    Pervasive neglect and unsafe care

    I placed my aunt here and while a few staff were wonderful - especially Ms. Tamaya and some trustworthy CNAs who showed teamwork and improved my aunt's mood - the facility overall failed her. I saw pervasive neglect: urine/waste odor, flies and mold, a room unclean for days, disgusting/missed meals causing weight loss, soiled sheets, bedsores, and clothes repeatedly lost. Nurses were often unresponsive, delayed pain meds, ignored call lights (sometimes over an hour), and administration was rude, dismissive and uncooperative about care plans or documentation. Unsafe practices (no locks on adjoining restroom doors, unsafe transport, residents left on the floor) led to injury and deterioration. I would not recommend this place and believe it needs immediate outside intervention.
