    Dirty unsafe facility, negligent staff

I would not recommend this facility for rehab or medical care. I found it dirty and unsafe, with unresponsive night nurses, rude staff and supervisors, poor communication (including delays notifying family of death), medication and equipment mix-ups, and negligent practices that led to infection and hospitalization. There was no one-on-one care, rooms were dark with full trash cans and no family seating, and discharge once lacked essential equipment - all for an outrageous price. A few therapists and attendants were caring and rehab staff were good, but the overall care, cleanliness, and staff attitude make me avoid this place.

    Summary review

Overall sentiment across reviews for Windsor Manor Rehabilitation Center is mixed but heavily polarized: several reviewers praise the therapy services and individual caregivers, while multiple other reviewers raise serious safety, hygiene, staffing, and communication concerns. The most consistent pattern is variability — some patients and families experienced compassionate, effective rehab and proactive communication, while others reported neglect, unsafe medical practices, and poor facility conditions. This variability appears tied to staffing instability and inconsistent processes.

Care quality and clinical safety: Reviews show a sharp divide. Positive reports highlight excellent, caring staff and strong rehabilitation outcomes; multiple reviewers specifically praised physical therapists and noted successful rehab stays. However, negative accounts include alarming clinical problems: frequent nurse turnover, lack of continuity of care, delays in medication administration, and at least one report of grossly unsafe practice (a catheter reused for months) that preceded hospitalization for infection. Several reviewers explicitly advise avoiding the facility for supervised medical care or rehab because of these safety concerns. There are also reports of lack of one-on-one nursing attention and night nurses rarely checking in, indicating potential staffing shortages or poor supervisory oversight.

Staff, communication, and management: Some families experienced proactive, communicative staff — timely updates, multiple points of contact, and even video visits — and a supervisor who managed scheduling issues by providing an extra week at no charge. In contrast, other reviewers reported miscommunication about contact information, delays in notifying families about serious events (including delayed notification of a death), rude or unfriendly frontline staff at the desk, and an overall dismissive attitude toward concerns. High turnover among nurses and inconsistent communication channels are recurring themes that likely contribute to the uneven experiences.

Facilities, cleanliness, and safety concerns: Accounts of the physical environment are mixed. A few reviewers described the facility as clean and well-maintained, while several others reported extremely unsanitary conditions, dark rooms with full trash cans on arrival, worn-out furniture and beds, and general aesthetic decline. These conflicting impressions reinforce the broader pattern of inconsistent standards — cleanliness and maintenance may vary by unit, shift, or time period. Safety-related issues — such as discharge without essential equipment (hospital bed, lift) and reports of negligent clinical practices — are among the most serious complaints and warrant careful consideration by prospective residents and families.

Dining, activities, and comfort: Food quality is another mixed area. Some reviewers complimented the food and called it nice, while others strongly criticized the dining as terrible. Activities are available according to reviews, but at least one resident had not been able to participate yet, suggesting variability in activity access or individual eligibility. Comfort issues such as poor-quality beds, slow response to calls, and lack of family seating in rooms were commonly noted and contribute to dissatisfaction among several reviewers.

Patterns, risk indicators, and recommendations for prospective families: The dominant pattern is inconsistency — positive experiences (good rehab outcomes, caring therapists, proactive communication) exist alongside serious negative reports (unsafe medical practices, unsanitary conditions, rude staff, medication and equipment failures). Recurring risk indicators include frequent nurse turnover, documented clinical negligence (infection related to catheter reuse), discharge without promised medical equipment, and communication breakdowns with families. These issues collectively suggest systemic problems in staffing stability, infection control, and management oversight.

If considering Windsor Manor, prospective residents and families should verify current staffing levels and turnover, ask about infection-control practices and medication administration protocols, confirm discharge planning procedures (including guaranteed equipment delivery), request references or recent inspection reports, and inquire how the facility communicates with families, especially after-hours. The facility appears capable of providing high-quality rehab for some patients, but the reported variability and several serious safety incidents mean that careful, case-specific due diligence is strongly advised before placement.
