    Attentive family-like long-term care facility

I'm very pleased with this facility - my parent has lived here nine years and improved; the staff are caring, responsive, and go above and beyond, with director and nurses available for problem solving. The building is spotless, well-laid-out and attractively furnished, with comfortable rooms, homemade meals, plenty of activities, library/activity rooms, laundry service, and lovely landscaped grounds with outdoor seating. Medicaid transition was possible, it's within an hour of home, and I'd recommend it for attentive, family-like long-term care.

    Summary of reviews

Overall impression: Liberty Village presents as a well-appointed assisted-living environment with strengths in staffing, cleanliness, and amenities, but with recurring operational concerns that warrant verification during any tour or family meeting. Many accounts emphasize warm staff interactions, an active calendar, attractive common areas and grounds, and an environment that some residents describe as hotel-like and comfortable. At the same time, reviewers raised several persistent issues around safety, regulatory adherence, and the facility’s ability to meet higher-acuity needs.

Care and staff: The most consistent positive theme is the quality of interpersonal care. Staff are frequently described as caring, attentive, and familiar with residents; families report improvement in well-being and responsiveness from directors and nursing leadership. Several long-term residents and families cite above-and-beyond attention and good communication with relatives. However, there are also operational limits: multiple accounts indicate residents whose clinical needs exceed the staffing scope or licensed services available at this level of care. Prospective families should clarify staffing ratios, the facility’s accepted acuity range, and transfer protocols when residents require a higher level of nursing support.

Dining and activities: Activity programming appears robust, with regular events, seasonal celebrations, in-house crafts, and on-site amenities such as a library and activity rooms supporting social engagement. Grounds and indoor gathering spaces are noted as pleasant and walkable. Dining receives mixed feedback: several reviewers praise homemade, varied meals, while others express dissatisfaction with food quality or consistency. This variability suggests meal experience can differ over time or between units; families may want to sample meals and ask about dining policies, menu rotation, and therapeutic-diet capabilities.

Facilities and safety: The physical plant is generally described as attractive, well-laid-out, and clean, with upscale furnishings and private rooms that include en-suite bathrooms. Criticisms focus on room size relative to price and specific safety and sanitation concerns. Multiple reviewers raised issues that map to facility-level risks: pest-control concerns, unlocked doors during daytime hours, and allegations of lapses in fire-safety/OSHA/other regulatory compliance. These are significant operational items; prospective families should request documentation of inspection histories, pest-control contracts and remediation records, fire-safety drills and evacuation plans, and any corrective-action communications from regulatory bodies.

Management and operational patterns: Accounts of management responsiveness are mixed. Several families emphasize accessible leadership and prompt problem-solving, while others describe variability in operational consistency and disappointments with how some incidents were handled. There are also statements indicating limited budgetary support for off-site outings and transportation, which can constrain residents’ community engagement. Given these mixed signals, verify written policies on family communication, incident reporting, transportation/outing schedules, and financial transparency.

Bottom line: Liberty Village offers many of the hallmarks families seek—compassionate staff, cleanliness, active programming, and an attractive environment—balanced against discrete but important operational concerns around safety, pest control, regulatory compliance, dining consistency, and appropriate clinical scope of care. An in-person visit that includes a meal, inspection of recent inspection/certification records, discussion of staff credentials and ratios, and review of policies for handling higher-acuity needs and pest remediation is recommended to determine fit for an individual resident.

    About Liberty Village Senior Community

Liberty Village Senior Community sits just west of Chillicothe on Route 50, right within Scioto, Ohio, and is a single-story place with a family feeling where residents can keep their independence while getting the care they need as they age, which some folks really like because there are 40 private rooms and each suite can have personal touches, photos, or decorations so it feels like home. The community has a long list of amenities like spacious closets, private bathrooms with wheelchair-accessible showers, kitchenettes, individual thermostats, and common areas with fireplaces, parlors, and a lovely interior courtyard where people often sit. Residents can enjoy meals in the dining room or choose all-day dining with restaurant-style service and special menus for people with dietary restrictions like diabetes, allergies, or if they just want vegetarian food, and there's help available for medication management, bathing, dressing, and other daily activities by licensed nurses and trained staff. Liberty Village Senior Community is the first assisted living community serving Chillicothe, and staff are always around since there's 24-hour supervision, an emergency alert system, fire alarms and sprinklers in every suite, and a 24-hour call system, so residents and their families know someone can always help if something happens. It's a Continuing Care Retirement Community, which means people can get independent living, assisted living, respite care, rehabilitation, and memory care on the same campus, letting them stay in familiar surroundings if their health needs change, and some healthcare services on-site include visits from nurse practitioners, physicians, mobile X-ray and lab, blood pressure checks, and therapies like physical, occupational, and speech therapy. You'll find a long schedule each day with social, educational, and recreational activities, including outings, gardening, arts and crafts, a movie room, a library, and resident-run events, and some people really appreciate the transportation service and the fact that it's near a busline, which can make trips to nearby health providers like Adena Health Center, Chillicothe Family Physicians, or the pharmacy easy. There are also hospice and respite care options, and the community helps coordinate home health agency services for short-term skilled care if needed, all in a straightforward, home-like environment where folks tend to become friendly neighbors over time, and with licensed residential care status in Ohio, safety and tidiness are kept up, and reviewers, from what's shown, give the place a community score of about 4.8 for safety, livability, and sentiment, though some online reviews rate it closer to 3.3 out of 7 reviews. Wheelchair-accessible suites, welcoming lobbies, laundry, housekeeping, beautician, barber, and spa services, along with things like pet-friendly policies, give extra touches for comfort, and the grounds feature controlled building entry and both indoor and outdoor areas for walking and quiet time, making it a spot where adults aged 55 and over can find as much independence or support as they need, whether they're in assisted living, independent living, memory care, or just needing a bit more help after a hospital stay.
