    Clean facility but serious neglect

I have mixed feelings: the building and rooms are clean and spacious, the courtyard is beautiful, and several staff were loving, professional, and kept my loved one comfortable. However, I witnessed dangerous lapses - rude/abusive therapists, uncaring or hurried staff, delayed call responses, dirty bedpans, untreated bedsores, an infection that required hospitalization, roach and hygiene issues, ignored dietary needs, missed/undelivered therapy, and administration that didn't follow up. A few caregivers are wonderful, but the serious neglect and poor management left me worried and I can't recommend without major reservations.

    Summary review

Overall sentiment across the reviews is strongly mixed and polarized: many families describe extremely positive experiences centered on caring staff, attractive facilities, and a home-like environment, while other reviewers report serious quality and safety failures including neglect, poor hygiene, and abusive behavior. The volume and severity of negative reports are notable and describe issues that could pose clinical risk (untreated pressure ulcers, dehydration, infections) alongside operational and interpersonal failures (unresponsive staff, rude behavior, inadequate documentation).

Care quality and clinical concerns are a central pattern. Positive comments frequently highlight attentive nurses who kept residents comfortable and professional staff who provided needed care. Conversely, multiple reviewers report serious lapses: therapy promised but not delivered, untreated bedsores, infections requiring hospital care, dehydration, insufficient fluids, lack of physician evaluation, and incomplete or outdated charts. Several reviews mention administration not following up on clinical concerns and potential negligence, indicating unresolved escalation pathways. These clinical and documentation problems suggest variability in clinical oversight and consistency of care.

Staff behavior and staffing levels show a stark divide. Many reviewers praise specific caregivers as caring, proactive, and honest; several families say their loved ones were well looked after and that staff were friendly. At the same time, other reviews describe rude or abusive therapists, staff yelling at residents, scolding for basic self-care (e.g., brushing teeth), and dismissive in-charge nurses. Recurrent reports of delayed call-light responses, understaffing, and unreturned phone calls indicate staffing shortages or workflow issues that negatively affect response times and daily care.

Facility condition and cleanliness comments are also mixed. Positive reviews describe a neat, attractive facility with a beautiful courtyard, well-decorated meals and holiday events, secure doors, visibility to nurse stations, and pleasant communal spaces such as a game room. Negative reviews, however, raise serious sanitation concerns: roach infestation, dirty bedpans, poor personal hygiene management, piles of dirty clothes, and generally rundown indoor areas. This split suggests that cleanliness and environmental maintenance may be inconsistent across units or change over time.

Dining and dietary management receive both praise and criticism. Several families report tasty, healthy food and festive meal presentations, while others criticize the menu as starchy, unappealing, or unhealthy. More critically, at least one review indicates failure to follow dietary restrictions for a resident with kidney disease, a specific and potentially dangerous lapse in individualized dietary management.

Management, communication, and policy issues are frequent themes. COVID-related visiting restrictions were mentioned as a communication barrier and impediment to contacting staff. Some reviewers report administration’s lack of follow-up on complaints and poor record-keeping (charts not updated). Privacy concerns (shared phones) and inconsistent enforcement of policies are also noted. A few reviewers explicitly considered or mentioned potential legal action, reflecting high frustration with unresolved safety or neglect issues.

In summary, Destin Healthcare And Rehabilitation Center receives both strong praise and serious criticism. Positive reports consistently point to attractive grounds, a secure facility, engaging communal activities, and many compassionate caregivers who create a home-like environment. Negative reports emphasize inconsistent staffing and supervision, serious clinical lapses (wounds, infections, dehydration), hygiene and pest problems, dietary mismanagement, rude or abusive staff in some cases, and poor communication or administrative follow-through. The pattern indicates variability in resident experience—some families encounter reliable, high-quality care, while others face substantial risks and neglect.

For prospective residents and families, these reviews suggest it is important to verify current staffing ratios, infection-control measures, how individualized care plans (including diet and therapy) are implemented, and how complaints are handled. Observing staff-resident interactions, asking for recent inspection results, reviewing care plans and charting practices, and requesting documentation of therapy schedules and dietary accommodations are prudent steps. Regular visits and clear lines of communication with management may help detect and address problems early given the inconsistency reflected in these reviews.
