    Compassionate attentive rehab; excellent communication

I placed my dad here for rehab and overall I'm very pleased. The nursing and admin teams - especially Rita, Cristina, Esmeralda and Cesar - were attentive, compassionate and went above and beyond; therapy, tasty meals and activities kept him engaged. The facility is clean, well run and communication was good; a few small tweaks would make it perfect, but I feel grateful his care is excellent and would recommend.

    Summary of reviews

Overall impression: Reviews of Ebony Lake Nursing and Rehabilitation Center present a mixed but instructive picture. Numerous families and patients describe strong clinical and rehabilitative care delivered by compassionate nursing and therapy teams, while a subset of reviews raises operational concerns that prospective residents and families should assess during a visit.

Care and staff: The dominant positive theme is the quality of interpersonal care. Nurses, certified nursing assistants, therapy staff, and members of admissions and administration receive repeated praise for compassion, advocacy, and going beyond routine duties to assist families and residents. Clinical strengths highlighted include wound care, physical and occupational therapy, and effective coordination for hospital-to-rehab transfers. At the same time, reviewers flagged persistent operational weaknesses: understaffing or uneven staffing on some shifts, delayed responses to call buttons, and at least one noted medication error. These latter items point to areas where clinical processes and staffing consistency merit scrutiny.

Dining and activities: Dietary services and the kitchen staff are frequently commended for tasty meals and an attentive dietary manager. Recreational programming (for example, bingo and small events) is cited as engaging and valued by residents. However, there are intermittent concerns about meal quantity and monitoring for certain residents, suggesting variability in how dietary needs and intake are supervised.

Facilities and sanitation: Many comments describe an orderly, attractive facility and well-maintained common areas. Conversely, some reviewers reported sanitation or odor concerns and uneven room upkeep. These contrasting observations indicate that cleanliness and environmental maintenance may be strong in some areas or units and suboptimal in others; prospective visitors should inspect resident rooms and common spaces directly.

Management and operational patterns: Admissions and administrative coordination receive positive marks, including help expediting paperwork and facilitating transfers. Nevertheless, operational gaps emerge as recurring themes: inconsistent staffing levels, unreliable call-button response or emergency-response procedures, medication-management inconsistencies, and occasional omissions in bedside safety equipment (such as missing bed rails). There is also variability in staff communication style and conduct across shifts, ranging from highly caring interactions to reports of brusque or inattentive behavior.

Notable patterns and guidance for families: Ebony Lake appears to offer strong person-centered nursing and rehabilitation services, with staff who build rapport and advocate for residents. At the same time, the facility shows variability in operational reliability—particularly around staffing consistency, emergency responsiveness, sanitation in specific areas, meal monitoring, and some safety equipment. Prospective residents and families should balance the facility's clear strengths (therapy, compassionate nursing, admissions coordination, and active programming) against these operational concerns. Recommended actions before placement include an in-person tour focused on resident rooms and common areas, direct questions about staffing ratios and call-button response times, review of medication-safety protocols, and confirmation of safety equipment and dietary monitoring for the prospective resident’s needs.

About Ebony Lake Nursing and Rehabilitation Center

Ebony Lake Nursing and Rehabilitation Center, founded in 2012 and managed by Wellsential Health, stands as a single-story senior care facility with 122 certified beds and a commitment to providing safe, supportive care for older adults, including those with Alzheimer's or dementia. Residents can choose between private and semi-private rooms, all with thoughtful safety features, easy access to washers, dryers, and kitchens or kitchenettes, and housekeeping included to keep daily life comfortable. The center accepts Medicare, Medicaid, Managed Care, and Private Pay, helping many families have options for payment.

A skilled and caring team delivers 24-hour nursing care, rehabilitation, memory care, and services like wound care, diabetes management, cardiac care, respiratory therapy, and stroke care. Therapies offered include physical, occupational, aquatic, speech, and VitalStim therapy, with both short-term rehabilitation and long-term care. For those who need extra support, staff offer help with walking, wheelchairs, dressing, bathing, grooming, and going to the bathroom. Ebony Lake also helps families decide which level of care fits best, whether it's assisted living, independent living, nursing home care, or specialized memory care services.

The facility gives attention to safety for memory care residents with a secure memory support unit, special programming, and activities matched to those living with dementia or Alzheimer's. Residents take part in regular social activities, arts and crafts, exercise programs, and wellness education, while dining options include prepared meals in a comfortable setting. Ebony Lake arranges transportation for appointments and outings, offers WiFi and cable TV, and has spaces like an activities room, a fitness center, guest parking, a beauty salon or barbershop, and a maintained outdoor courtyard.

There's a focus on resident-centered care and efforts to involve families in planning and decisions, though some reported deficiencies have involved care planning, nutrition, and safeguarding information, along with infection-related issues, but the facility has worked to respond with proper protocols and oversight. Ebony Lake features lower nurse turnover rates than the state average, and nurse-to-resident ratios are close to the state norm, though slightly lower. Affiliated with the non-profit Val Verde County Hospital District, the center values community partnerships and aims to improve comfort and quality of life, providing accreditations and skilled medical staff for those in need of inpatient, rehabilitative, hospice, or even home health care.

Staff help organize move-ins, manage wait lists, and welcome potential residents and their families to tour or visit, either in person or virtually. The atmosphere feels warm and community-oriented for both residents and visiting families.
