    Attentive staff and excellent care

I'm very pleased with Green Oaks. The nurses, CNAs and caregiving staff are attentive and genuinely caring-admissions and management (Kimberly/Leigh stood out) were professional and responsive. Therapy/rehab was excellent, meals were tasty, activities kept my loved one engaged, and the building was clean and welcoming. My mom was well cared for and our family feels grateful and reassured.

    Summary of reviews

Green Oaks presents a highly polarized set of impressions. Many families and residents praise the facility's rehabilitation services, activities program, and portions of the clinical and admission teams. Physical and occupational therapy are frequently cited as effective and outcome-focused, and several staff members (including activities leadership and some nurses, therapists, and social workers) receive consistent positive mention for compassion, engagement, and hands-on management. The building's common areas and some rooms have been refreshed and are described as welcoming and hotel-like, and the facility offers dementia accommodations, an on-site salon, and a range of social programming that many residents enjoy.

Despite those strengths, reviews reveal persistent operational weaknesses that affect day-to-day care. Staffing instability and high turnover create inconsistent coverage across shifts; reviewers commonly describe slower response times to call buttons and care requests, especially on nights and weekends. These staffing challenges are associated with gaps in personal-care routines (including bathing and toileting assistance), missed or delayed medication administration and reconciliation problems, and inconsistent therapy delivery. Several accounts also point to lapses in infection-control practices and sterile technique and to uneven housekeeping and laundry processes, which generate sanitation and odor concerns in certain units.

Dining and dietary management are areas of mixed performance. Some residents find meals well balanced and enjoyable, while others report frequent cold plates, incorrect meal delivery, and diet-management errors that pose risks for residents with diabetes, anticoagulation needs, or specific allergies. Communication and care coordination with families and external providers are likewise inconsistent: families describe both accessible, responsive administrators and occasions of unreturned calls, voicemail problems, and unclear discharge/transfer planning. Limited physician presence on-site and variable clinical oversight amplify concerns about timely escalation of medical issues and hospital transfers.

Notable patterns include a strong dependence on family advocacy to maintain acceptable care for some residents, improved conditions when management is actively present, and recurring weekend/night variability in service quality. There are also repeated comments about property-handling and security processes (missing or misplaced belongings, unplanned room moves) that suggest gaps in administrative systems. Prospective residents and families should balance the facility's clear strengths in rehab, activities, and individual staff members against operational risks tied to staffing, medication and dietary controls, sanitation practices, and communication. If considering Green Oaks, ask specific questions about current staffing levels across all shifts, medication-safety protocols, infection-control audits, weekend coverage, and recent corrective actions taken by administration; arrange an in-person visit during the times you expect most contact (evenings or weekends) and request references from recent families when possible.

About Green Oaks Nursing and Rehabilitation

Green Oaks Nursing & Rehab, which some might also know as Green Oaks Nursing and Rehabilitation Center, is a nursing home that's been in business since April 2002 and sits mostly on the southwest side of Arlington, Texas, in a quiet, older neighborhood, though there's another spot up in Austin at 8701 N Mopac Expy, which means they've got more than one location if you're looking across the state. It's managed by Mr. Tom Tolleson as Administrator, and it's officially run by Hmg Park Manor Of Green Oaks, LLC since October 2021, but Winnie Stowell Hospital District owns all of it, and there's some ties to Trisun Healthcare, LLC and Hmg Healthcare, too, so there's some bigger organizations involved. The Texas Department of State Health Services is the group in charge of oversight, and the place has passed BBB's basic vetting even though it isn't BBB accredited.

Green Oaks can take in about 100 residents a day and is licensed for up to 142 certified beds, with a nurse turnover rate of 61.4% and about 3.33 nurse hours for every resident, every day, and you'll see a mix of care provided, such as skilled nursing, rehabilitation, and long-term care, as well as independent living, assisted living, and memory care. It's got two therapy gyms stocked with weights, resistance bands, and all kinds of equipment to help people get stronger or recover from illness or injury, plus specialized therapies like occupational, physical, and speech therapy, with respiratory therapy being offered too, and there's 24-hour nurse coverage for those that need help at any time. Short-term care, respite care, and help with daily life-say, bathing, eating, getting around, or medication-are available. There's also wound management and IV services, including safe handling of IV fluids, and customized care plans for each resident, and they've handled infection-related deficiencies according to recent state inspection reports, so folks ought to be aware of where things have been and how things are being improved.

The place has semi-private, well-lit rooms, adjustable beds, and shared seating, with controls for temperature in each room, and the whole environment is decorated with a welcoming entrance, stone accents, a green door, and beautiful landscaping, including an outdoor gazebo with a grill and seating. There's an emergency call system for safety, and residents can use furnished rooms, enjoy restaurant-style meals that can also be customized if needed, and a private dining room for special occasions. Other features are the barber and beauty shop, library, housekeeping, laundry, and a lounge where people visit, as well as activities run by the Activities Director, a photo gallery, and spiritual care from the on-site chaplain. Green Oaks offers case management, nutritional care, and an ombudsman to help with any complaints. Residents' privacy and rights are protected, and they're promised non-discrimination. X-ray and laboratory services are available, and if someone needs enteral therapy, that's something they're set up for, too.

Green Oaks Nursing & Rehab shows dedication to creating a comfortable, caring place that tries to feel like home, with staff on hand by phone or email, and a virtual tour online, and it tries to meet a wide range of health and recovery needs while still keeping its focus on respect and individualized attention.
