    Caring staff, excellent rehab, clean

I placed my loved one here and was very pleased - the staff were consistently caring, compassionate and responsive, and the rehab team did excellent work that led to a remarkable recovery. The facility is clean, well-kept and family-friendly with flexible visiting, private rooms for visits, good food, and a true team approach to personalized care. Overall I'm very satisfied and feel they genuinely looked out for us.

    Summary of reviews

The reviews present a polarized picture of Hilltop Healthcare and Rehab. Many families and former patients praise the facility’s compassionate caregivers, competent nursing, and a strong rehabilitation program that supported notable recoveries and safe discharges. Comments highlight a family-like atmosphere, personalized attention from nurses and aides, helpful front-desk staff, and generally well-kept common areas; several reviewers specifically noted timely clinical follow-up, teamwork among staff, and a comfortable environment for family visits.

At the same time, a substantial number of reviews describe operational shortcomings that affect day-to-day care. The most consistent concerns are understaffing and uneven nurse availability, which reviewers link to delays in responding to call bells and resident needs. Several accounts describe gaps in sanitation and hygiene maintenance in particular units, inconsistent bathing schedules, and irregular linen changes. These operational lapses appear to be intermittent rather than uniform across the facility, producing markedly different experiences depending on unit or shift.

Communication and management issues are another recurring theme. Families described breakdowns between the clinical and business sides of the operation, difficulty coordinating discharges and post‑acute services, insurance-authorization limitations, and occasional delays arranging home nursing after discharge. Visitors also noted inadequate in-room communication infrastructure (no patient room phones and unreliable cordless phones at the nurses’ station), which compounds family-staff communication challenges. Visitation enforcement varied during infection-control periods, which some families experienced as restrictive.

There are also serious safety and security concerns raised by reviewers, including missing personal items and the use of legal counsel in response to unexplained resident injuries; these are characterized in several reviews as allegations that families pursued externally. Dining and daily routines receive mixed feedback: some reviewers praised food quality and meal presentation, while others reported late meals or inconsistent service. Facility cleanliness is likewise described inconsistently — multiple reviewers called the building clean and well maintained, while others cited localized sanitation issues.

Overall, Hilltop shows clear strengths in rehabilitative services, individual caregiver–resident interactions, and certain areas of facility upkeep. However, variability in staffing, sanitation maintenance, internal communication, discharge coordination, visitor communication infrastructure, and security controls creates an uneven experience. Prospective residents and families should verify current staffing levels, wound- and skin-care protocols, call-bell response metrics, discharge-planning processes, valuables/security policies, and infection-control/visitation procedures during an in-person visit to assess whether the facility’s present operations match their expectations and needs.

    About Hilltop Healthcare and Rehab

Hilltop Healthcare and Rehabilitation Center, which sits in Altoona, Pennsylvania, provides a range of services for seniors, including nursing home care, skilled nursing, rehabilitation, assisted living, independent living, and memory care, so residents with different care needs can find what they're looking for here, and the rooms come in various studio layouts to fit different preferences. The community has 133 certified beds and is for-profit, run by Melissa Andolina, Brett Scharf, and Abraham Smilow since April 2023, and carries an affiliation with Abraham Smilow. Residents get services like nursing care, rehabilitation to help them recover after hospital stays, hospice care for those at the end of life, and memory care services made for people facing memory issues, while long-term and short-term stays are both options. Staff provide an average of 3.33 nurse hours per resident each day, which is beneath the state average, yet their nurse turnover rate at 19.8% is lower than Pennsylvania's average of 47.0%, which often helps with consistency. The community focuses on resident rights, provides nutrition and dietary services, and offers a fitness center, a library, a beauty salon, and a peaceful chapel, while social activities and events are set up to help people stay busy and connected. Residents live in a quiet, calm place, and the facility works on goals like quality improvement and resident-centered care, but it's important to note that inspection reports list 40 deficiencies, including those about infection control, nutrition, and resident rights, and the community holds a 2-star rating from the Centers for Medicare & Medicaid Services with a general community rating of 7.9 out of 10, so families might want to review reports and visit in person if possible.
