    Responsive staff and excellent care

I'm very pleased with my loved one's stay - Administrator Dana and the admin team were responsive, staff were warm and professional, and the building felt clean and home-like. Nursing and therapy (including excellent respiratory/trach care), good food, strong communication, and obvious ongoing improvements give me confidence to recommend this facility.

    Summary of reviews

Reviews present a divided but information-rich picture of care at this facility. On the positive side, many accounts highlight a capable clinical and therapy workforce: experienced respiratory therapists and a strong rehabilitation team are credited with meaningful functional gains, successful ventilator weaning, and tracheostomy management. Several families described effective restorative therapy, good wound-care support, and admissions staff who eased transitions. Facility upgrades and an improving appearance under new leadership were also noted alongside a welcoming dining atmosphere and regular resident activities such as family nights.

At the same time, reviewers describe persistent operational weaknesses that affect safety and family trust. Staffing levels and coverage are characterized as inconsistent, with consequences for responsiveness to call lights and routine care tasks. Multiple accounts raise concerns about medication-management practices and timing, and there are reports of delays or failures in arranging transfers to higher-acuity care when recommended by treating physicians. Communication gaps appear across clinical and administrative channels: phone lines, follow-up on billing and refunds, and clarity around care plans were frequently cited as problematic.

Facility infrastructure and housekeeping present mixed impressions. Several reviewers praised recent cleanliness improvements and renovations, while others noted sanitation and odor concerns in particular units and described overcrowded rooms and bathrooms that challenge accessibility and safe mobility. Dining and activity programming earn consistent praise for food quality, balanced meals, and social engagement opportunities, which families associated with improved resident morale.

Management and culture appear to be in transition. New administrators and nursing leadership receive positive comments for responsiveness and a renewed focus on resident well-being; these changes correlate with reports of improved morale among some staff. Conversely, there are recurring accounts of unprofessional staff conduct, payroll and employee-management issues, and uneven enforcement of standards that appear to undermine consistency of care.

Notable patterns: the experience at this facility is highly polarized—some families describe life-changing rehabilitation and attentive care, while others recount serious care-process failures and poor communication. There is evidence of organizational improvement under newer leadership, but several operational risks remain salient: staffing reliability, medication controls, emergency-transfer coordination, restroom accessibility, and administrative responsiveness. Prospective families should confirm current staffing ratios, medication and emergency-transfer protocols, sanitation and accessibility accommodations, and billing practices during a facility tour and care-planning discussion.

About Southern Specialty Rehabilitation and Nursing

Southern Specialty Rehabilitation and Nursing sits under the management of Lubbock I Enterprises, LLC, with Gary Blake and Malisa Blake involved, and it's run for profit. The place can serve up to 144 people, but usually tends to about 69 residents at a time, and while it's part of Creative Solutions in Healthcare, it's got people of all ages coming through, not just older folks, so there's a real mix. The facility provides short-term and long-term skilled nursing, rehab care, ventilator services, memory care, dialysis right inside the building, and there's support for things like speech therapy, wound care, spinal cord and brain injury, and recovery from strokes, surgery, or heart and diabetes problems; folks can get help with podiatry, dentistry, optometry, hospice, and medicine, and daily needs like meals, laundry, housekeeping, and transportation get handled too. Southern Specialty runs a rapid recovery unit, brings in a team for vent care with 24/7 respiratory therapists on hand, and supports veterans, making plans for each resident so care matches the person. There's a nurse turnover rate of 64.4%, which runs higher than Texas' average, but the place has 3.77 nurse hours per resident per day, which is a bit above the state's 3.4 hours, and staff keep up services like pulse oximeter monitoring, IV support, and help with dignity around continence care. Folks can get involved with activities like Bingo, volleyball, arts and crafts, and holiday events, and there's special attention to environmental sustainability worked into daily routines as well as an online portal for families to keep up with how their loved ones are doing, and a customer care line for feedback or concerns. Meals get tailored to local tastes with a focus on nutrition, and there's a White Glove Program that aims for extra cleanliness and infection control, although the facility has had its share of citations in recent years: in the standard April 15, 2025 inspection, three deficiencies showed up-one for infection control-and over time, 26 deficiencies got reported, including five tied to infection control, and there are past citations for not buying or serving food the right way, issues with incontinence and catheter care, and not always handling infection prevention as it's supposed to. Even so, staff stay focused on making daily life as home-like and respectful as possible, supporting residents with everyday living and social needs.
