cases.

However, these positive impressions are contrasted by several serious and recurring negative themes. A number of reviewers reported safety and basic-care failures: incidents include a resident being left soaked with wet bedding, inadequate hygiene during diaper changes, equipment left on floors creating trip or safety hazards, lack of available water for residents, and reports alleging neglect including failure to provide oxygen. Staffing concerns appear in multiple summaries — reviewers describe unresponsive staff or understaffed shifts, which likely contribute to delayed medication administration (notably a 6–7 hour wait after hospital transfer reported by one reviewer) and inconsistent care quality. These types of lapses represent significant risks for medically fragile residents and are a prominent cause of the more negative reviews.

Administration, billing, and care coordination are another area of consistent complaint. Several reviewers reported poor administration or dismissive behavior from leadership, unclear or problematic billing practices (including charging for a private room without reimbursement), and poor discharge coordination that created confusion and stress. While individual staff members in billing and social work were praised for being helpful, the systemic issues around billing clarity and discharge planning recur enough to be a pattern rather than isolated incidents.

Food and comfort elements show variable results: some residents and families say they “loved the food” and enjoyed the environment and activities, while others call food quality hit-or-miss. Room comfort and facility quality are generally described positively, though at least one reviewer mentioned the room being cold due to low AC. COVID-era restrictions were noted as limiting visits for some families, which affected experiences during that time frame.

In summary, Liberty Commons Nursing & Rehabilitation Center of Lee County receives frequent praise for frontline caregivers, therapy services (especially PT), social work, and many aspects of its environment and amenities. These strengths lead multiple reviewers to highly recommend the facility. At the same time, there are recurring and serious concerns about safety, basic hygiene, some aspects of medical care, medication timeliness after transfers, administrative responsiveness, billing transparency, and discharge coordination. The most actionable patterns from the reviews are: (1) reinforce staffing levels and responsiveness to prevent lapses in basic care and medication delays; (2) address reported safety hazards and hygiene protocols to reduce risk of neglect; and (3) improve administrative communication and billing/discharge processes to resolve recurring family frustrations. Prospective residents and families should weigh the facility’s clearly strong therapy and compassionate direct-care staff against the potentially serious and inconsistent administrative and clinical-safety issues reported by a subset of reviewers.

About Liberty Commons Nursing & Rehabilitation Center of Lee County

Liberty Commons Nursing & Rehabilitation Center of Lee County sits at 310 Commerce Drive in Sanford, North Carolina, right off Route 1, and is part of a Continuing Care Retirement Community, so you've got different levels of care and housing choices all in one place. The facility has 80 certified beds, with an average census of about 73 people each day, and you can find both private and semi-private suites, each one offering things like a private bathroom, kitchenette, furnished accommodations, cable TV, WiFi, air conditioning, and telephone service, which helps to make it feel a little more like home. The dining room looks like a restaurant, with a professional chef on staff, so you can get your meals whenever you like because they offer all-day dining, and there are menu options for diabetes or allergies too, and if you'd rather eat alone or need a quiet place, there's in-room dining.

Housekeeping and laundry both happen every day, so rooms stay tidy and clothes are fresh, and outdoor walking paths, a garden, and scheduled group activities, like fitness programs, movie nights, music, and resident-run clubs, give people something to do both inside and outdoors. You'll see a spa and wellness room if you need a haircut or some relaxation, plus a movie theater, an activity room, a business room, and a game room for social time. Staff run family support groups, and transportation and parking are available if you need a ride to appointments. The building has emergency alert systems and a 24-hour call system for safety, with registered nurses, licensed practical nurses, and certified nursing assistants working around the clock, and you'll find care for everything from transfers to bathing, dressing, medication reminders, and help for folks who can't walk on their own.

On the rehab side, there's a special unit and a therapy gym with an Activities of Daily Living Suite where people practice things like dressing and making meals, and care plans get put together for each person by in-house experts in speech, occupational, and physical therapy, offering treatments like diathermy, electrical stimulation, and vital stimulation to help with pain, joints, swallowing, and muscle strength. The place covers everything from short-term rehab, skilled nursing, long-term living, outpatient therapy, respite, palliative, and hospice care, and special programs for folks with memory conditions like Alzheimer's and dementia.

Folks here get medical checkups, wound care, physician coverage, cardiopulmonary therapy, fall prevention, incontinence care, IV therapy, and access to services like geriatric psychiatry and podiatry, so there's broad healthcare help right in the building, and the team tries to cut down on hospital trips by making care plans focused on the person's needs. Liberty Commons belongs to Liberty Senior Living, with ownership as a for-profit limited liability company, and the facility is run by an administrator named William "Bill" Watson. They welcome Medicare and Medicaid, and admissions happen seven days a week for flexibility.

But it's also worth noting some issues, since the Department of Health's April 2024 inspection listed nine deficiencies, and the facility has reported 56 total in past inspections, including assessment, care planning, infection control, and dental service issues, with a higher nurse turnover rate at 70.5% (compared to the state's 50.9%), and nursing hours are below average at 3.20 hours per resident per day, compared to the state average of 3.8. Still, the staff keeps on with daily routines and group activities, and the grounds offer indoor and outdoor spots for relaxing or meeting with family. So, Liberty Commons Nursing & Rehabilitation Center of Lee County gives a full range of nursing and rehab services in one place, with some nice comforts, and a mix of strengths and areas that need improvement, much like a lot of other care homes around.
