    Kind staff, clean and welcoming

I moved my mom here and we're very happy - the staff are consistently kind, helpful, and responsive, management is outstanding, and the facility is clean, well-kept, and welcoming. Spacious, bright apartments (with in-building laundry and sitting rooms on every floor), regular activities and church services, salon visits, and a wooded location give her plenty to enjoy and provide real peace of mind. Staff and maintenance go the extra mile, and I'd highly recommend this place, though there's always room for improvement.

    Summary of reviews

Liberty Village of Tomah presents a mix of clearly stated operational strengths and recurring weaknesses. The facility is frequently described as clean and well-kept, with bright, spacious apartment floorplans, ample storage, and convenient features such as in-building laundry and communal sitting rooms on each floor. Families highlight a welcoming, social environment and note that residents can personalize apartments with their own furniture. Maintenance responsiveness and move-in assistance are commonly praised, contributing to an overall sense of a tidy, comfortable living environment.

Care and staff performance receive generally positive comments: many accounts describe staff as friendly, caring, and attentive. Nursing staff are noted for competent medication management and for communicating changes in residents’ behavior or eating patterns; the facility also coordinates hospice services when required. At the same time, reviewers indicate operational variability in staffing. There are repeated references to short staffing, high turnover, and inconsistent coverage across shifts, which reviewers link to occasional lapses in responsiveness and uneven professionalism among individual employees.

Dining emerges as an inconsistent area. Several reviewers compliment healthy, nutritious meals, good portion sizes, and a large, clean dining area; others describe average-to-poor meal execution (examples include dry entrees and limited menu items). The pattern is one of unpredictable food quality and occasional issues with meal variety and preparation rather than a uniform standard of service.

Activity programming and resident life are strengths. The facility offers a broad slate of activities — crafts, puzzles, Bingo, music, weekly schedules, exercise classes, a library, and regular salon and religious services — and many families indicate residents enjoy and regularly participate in these offerings. A few amenities have experienced maintenance-related interruptions (for example, a soaking tub or a removed workout room), which suggests occasional gaps in amenity upkeep.

Operational and management patterns are mixed. While some reviewers praise proactive, helpful management and office staff, others raise concerns about organizational disarray, managerial turnover, and communication problems. Financially, reviewers cite multiple rate increases within short periods and link these to occupancy fluctuations; this points to pricing instability that prospective residents should evaluate. Security-process lapses are also noted (for example, unsecured exterior doors at night), indicating that campus-entry procedures and evening staffing protocols warrant clarification during a tour. A small number of reviewers mention periodic odor concerns in common areas, suggesting sanitation monitoring could be strengthened in certain locations.

Overall, Liberty Village of Tomah offers many amenities, a robust activities program, and a generally caring staff culture, alongside operational risks tied to staffing consistency, management stability, food-service quality, and some maintenance/security practices. Prospective residents and families would benefit from focused questions during a visit: ask about current staffing ratios and turnover, request a sample meal or recent menu, review the history and policies on rate increases, confirm security procedures for evenings, and inspect any amenity areas important to the resident. That approach will help determine whether the facility’s strengths align with an individual’s priorities and whether the identified operational weaknesses have been addressed.
