    Clean quiet home-like caring staff

I've had a very positive experience here - a clean, quiet, home-like place where staff treat me like a queen. Nurses, CNAs, and dietary staff are loving, patient, and go above and beyond (even researching solutions); meals are tasty, birthdays are personalized, activities are frequent, and I'm grateful for the strong, friendly team in 608 - I highly recommend it.

    Summary of reviews

Reviews present a mixed picture in which individualized, compassionate care coexists with operational and compliance challenges. Multiple accounts praise the bedside manner of nurses and CNAs, noting attentive assistance, willingness to research solutions for complex care needs, and personalized touches such as birthday celebrations and community events. Several families describe the environment in positive termsโ€”clean, quiet and home-like in parts of the buildingโ€”and highlight a strong activities program and community involvement, including social events that engage residents.

At the same time, a number of consistent operational concerns emerge. Staffing levels and turnover are recurring issues, which reviewers link to delays in responding to call bells, long wait times for assistance, and inconsistent help with feeding and personal care tasks. There are specific process gaps described around wound care and dressing changes, suggesting uneven clinical follow-through. Reviewers also cite cleanliness and maintenance shortcomings in parts of the facility, creating sanitation concerns that warrant attention.

Dining is another mixed area. Some reviewers compliment meal taste and the dietary staff's patience, while others describe problems with meal-service continuity and accessibilityโ€”examples include the need to request basic items at the point of service and food textures that are not consistently adapted for older adults or those with chewing/swallowing needs. These accounts point to reliability and menu-consistency problems rather than uniformly poor cuisine.

Management and oversight concerns are reflected in mentions of high staff turnover and a state survey with multiple deficiencies; these items indicate systemic issues that could affect quality and consistency. Quality appears to vary by unit, with specific areas (for example, the 608 area) singled out for particularly strong staffing and care. This variability suggests that prospective residents and families may have very different experiences depending on room/unit assignment and the current staffing mix.

For prospective residents and families: consider an in-person visit focused on observing staffing levels during peak times, reviewing the facilityโ€™s most recent state survey and corrective-action plans, asking about wound-care protocols and feeding assistance procedures, and sampling a meal. These targeted checks will help clarify whether the facilityโ€™s strengthsโ€”compassionate caregivers, active programming, and parts of the building that feel home-likeโ€”are consistent with the specific unit and care needs you anticipate.

    About Mineral Point Health Services

Mineral Point Health Services sits in Mineral Point, Wisconsin, and works as a Skilled Nursing Facility with 30 certified beds, helping about 27 residents daily. This place offers skilled nursing and intermediate care for those who need a lot of help and for folks trying to get strength back before heading home from the hospital, and there's help for both short-term rehab and longer stays. They've got private rooms, a garden bed for residents, an outdoor covered patio, a sunroom, a chapel, and a library, so there's a good mix of spaces to either sit quietly with a book or spend some time outdoors. You'll find 24/7 admissions, so folks can come in any time they need to, and partnerships with groups such as Medrina Services, Select Rehab, and the DaVita Dialysis Center keep medical services on hand without needing to travel out. Nursing staff have an average daily turnover of 25.8%, which is lower than the state average, but the total nurse hours per resident per day-3.44-is below the state average.

The facility is managed by North Shore Healthcare LLC under Nshr Operations LLC since December 2019, working as a for-profit limited liability company. Pharmacy services provide full support, but there was a citation for not labeling and storing drugs properly, and the facility was cited for not reporting suspected abuse, neglect, or theft as quickly as rules require. There's been one infection-related deficiency and a total of eight deficiencies reported in state inspections.

Services offered include dental, vision, audiology, podiatry, behavioral health, speech therapy, respiratory therapy, occupational and physical therapy, plus on-site dialysis, wound care with certified wound nurses and wound physicians, and hospital-related infection and accident management. Specialty care like brain injury rehab, home health, hospice, and assisted living services are also available, and telehealth is an option for certain medical needs. The staff can help with discharge planning and recovery information, and the facility runs life enrichment programs and has comprehensive dining services. Folks can use in-house x-ray and lab testing. Infection control and isolation care are available, and there's quick access to an automatic defibrillator. Serious medical events like blood clots or infection complications are rated, and the staff do ongoing monitoring of glucose, weight, and breathing, usually with doctor's orders. Nurse, doctor, and staff communication, as well as cleanliness and quietness, are included in experience ratings. Mineral Point Health Services shows recognition by the Women's Choice Award for focusing on quality care for female residents, and it claims a five-star rating by CMS. The facility states that it tries to treat residents like family in a home-like setting, with support for both medical and social needs.
