    Compassionate hands-on care, administrative issues

I placed my mom here and had mixed feelings: the nurses, CNAs and caregivers were warm, attentive and professional, the private room was clean and quiet, and activities/rehab were excellent. On the other hand admin and social work were unhelpful, Medicaid navigation was a problem, and we saw troubling lapses (long gaps without baths, medication delays, a fall with poor supervision, and unclear/no-salt meal info that left my mom refusing to eat). Staff on the floor were wonderful and family-focused, but insist on clear answers about diet, discharge and Medicaid up front. I would recommend the facility for its compassionate hands-on care and rehab - with serious caveats about administration and oversight.

    Summary review

Overall sentiment: Reviews for Self Health Care & Rehab Center are mixed but lean positive overall. A strong majority of reviewers emphasize compassionate, family-like nursing care, attentive one-on-one attention, and an effective rehabilitation program that supports recovery. Many families praise individual clinicians—nurse practitioners, doctors, nurses and CNAs—by name or description, citing high-quality clinical care and personal attention that made residents comfortable and improved outcomes. Multiple reviewers explicitly state they would recommend the facility or would return a loved one there.

Care quality and clinical issues: A dominant theme is that clinical and rehabilitative care can be excellent: rehab is described as “actually taking place,” recovery support is strong, and clinicians go “above and beyond.” However, a notable minority of reviews describe serious lapses. These include medication delays and errors, falls left unattended, delayed medical intervention after injury, and hygiene neglect (for example, residents reportedly not bathed for many days and instances of feces left on commodes). At least one account describes catastrophic consequences after a fall. These contrasting reports indicate that while many experience very good clinical care, there are intermittent but significant safety and quality failures that families should consider and monitor closely.

Staff, culture and management: Many reviewers highlight a warm, family-oriented culture—staff are described as caring, compassionate, and loyal, with personal touches like prayer, hand-holding, birthday parties, and owner visits. The activities director and programming receive frequent praise, contributing to residents’ morale. Several reviewers also note staff willingness to fix problems when issues are raised through the proper channels and praise visible management involvement. Conversely, there are repeated criticisms of administration and social work in some cases—families report poor Medicaid assistance, lack of transparency, coercive recommendations, rude or unprofessional individuals, and inconsistent administrative responses. These mixed reports suggest variability in administrative performance that may depend on individual staff members or circumstances.

Activities, social life and environment: The facility’s social and activity offerings are a clear strength for many residents: Bingo in communal spaces, spa days, hair and nail services, concerts (including themed events), crafts, card-making, and visiting youth choirs and religious programs are frequently cited. These programs often create a lively, engaging environment that families appreciate. Physically, the facility is described as clean, pleasant, and hospital-like by several reviewers, with private rooms, secure porches, and good common areas. A minority mention occasional smell/odor problems, indicating that environmental maintenance may be inconsistent at times.

Dining and dietary concerns: Food receives generally positive remarks—hot meals, specific liked dishes (fish and slaw, ice cream), and personable dietary staff are noted. Yet some families report problems with dietary transparency and accommodation: for example, a no-salt/no-sugar diet was not clearly followed or communicated, resulting in a resident refusing meals. Families with strict dietary needs should verify meal preparation and communication practices on admission.

Rehab, discharge and insurance navigation: Multiple reviewers commend the facility’s rehab services and say they would use them again. At the same time, there are accounts of problematic discharge practices—early discharges, unwillingness to transport residents, and difficulties navigating Medicaid assistance. Social work performance is described as excellent by some and “awful” by others. Prospective families should confirm discharge planning procedures, transportation policies, and Medicaid assistance processes prior to placement.

Patterns and recommendations: The reviews reveal a pattern of generally strong, individualized caregiving and activity programming, offset by intermittent but serious lapses in basic care, administration, and communication. This suggests variability in staffing levels, staff training, or oversight at times. For families considering this facility: visit multiple times and at different hours, meet the clinical team and social worker, ask about staffing ratios, bathing and hygiene schedules, fall-prevention protocols, medication management procedures, and discharge/transport policies. Request written confirmation of dietary accommodations and observe activity programming. If problems arise, use the facility’s formal complaint channels while documenting incidents and escalating as needed.

Bottom line: Self Health Care & Rehab Center receives many strong endorsements for compassionate nursing, effective rehab, and engaging activities, creating a warm, family-like environment for many residents. However, there are consistent, serious concerns from a meaningful minority of reviewers about neglect, safety incidents, medication and hygiene lapses, and inconsistent administrative support. Given this mix, families should weigh the positive caregiving culture against the reported risks, conduct thorough tours and interviews, and maintain active oversight during a loved one’s stay.

The center takes both Medicare and Medicaid, and they keep up with safety and infection control, offering yearly flu shots and strong infection management, with low rates of injuries from falls and hospital visits due to infections, and there's respite care when family caregivers need a break. There's transportation for doctor's visits and other trips, and support for non-ambulatory folks, with personal care always tailored to what each person can do on their own and what they need help with.
