    Compassionate staff and excellent communication

I moved my mom here and I'm very pleased. The staff are wonderful-compassionate, attentive, and accommodating-and helped with feeding and rehab until she was walking and feeding herself. The facility is clean, well-maintained, offers many activities, good meals/snacks, and excellent family communication; I would recommend it, with only small room for improvement.

    Summary of reviews

Overall impression Reviews reflect a facility with a generally positive interpersonal culture and clear strengths in one-to-one care and rehabilitation, alongside operational challenges tied to the building and staffing. Families frequently highlight compassionate, communicative nursing and therapy teams and describe successful restorative outcomes for some residents. At the same time, reviewers identify recurring operational issues that affect day-to-day resident experience.

Care and staff The predominant theme is strong, empathetic caregiving: many accounts describe attentive nurses, helpful aides, and staff who communicate well with families. Rehabilitation services receive praise in multiple accounts, with examples of residents regaining mobility and feeding independence. However, there is a noticeable pattern of inconsistent staffing levels and periods of reduced responsiveness; reviewers describe delays in attending to resident needs and variability in the timeliness of clinical assistance. These operational gaps appear to drive the most significant family concerns despite generally positive comments about staff attitude and compassion.

Dining and activities Dining impressions are mixed. Several reviewers characterize meals as decent or acceptable and note the availability of snacks and beverages; others describe food quality as inconsistent. Feeding assistance is available and has been helpful for residents who need it. The facility offers social programming and a range of activities, which families report as valuable for resident engagement and social connection, contributing to a home-like atmosphere for many residents.

Facilities and environment The building is described as older but clean and reasonably well maintained, with some units or apartments remodeled. Room configurations include private and two-person rooms; however, several comments point to small room footprints and limited closet/storage space, and a subset of private rooms have shared-bathroom arrangements that some families found inconvenient. Cleanliness of common areas is generally noted positively, but the aging physical plant contributes to mixed impressions about comfort and layout.

Management and notable patterns Management strengths include family communication and accommodating staff when they are available; some families specifically note peace of mind as an outcome of the facility's communication and attentiveness. Operational weaknesses cluster around staffing consistency, response times to call lights or requests, irregular bathing and personal-care scheduling, and variability in therapy outcomes and meal quality. Cost or affordability is also mentioned as a concern by a few families.

Practical takeaways for prospective families The facility appears to deliver strong interpersonal care and effective rehabilitation for many residents, within an older but clean environment that offers both private and shared-room options. Prospective families should verify staffing patterns during the times their relative will need the most support, ask about call-light response metrics and bathing/personal-care schedules, tour sample rooms to assess storage and bathroom layouts, and sample meals if dining quality is a priority. Clarifying expected therapy goals and costs up front may help set realistic expectations given the variability noted in outcomes and service consistency.

    About Crescent Health Care

Crescent Health Care started back in 2005 and sits at 505 N 40th Ave, where it operates as a nursing home with 85 Medicare-certified skilled nursing beds, and it's known locally for helpful staff and a friendly, welcoming atmosphere. The place offers inpatient nursing and rehab services, so you'll find skilled caregivers and nurses who handle everything from wound care and medication management to catheter care, incontinence services, G-tube feeding, and diabetic support, making sure needs are met for a wide range of health concerns. People who need help with daily activities like dressing, bathing, grooming, or managing mobility get support from trained aides, while those recovering from hospital stays can use short-term rehab with physical, occupational, and speech therapy right on site, so seniors don't have to leave for appointments. There's also specialized care for folks facing brain or spinal cord injuries, and those living with Alzheimer's or dementia have access to a memory care unit designed to prevent wandering and confusion, with safety features and staff trained in gentle care for memory loss.

Meals come three times a day, freshly made with options for special diets, and there's always someone making sure food meets nutritional needs or personal and cultural preferences. Caregivers provide laundry and housekeeping, and there's an on-site hairdresser by appointment, plus amenities that include common rooms for games, crafts, and group activities like bingo, outings, or even live performances, so residents find ways to stay engaged and connected. The place also makes education, wellness, and spiritual care available through programs, chapel services, and Bible study, all aiming to support body, mind, and spirit. Whether someone wants a private or semi-private room, Crescent Health Care gives choices, including assisted living, independent living, memory care, or even residential care homes with a live-in caregiver for those needing a more home-like setting. Residents can use property features like dining rooms, activity rooms, a fitness center, salon, WiFi, cable TV, and laundry facilities, while safety and accessibility features make moving around easier for anyone with mobility issues.

Crescent Health Care prepares individualized care plans and regular assessments, helps with transportation, keeps up with regular vitals and medication reminders, and talks openly about costs, payment options, and insurance coverage for those planning long-term care. The administration, including Stephanie Woodcock, makes sure the community keeps striving for improvement with awards and recognition for excellent activities, plus caregiving resources for families. Community feedback stays positive, with people mentioning the kind, joyful culture among staff and residents, and the facility holds a 4.1 rating from reviews. For folks who need care for physical, emotional, or mental health changes, or who want somewhere safe and social as they age, Crescent Health Care covers many types of senior living and nursing care under one roof.
