    Excellent therapy, inconsistent overall care

I had a mixed but overall helpful experience: the therapy team was outstanding (twice-daily PT, motivated staff and students) and nursing/activities staff often felt loving and family-like - the facility was clean, homey, and my dad left stronger and happy. That said, care was inconsistent: some shifts were neglectful (unresponsive call buttons, patients left soiled, staff distracted or rude), management dismissed complaints, turnover is high, and safety/coordination issues were reported. It's expensive (≈$6,000/month) and feels for-profit. I'd recommend it for short-term rehab and therapy, but I'd be cautious about long-term placement.

    Summary review

Overall sentiment across the reviews is mixed but leans toward positive for short-term rehabilitation and for families who encounter the facility with strong, consistent staffing. Many reviewers praise the core caregiving teams — nurses, CNAs, dietary, housekeeping, and therapy staff — describing them as compassionate, family-like, responsive, and supportive. Multiple accounts note excellent physical therapy services, including twice-daily PT, knowledgeable therapists, therapy students assisting with ADLs, and successful outcomes that enabled residents to return home stronger. The facility is frequently described as clean, pleasant smelling, well-decorated, and offering comfortable communal spaces such as a living room/library, outdoor seating, and places to gather for activities and events. Several staff members and leaders receive individual praise, with one admission director mentioned by name for helpful transition assistance.

Care quality is a recurring theme with sharp contrasts. On the positive side, many families report outstanding care planning, teamwork, quick responses, and hands-on support that led to meaningful recovery. Reviewers highlight kindness and dignity provided by staff, good food options for many residents, and a lively social program with bingo, bowling, book talks, seasonal festivals, and other engaging activities. The facility appears well-suited for short-term rehab stays, and many recommend it highly for therapy and recovery needs.

However, there is a significant and repeated set of negative reports that must be weighed carefully. Several reviewers describe an inconsistent experience between shifts and over time: while some shifts and staff are praised, other shifts are accused of neglect, leaving residents unattended, unresponsive call buttons, and even leaving patients in soiled conditions for hours. Multiple reports allege serious harms, including injuries from mishandling, weight loss attributed to neglect, delayed or inadequate hospital transfers, and at least one reviewer alleging a preventable death. There are also reports that management has denied issues and failed to address complaints, leading to repeated problems and family frustration. A few reviews go further, alleging staff theft and claiming the facility changed its name to avoid negative reviews. These are serious concerns that contrast strongly with the many positive accounts.

Staffing and leadership appear to be a major dividing line in the reviews. Several positive reviews emphasize top-notch leadership, strong collaboration, and a positive workplace culture, while a distinct cluster of negative reviews describes staffing decline, high turnover, new directors who mistreat staff, and Med Techs or aides with limited knowledge of residents. These staffing inconsistencies are tied directly by reviewers to lapses in care quality and patient safety. Physician coverage is another concern in negative accounts, with at least one reviewer saying the doctor was rarely seen and came at night, and others reporting poor communication from medical staff.

Facilities, dining, and activities receive mostly positive feedback. Multiple reviewers note that the building is clean and well organized, with attractive decorations, lively social interaction among residents, and plentiful activities. Food receives mixed but generally favorable comments: many describe good or mostly good meals, though one reviewer said some menu items did not match their personal preferences. Physical spaces like private rooms, outdoor seating, and communal game areas are highlighted as strengths by numerous families. A few reviews contradict this, noting small rooms, cleanliness problems in specific areas, crowded shared rehab rooms, or mishandled laundry.

Management and business concerns are also prominent. Several reviewers perceive the facility as for-profit and prioritize profit over patient care, a sentiment strengthened by the reported high monthly cost of about $6,000 in one review. Complaints about management response, lack of remediation after incidents, and allegations of name changes to avoid negative reputation contribute to mistrust among some families. Conversely, other reviewers praise administrative responsiveness and leadership, indicating there may be variability over time or by department.

In summary, the Greens at Weaverville shows strong, repeatedly praised strengths in therapy and short-term rehabilitation, compassionate and family-like caregiving from many staff members, clean and pleasant facilities, and a robust activities program. Those positives have led many residents to recover and return home and have left families grateful and highly satisfied. At the same time, there are serious and recurring negative reports about staffing inconsistencies, neglect on some shifts, poor complaint handling, and even alleged safety incidents including injuries and weight loss. These negatives introduce substantial risk for prospective long-term residents or families who may encounter periods of reduced staffing or management problems.

Recommendation for families considering this facility: visit in person multiple times, meet the therapy team and clinical leadership, ask for staffing ratios and recent inspection or incident reports, confirm physician coverage patterns, tour typical rooms (not just staged options), observe mealtime and activity periods, and inquire about how complaints and safety incidents are handled and documented. If short-term rehab is the need, the facility has many strong endorsements; for long-term placement, weigh the positive staff and therapy reports against documented variability in care and the serious nature of some negative allegations before deciding.
