    Excellent care, clean facility, affordable

I've had a pleasant stay - the staff are excellent and helpful, and the facility is clean and well maintained. On-site nurses and physical therapy provide strong care, medications are timely, and they help with Medicare. Meals are balanced and individualized, daily activities (including live bands) keep things lively, and pricing is affordable; I would recommend this community.

    Summary of reviews

Reviews present a markedly mixed picture: several reviewers praised the facility's on-site clinical resources and therapy outcomes, while others described operational and communication problems that affected care continuity. Positive comments center on accessible clinical services, active programming, and perceived value; critical comments emphasize inconsistent care, staffing and management issues, and maintenance concerns.

Care and staffing: Many families and residents noted the presence of on-site nurses and medical personnel and reported that occupational and physical therapy services were effective for rehabilitation goals. Timely medication administration and helpful direct-care staff were also described. At the same time, there are persistent accounts of inconsistent care quality, staffing shortages or turnover, and delays in routine personal care (including irregular bathing schedules). Communication gaps between staff and families were raised repeatedly, and reviewers noted difficulties coordinating higher-acuity needs (for example, dialysis requirements) and external medical appointments.

Dining and activities: The dining program is described positively by some as providing balanced, resident-specific meal planning; social programming including live music and daily activities was highlighted as a strength for resident engagement. However, other reviewers described variability in food quality and preparation, with a perception of some meals being less fresh or appealing. Prospective families should expect active programming but ask about current menu practices and mealtime satisfaction monitoring.

Facilities and maintenance: Several reviewers described the facility as clean and well maintained in common areas, contributing to a sense of value. Conversely, others reported maintenance and sanitation concerns in particular areas (including carpeting and localized odor concerns), and comments noted that portions of the building would benefit from updating. Room configurations are sometimes crowded, with two-person rooms limiting privacy for some residents. Practical issues such as limited snow removal (only paths cleared) and overnight clinical checks that may interrupt sleep were also raised.

Management and patterns: Affordability and assistance with Medicare/benefits are frequently cited as positives, supporting the facility's value proposition. At the same time, there are recurring concerns about management transparency, perceived prioritization of cost control over advocacy, canceled external appointments, and an organizational culture that some families find unresponsive. Experiences appear polarized: several families would recommend the facility and described pleasant, trustworthy staff interactions, while others had strongly negative experiences. This variability suggests uneven operational performance across units, shifts, or resident acuity levels.

Recommendation for prospective families: Given the mixed reports, a targeted tour and specific questions are advisable. Ask about current staffing levels and turnover, bathing and personal-care schedules, how the facility coordinates external medical appointments and dialysis care, medication-administration protocols, recent facility maintenance or refurbishment plans, and how family concerns are escalated and resolved. Visiting during an activity period and mealtime can also help assess programming, food quality, and day-to-day staff–resident interactions.

    About Avantara North

Avantara North sits at 1620 North 7th St. in Rapid City, SD, and the whole place works to create an easy, welcoming atmosphere for seniors who need help at different levels, whether that's independent living, assisted living, memory care, skilled nursing, or short- and long-term rehabilitation, and what stands out most is how thorough their services are, since their team provides round-the-clock care every day of the year-including weekends and holidays-with 12-16 hour nursing shifts, a 24-hour call system, and regular health checks by a trained interdisciplinary team that covers a big range of medical and support needs from orthopedic care and cardiac rehab to pulmonary programs that really try to help improve breathing and chronic lung conditions, and they take wound care seriously by using things like E-Stim, ultrasound, and negative pressure therapies for better healing, so people coming from surgery or hospital stays can feel supported with special attention to surgical incisions, suture and staple removal, and proper lung exercises. Meals are planned and made by a professional chef, and the staff handles all kinds of diets-whether it's allergy-sensitive or diabetic, and the dining setup means folks can eat throughout the day instead of being boxed into set meal times. Rooms come furnished with private bathrooms, kitchenettes, cable TV, phones, air conditioning, and Wi-Fi, plus an emergency alert system, and the spaces have a calm, tranquil design that tries to help the mind and the body, while walking paths, a fitness room, spa, garden, wellness room, game and arts rooms, a library, and a business center give a lot of ways for people to spend their time-whether that's at a community event, music night, art project, or just quiet reading time. Avantara North offers help with personal care like bathing, dressing, and taking medications, and they provide supervision and support for anyone who has memory challenges, including Alzheimer's and Dementia, through their on-site Memory Care. They handle moves, arrange transportation and parking, and help with insurance paperwork, and the staff tries to make everyone's life easier by setting up concierge services and tours for anyone interested, plus they hold a 2023 AHCA Bronze award for quality, and people there often join in local events or lend a hand with community groups, all while the facility works with many national and local insurance providers, keeps up with civil rights laws, and strives to be good neighbors in Rapid City. The whole focus is on caring for people, making sure they get what they need to be comfortable, safe, and supported, whether they're there for a short stay after surgery, longer rehabilitation, or a new chapter in their lives, and all the care goes hand in hand with simple comforts like home-cooked meals, music and movies, garden walks, and having someone nearby who knows what to do and who tries to pay attention to each resident. Avantara North runs a 44-bed health care and rehabilitation center that's recognized as a Great Place to Work and one of the Best Workplaces for Aging Services, and anyone who wants to learn more can see details at avantaranorth.com.
