    Kind staff and excellent care

I placed my grandma here and she loves it - the staff are kind, helpful, and treat residents like family. Nursing, therapy, and activities are excellent; the team keeps us informed, goes above and beyond, and the clean, welcoming facility offers lots of fun events and pet visits. Great care and value overall - I highly recommend it.

    Summary of reviews

Reviews of Willow Haven Healthcare & Rehabilitation present a mixed picture with distinct strengths and recurring operational concerns. Many families and some staff describe genuinely compassionate, attentive nursing and strong therapy/rehabilitation services. The activities program and community events are frequently praised — residents enjoy seasonal events and pet visits, and the activity department is described as energetic and engaging. Several reviewers also cited a warm, family-like culture among particular employees, timely communication with families, and a clean, recently updated interior in parts of the building.

Care quality appears variable depending on shift and staffing. Positive accounts highlight attentive nursing that treats residents like family and thorough updates to families. Contrasting accounts describe delays in responsiveness, particularly after hours and overnight, and inconsistent personal-care practices including irregular bathing and delays in attending to incontinence needs. These patterns point to uneven coverage and performance across different shifts rather than uniform facility-wide standards.

Staffing and management are a central theme. Praise is concentrated on full-time, long-term employees who are described as going above and beyond, while criticism centers on heavy use of agency staff, high turnover, and inconsistent professionalism on certain shifts. This staffing variability contributes to inconsistent resident experiences. There are also multiple mentions of administrative issues such as billing and authorization confusion and placement promises that were not consistently fulfilled, suggesting opportunities to improve admissions and billing processes.

Dining and housekeeping impressions are mixed. Several reviewers found the facility clean and the staff welcoming, while others described inconsistent meal quality and concern about sanitation or odor control in particular situations. Facility amenities and aesthetics have been updated in some areas and are appreciated by families, though some reviewers found certain units cold, sparsely furnished, or lacking appropriately sized furniture and equipment (beds, chairs), which can affect resident comfort.

Activities and community engagement are clear strengths; organized events, family-oriented celebrations, and proactive activity staff are repeatedly mentioned. For prospective residents and families, the most important considerations emerging from these reviews are to verify current staffing levels (especially overnight), ask about the ratio of agency to full-time staff, confirm admissions and billing procedures (including any VA authorizations), and observe mealtime and personal-care routines during a tour. Overall, the facility shows capacity for high-quality, compassionate care in some units and shifts, but consistent delivery of that care appears to depend on staffing stability and management follow-through.

About Willow Haven Healthcare & Rehabilitation

Willow Haven Healthcare & Rehabilitation sits in Zanesville, Ohio, on a big campus that feels a little like the countryside, with 81 rooms offering mostly semi-private living for an average of 76 residents each day, and folks there will find both short-term and long-term care plus a wide range of health services, though the place does have a much below average rating for health inspections, staffing, and quality, and a slightly below average overall score according to CMS, which means there are some concerns, especially since recent inspection reports found three infection-related issues. The staff includes licensed nurses, therapists, dietitians, social workers, and medical directors, and they do provide skilled rehabilitation services for people who need inpatient or outpatient care, including physical, occupational, and speech therapies, and they also offer help with nutrition, recreation, hospice, post-acute stays, respite, and more, and while there's a beauty parlor and barbershop on site and activities for folks to join, it's important to know the nurse turnover rate is pretty high at 50.7% and nurse staffing hours come out to about 2.98 per resident per day, a rate that's lower than many other places. The facility is operated by Certus Healthcare, a for-profit company with several locations around Ohio, and they do accept both Medicare and Medicaid, but it should be noted that right now Willow Haven isn't taking new patients. Recent inspections found some problems about honoring residents' rights, including how residents are allowed to voice concerns and keep their communications private, so while staff members are described as compassionate and they do work to tailor care to individual needs, these issues are something families and residents should be aware of. The place has semi-private rooms and a variety of care plans, and they say they're working on improvements with renovations and some upcoming events, plus there are online updates for provider directories and COVID-19 notices. The continuing healthcare programs at Willow Haven include regular medical care, therapy, and administrative support, so while there are plenty of services under one roof-ranging from skilled nursing, rehabilitation, and social activities to specialized programs and care for people with different needs-it's always a good idea to review the inspection reports and ratings to get a clear picture before making any decisions.
