    Compassionate professional staff and rehab

I was very satisfied with the caring, professional staff-nurses, CNAs, administration and social work-and the outstanding PT/OT rehab (Amy, Stepha, Joanne and Deanna) that set clear goals and produced real progress. They offered compassionate hospice support, helpful placement/discharge planning, deaf-friendly communication and good meals; I felt respected and supported and would choose them again.

    Summary of reviews

Reviewer feedback for Crossroads Care Center of Green Bay West is mixed, with a consistent pattern of strong rehabilitation services and caring frontline staff alongside operational and clinical reliability concerns. Many accounts highlight the facility's capacity to deliver effective PT/OT and goal-focused therapy, with therapists described as motivating and outcomes often showing measurable progress. Families and patients frequently commended CNAs, nurses, social-work and administrative staff for compassion, dignity-preserving interactions, and helpful discharge and placement assistance. The facility also offers deaf-friendly intake and communication options, and some reviewers reported attentive family communication and solid hospice support.

At the same time, several operational issues recur across reviews. Communication failures—both between departments and with families—are a prominent theme, manifesting as a difficult phone system, dropped or unanswered calls, and challenges reaching residents. Call-bell response delays and understaffing were cited as contributors to slow responsiveness. Reviews indicate concerns about clinical consistency: medication coordination and oversight have been described as inconsistent, and there are reports of unreliable monitoring equipment and discrepant vital-sign readings. These clinical and equipment reliability issues translate into perceived safety and timeliness gaps in care for some residents.

Dining and activities received mixed feedback. Some residents found the food satisfactory or very good and praised the social programming and activities; others noted inconsistent meal quality and a desire for more menu options. Facilities impressions are likewise variable: several reviewers described clean, well-maintained areas and a pleasant setting, while others raised sanitation and bathroom-cleanliness concerns and issues with laundry/property handling (for example, missing personal items).

Management and administrative responsiveness also appear uneven. Multiple reviews praised individual staff members — administrators, placement coordinators, and social workers — for being helpful and communicative, but other accounts described variable responsiveness and difficulty resolving problems. Visitation procedures were described as restrictive or poorly managed in some instances, creating frustration for families trying to maintain contact.

Overall, the strongest and most consistent positives are the rehabilitation program and many frontline caregivers' compassion and professionalism. The most significant operational liabilities are interdepartmental and family-communication failures, staffing shortages that affect responsiveness, inconsistencies in clinical oversight and equipment reliability, and uneven sanitation and laundry practices. Prospective residents and families may want to ask the facility about current staffing levels, call-bell response times, medication-reconciliation and equipment-maintenance protocols, visitation policies, and interpreter services to validate whether recent improvements or corrective actions have been implemented.

About Crossroads Care Center of Green Bay West

Crossroads Care Center of Green Bay West sits on Shawano Avenue near St. Mary's Hospital Medical Center in Green Bay, Wisconsin, and has served as a skilled nursing facility for many years, offering both long-term and short-term care to adults needing extra help with daily living, and with 105 beds according to state records, the place once provided services as a site where residents had to move out when it closed for sale to Hospital Sisters Health System. The nursing home has been known for its compassionate, patient-centered care, where the staff aim to help people feel comfortable and respected, making room for them to feel as independent as possible while always keeping someone on hand to help out with what's needed, whether it's getting dressed, taking medicine, or joining in on activities and therapy sessions. Crossroads cares for people needing skilled nursing services and specializes in rehabilitation, offering physical, speech, and occupational therapy to support recovery and everyday function-along with wound care and help for folks with diabetes or kidney problems through offerings like on-site dialysis, and it's designed to assist those needing memory care, including people with Alzheimer's or dementia, so families see various support systems for their loved ones.

Meals are planned by dietary staff to keep residents healthy, and people can get hospice or palliative care if they're dealing with major illness or at the end of life, and respite care is also available for short stays. There's always a mix of registered nurses, LPNs, CNAs, and therapists in the building, with a relatively high staff-to-resident ratio, and the facility says its care is tailored to each person's physical, emotional, and social needs, and while the nursing home has gotten nursing and facility inspection grades over the years-ranging from B+ to F in nurse quality and an overall facility grade of C with no major safety deficiencies in the most serious inspection categories-it tries to keep up with CDC and government safety guidelines to make sure people stay safe. Residents at Crossroads Care Center of Green Bay West take part in activities and have some fine dining experiences, and the rooms are designed for comfort as much as possible while still making sure all the medical needs are met.

People can expect individualized care, 24/7 nursing and therapy support, and access to therapy and specialty services like wound treatment and diabetes management, though the facility currently isn't accepting new patients and only provides information in English since there's no note about other languages. With a rating of 3.3 from forty reviews, Crossroads Care Center of Green Bay West is a place where people can find skilled healthcare, therapy, meals, and accommodations while being close to hospital resources if they're needed, and it's always aimed to put resident dignity and well-being at the center of its approach, right in the heart of the Green Bay community.
