    Friendly staff and excellent care

I'm very pleased with my experience - the staff are friendly, smiling, and genuinely attentive, the rehab/therapy team helped my loved one regain strength, and the activities, gym and soup-and-salad dining keep residents engaged. The building is clean, well-maintained and welcoming, and the nurses, therapists and front-desk are professional and caring - I'd recommend this facility for rehab or long-term care.

    Summary of reviews

Timberwood Nursing & Rehabilitation Center generates largely positive impressions around its clinical rehabilitation capability, front-line caregiving, and communal atmosphere. Many families emphasize the strength of the therapy program and physical therapists, describing purposeful rehab that aims to return residents home. Nursing staff and select leaders receive repeated praise for compassionate, attentive care, 24/7 coverage, and individualized attention; several reviewers specifically highlighted nursing leadership and named employees as contributors to good outcomes. The facility's renovations, fitness area, attractive dining room, and lobby features (for example, a fish tank) contribute to a welcoming, home-like environment for residents.

Dining and activities are clear strengths for many residents. The facility offers an active activities calendar (bingo, board games, fitness classes) and an engaged activities department that supports socialization and resident morale. The dining experience is often described positively, with a notable soup-and-salad bar and some highly regarded menu items; dietary staff received individual commendations. Amenities such as an on-site beauty salon, guest Wi‑Fi, 24-hour housekeeping, and proximity to a hospital add practical convenience for residents and families.

Despite many positive comments, reviewers describe recurring operational weaknesses. Staffing consistency and coverage are a frequent concern: understaffing or uneven staff performance were linked to delays in medication, therapy scheduling, and clinical follow-through. Communication gaps among nursing, therapy, and physician teams — and between staff and families — appear as a persistent pattern that can affect care coordination and family confidence. Multiple reviewers also described variability in meal quality and in promised services (for example, scheduled specialty visits or timely therapies) not always being delivered as expected. There are also isolated but serious clinical-safety and supervision concerns cited, including falls and delayed responses that suggest areas for improvement in fall prevention and shift supervision practices.

Facility operations and management issues emerge in several areas. A few reviewers described billing or post-discharge financial communication problems and difficulties obtaining clear explanations for charges. Environmental and maintenance inconsistencies were noted as well: while many praised the cleanliness and pleasant smell on first impressions, others described odor issues in particular areas and occasional humidity or moldy/musty smells. Also, several reviews raise concerns about individual staff conduct and professionalism, including an allegation of discriminatory behavior; these comments point to a need for staff retraining and stronger accountability mechanisms where warranted.

In summary, Timberwood presents as a facility with substantive strengths in rehabilitation, therapy, and a generally compassionate caregiving culture, supported by recent physical upgrades and a robust activities program. Prospective residents and families should weigh these strong elements against patterns of operational variability: ask targeted questions about current staffing ratios, communication protocols with families and physicians, medication-administration procedures, fall-prevention measures, billing practices, and how the facility addresses complaints and staff conduct issues. An on-site tour that includes the therapy gym, dining during a meal period, and conversations with nursing leadership can help assess whether Timberwood’s strengths align with a specific resident’s care priorities.

About Timberwood Nursing & Rehabilitation Center

Timberwood Nursing & Rehabilitation Center sits right in Livingston, Texas, and the building has a clean and welcoming feeling with pleasant outdoor spaces and beautiful artwork throughout, and folks there can choose between studio or companion apartments, so there's a little flexibility for different needs. The center serves both short- and long-term residents with 120 certified beds but usually about 84 folks living there every day, which keeps things from feeling too crowded. Nurses provide care around the clock, though nurse staffing sits below the state average at 2.98 nurse hours per resident per day, and nurse turnover is actually lower than the state average, which means staff stick around longer than usual in most places. Timberwood's owned by a government hospital district and connects with the Livingston-Polk County Chamber of Commerce, and it's managed by Livingston Care Associates, Inc. since 2015 with several folks such as Christopher Barley, Soon Burnam, and Charles Stratton listed in leadership roles over the years.

Meals come from chefs and meal planners who use good ingredients, served in a light-filled dining room, although there've been some issues before with food sourcing and safe preparation that were noted in inspection reports. The center offers skilled nursing and rehabilitation, including care for folks recovering from surgery, joint replacements, stroke, and wounds, with occupational, physical, respiratory, and speech therapies. Therapy programs are personalized, combining newer technology with the hands-on attention most people expect, set up by working closely with residents along with their families and doctors. Amenities for daily life include art programs, cards, games, movies, social events, worship services, and transportation, so there's always something to do whether a person likes being busy or prefers peace and quiet. Residents can spend time on the patio or in comfortable rooms, which offer views of the surroundings.

The biggest challenges at Timberwood show up in state inspection reports, which note ten total deficiencies including two for infection control-with particular mentions about safe IV fluid administration, infection prevention, nutrition, and quality of life-so the facility's working through those and keeping a focus on cleanliness and comfort. Folks considering Timberwood can check the home's Medicare page for more inspection results and more detail about care quality. The staff tries to build every care plan around each person, aiming to help folks feel at home and supported during both the easy days and the harder ones. Timberwood falls under health and medical services as a skilled nursing and rehab center, welcoming seniors from Livingston and the wider area who need care and a place to recover or to call home long term.
