    Great rehab, understaffed, communication issues

I had a mixed experience at Three Rivers. The rehab team, nurses and CNAs were exceptional-compassionate, communicative, and instrumental in getting my mom from bedridden to walking 4-5 steps and off a diabetic med; the facility is very clean, hotel-like, with good meals and a roomy single room. That said, chronic understaffing and poor responsiveness (unanswered phones, declined admissions) led to negligent moments-missed tray setups and weight loss, instructions to urinate, multiple falls and inadequate fall-prevention-so I'd recommend it for rehab but caution families about long-term staffing and communication.

    Summary review

Overall sentiment in the reviews of Three Rivers Health & Rehabilitation Center is mixed but leans positive for rehabilitation outcomes, cleanliness, and the interpersonal skills of many frontline staff. A large portion of reviewers praise the nursing aides, therapists, cooks, and administrative staff for being kind, helpful, and attentive. Multiple accounts highlight meaningful clinical progress: residents who were initially bedridden regained steps, diabetic medication was discontinued for at least one resident, and therapy teams are repeatedly credited with improving mobility and function. The facility’s sanitation and physical environment are also frequently commended—descriptions include "very clean," "odor-free," and "hotel-like amenities," and some reviewers specifically called out single rooms and thoughtful janitorial service.

Staff quality and culture come through as a strong positive theme in many reviews. Several reviewers describe a family-like atmosphere, compassionate CNAs, respectful interactions, and good communication about care when staff are responsive. Administrators are noted in some cases for being helpful with insurance navigation and for providing outstanding personal attention. Dining receives positive mention too—meals are described as "perfect," and cooks are called accommodating. For prospective patients seeking rehabilitation-focused recovery, the center is explicitly recommended by multiple reviewers based on observed progress and the expertise of the therapy team.

However, there are significant and serious negative reports that temper the overall positive picture and point to inconsistency in care. Some reviewers reported nursing neglect with concrete adverse outcomes: multiple falls and injuries, inadequate interventions for high fall-risk residents, and instances where staff were too busy to provide basic assistance (one review stated the resident was told to urinate on himself). There are also reports of weight loss attributed to lack of assistance with meal trays. These safety and dignity issues are severe and were echoed strongly enough to be a prominent concern across the review set.

Communication and responsiveness are a polarized area. Many reviewers note quick responses, immediate care updates, and responsive administration; yet an almost equal number report difficulties reaching staff by phone, unreturned calls, declined admissions without reason, and poor follow-up that even escalated to accusations of dishonesty in at least one case. This variability indicates uneven performance—while some families experienced clear, proactive communication and rapid problem resolution, others encountered frustrating breakdowns in access and transparency.

In summary, Three Rivers Health & Rehabilitation Center appears to provide a high-quality rehabilitative environment for many residents, characterized by compassionate caregivers, effective therapy, clean facilities, and good meals. Those positive experiences are compelling, especially where measurable clinical improvements were achieved. At the same time, there are troubling outlier reports describing neglect, fall-related injuries, inadequate assistance with basic needs, and inconsistent communication. The pattern suggests that the facility can deliver excellent, individualized care but may have lapses in staffing, supervision, or processes that create pockets of substandard care. Prospective residents and families should weigh the strong rehabilitation and facility strengths against the reported safety and responsiveness concerns, and consider asking targeted questions about fall-prevention protocols, toileting and feeding assistance procedures, staffing levels and how the facility handles missed calls or declined admissions before committing.
