    Attentive compassionate staff, excellent care

I'm very pleased with my father's care at Pine Ridge. The staff - Angel, Cheryl, Sarah, Mercy, Sabrina, Brooklyn and Chrissy - were attentive, compassionate and treated residents like family; the facility was spotless with no odors, therapy and rehab were excellent, meals and activities plentiful, and their communication gave me real peace of mind. I happily recommend Pine Ridge.

    Summary of reviews

Overall impression: Reviewers convey a facility with many strengths in hands-on care, therapy, and daily life programming, balanced by discrete operational and communication weaknesses. Numerous family members praised individual staff members and described a warm, family-like atmosphere; others raised concerns about specific interactions and post-incident handling. The pattern suggests generally positive day-to-day caregiving with important opportunities for improvement in administrative processes and safety communication.

Care and staff: Many accounts emphasize compassionate, attentive nursing and caregiver support, with staff frequently described as going "above and beyond." Therapy services (physical and occupational therapy) are consistently noted as helpful and timely, and several commenters named specific providers positively. At the same time, there is variability in staff conduct and bedside communication: some families described unprofessional or belittling interactions. A few accounts also questioned how clinical incidents were investigated and communicated to families, which points to the need for clearer incident-response protocols and consistent staff training on family communication and bedside manner.

Dining, activities, and facility environment: Dining is often characterized as nutritious and accessible, with reviewers praising meal quality and food availability. The activities program, including bingo and live music, receives frequent positive mention and appears to contribute to resident engagement. The physical facility is described as clean, well maintained, and odor-free by multiple reviewers; therapy spaces and common areas were noted as welcoming. Conversely, some reviewers cited limitations in in-room amenities—most notably the lack of room phones or cordless phones—which affected family contact. A few comments referenced perceived outdated equipment or amenities, suggesting opportunities for incremental facility updates.

Management and operational patterns: Admissions and office staff are commonly described as responsive and helpful, and the facility appears to serve as a valued community resource. However, operational issues appear in several domains: billing and coverage explanations (including hospice/Medicare questions) caused confusion for families; administrative responsiveness varied across cases; and there are indications of uneven application of safety and transfer procedures. Importantly, a small number of serious safety-related complaints regarding resident injury and subsequent follow-up were described; these raise concerns about incident investigation transparency and the need to reinforce transfer and fall-prevention practices.

Conclusion: For prospective residents and families, Pine Ridge Health Care offers strong day-to-day caregiving, engaging activities, solid therapy services, and a clean, community-oriented facility. To complement those strengths, the facility would benefit from focused improvements in consistent staff communication and bedside manner, clearer billing and coverage communication, enhanced in-room communication capabilities, and tightened incident-response and resident-safety protocols. These targeted changes would help align administrative practices with the high level of hands-on care that many reviewers describe.

About Pine Ridge Health Care, LLP

Pine Ridge Health Care, LLP runs a senior living community in Livingston, Texas, at 1620 Highway 59 Loop N, where folks can get skilled nursing, long-term care, rehabilitation, assisted living, and memory care. This place feels small and close-knit, since it only has enough room for up to sixteen residents in a secure setting, which makes it good for people with Alzheimer's or dementia who need extra safety and personal care, and everyone gets their own suite with a private bathroom, air-conditioning, cable, and Wi-Fi, not too different from a hotel room and with some small kitchen space too. There's help with daily activities like getting in and out of the bath, dressing, grooming, taking medicine, and getting meals, and people can choose to join group things like bingo, music programs, craft workshops, movie nights, fitness classes, or outside trips, so you see friends and keep busy, and the staff, like Director of Nursing Zachary McDaniel and Medical Specialist Ashley Mosley, work day and night, with folks like MDS Coordinators Sabrina Welch and Jamie Watts helping with personalized care and treatments, sometimes for those getting over illness or surgery, or for hospice needs too.

This place has always aimed to be welcoming and treats everyone as family, with regular activities, social events, and meals where the kitchen can handle special diets for things like high blood pressure or diabetes, and sometimes people even bring pets because the place allows it, plus there's a reading room, a beauty shop, a garden, and spaces to sit outside or do games indoors. It has a strong rating, earning 5 stars from Medicare and 4.6 out of 5 based on thousands of reviews, and locals say the staff is friendly, courteous, and attentive, which helps with peace of mind when someone needs memory care or more supervision, since the doors stay secure to prevent wandering. Nursing and rehab services cover physical, speech, and occupational therapy, and people can use transportation for errands, appointments, or services, whether the community provides it or arranges it, and the home can also offer short-term respite care when a main caregiver needs a break. The monthly base price for memory care starts at $4,500, though the total cost might go up depending on how much care someone needs or what kind of room and amenities they pick, and anyone interested can confirm activities and see the place by taking a tour.

Departments and staff have clear roles, with Leslie Beane and Marissa Hernandez handling meals and cleaning, Amber Cates keeping medical records, Madison Robbins in community relations, Veronica Baker as assistant administrator, Lona Lilley with accounts, Activity Director Lori Brandon making the calendar, and Connie Allen offering social work. Pine Ridge Health Care, LLP has won several awards for care and service, and is considered a top long-term rehabilitation place in Polk County for several years. The facility focuses on helping folks keep as much independence as possible, whether they're moving in for a short recovery or looking for a home that provides nursing support for life.
