    Kind staff, great care, peace

I moved my grandmother here and have been very satisfied. The nurses, aides and staff are kind, attentive and professional - communication and discharge coordination are clear and staff often go the extra mile. Meals look and smell good, activities (bingo, trips, luncheons) are plentiful and engaging, PT/OT improved mobility, and the onsite dialysis center was a godsend. Facilities and amenities are extensive and I have real peace of mind; I would recommend them.

    Summary of reviews

Reviews of Riverside Health & Rehab Center present a mixed picture with clear strengths alongside recurring operational concerns. Many families praise direct-care staff — particularly nursing aides and some nurses — for being caring, attentive, and willing to go the extra mile. Rehabilitation services receive consistent positive feedback: physical and occupational therapy are credited with measurable mobility improvements. The facility's activities program is robust, offering bingo, local outings, luncheons and other social options, and an on-site dialysis center is highlighted as a practical advantage for residents with renal needs. Several reviewers also described meals as appealing and noted a generally well-maintained, hospital-like environment in parts of the building.

At the same time, reviewers describe variability in care quality depending on unit and shift. Recurrent themes include inconsistent staffing levels and responsiveness, with some accounts of delays in attending to toileting and hydration needs and concerns about pressure-area prevention and wound-management practices. Medication-administration timing and nighttime medication practices were also questioned. There are additional safety-related concerns such as a higher fall rate noted by some families and perceived gaps in emergency response and transfer procedures. Positive clinical outcomes in therapy coexist with reports indicating that clinical escalation and timely medical intervention have occasionally been insufficient.

Communication and management practices are another area of divergence. Several families praised case managers and specific admissions staff for clear explanations and proactive discharge coordination, while others characterized admissions interactions as overly sales-oriented and expressed frustration with billing and bed-holding practices that appear financially driven. After-hours phone access and responsiveness were described as inconsistent. Reviews also raise issues around privacy and property security, including confidentiality-control weaknesses and allegations of theft; one or two more serious claims include suspected staff substance misuse, which some reviewers described to ombudsmen or regulatory contacts.

For prospective residents and families, the pattern suggests that experience at this facility may depend substantially on the unit, the specific caregivers on duty, and how management addresses those operational gaps. When evaluating the facility, families may wish to ask targeted questions about staffing ratios by shift, protocols for clinical escalation and wound prevention, medication-administration policies, after-hours contact procedures, property-security safeguards, and oversight of professional conduct. Reviewing recent inspection records and speaking with the ombudsman or current family contacts can also help clarify whether the facility’s strengths in therapy, activities, and certain caregiving staff align with acceptable risk levels for an individual resident’s needs.

About Riverside Health & Rehab Center

Riverside Health & Rehab Center, also known as Riverside Nursing Center, sits at 100 8th Ave in McKeesport, PA, not far from McKeesport Hospital and looking out over the Monongahela River, and you'll find a big building with 120 beds, or to be exact, 119 certified ones, where about 93 seniors stay on an average day. This place is popular among older folks who need serious healthcare, and it takes patients with Medicare, Medicaid, and several other insurance plans like Aetna and Three Rivers, which helps families who are worried about paying for care. Riverside belongs to Saber Healthcare Group, with Grane Healthcare managing things like administration and consulting, but at its core this is a skilled nursing facility that takes in seniors who need short stays after surgery, long-term help, hospice, memory care, and respite when caregivers need a break, and they're set up for people with complex needs, whether that's physical or mental, including folks who are fragile or need machines and close medical attention. Doctors, registered nurses, licensed nurses, certified aides, therapists, psychologists, and dieticians are all part of the staff, and more than 70 doctors visit or care for residents, offering support for things like cardiac recovery, orthopedic issues, stroke, pulmonary problems, pain management, IV therapy, and wound care, plus there's on-site dialysis which can be important for certain patients.

Rehabilitation services fill a big role here, so physical, occupational, and speech therapies are all offered, and there's even a pre-surgery exercise and pre-registration program that helps guide people through the hospital-to-rehab-to-home process without extra confusion, which can be comforting when everything feels overwhelming. Riverside runs a special transitional care model called Progressive Approach to Home (PATH), meant to use proven methods to get folks healthy again and ready to go home, and this includes personalized treatment plans that involve the person's own doctor whenever that's possible. The Memory Care Neighborhood is designed for those with dementia or memory issues, with routines and activities to keep people safe and engaged, while regular restorative and activity programs, social gatherings, holiday events, and an interior courtyard make life feel less lonely and more like home. Most rooms, whether shared or private, let you look outside at the river, and that's a small comfort for many.

As far as quality, Riverside holds a high average review score of 9.8 out of 24 reviews, showing a lot of families are satisfied, though nursing hours per resident are a bit less than the state average, and the nurse turnover rate is higher too, meaning there's some staffing challenges, which happens often in busy places. Inspection reports list 28 total deficiencies with one related to infection and a recent inspection in January 2025 found eight issues, including infection control and lapses in following doctor's orders, some marked as immediate jeopardy, which folks should take into account if they're worried about safety, though nobody was actually harmed in these cases based on the report. With both skilled and intermediate care, Riverside covers most needs for people who are no longer able to live safely at home and who require 24-hour care, with insurance options and specialized programs that try to get people better and back to their familiar routines, but like many big places, it has areas it's working to improve even as it remains one of the busier and more trusted choices around McKeesport for everything from rehab to long-term nursing.
