    Filthy facility, neglect, injury, unsafe

    I moved my loved one out after 2 days. The private room reeked of urine, was filthy with roaches and a deplorable bathroom. Staff were inattentive, poorly trained for dementia, missed meds and bathroom assistance - my relative was dropped and suffered a broken toe; there were safety lapses that left me devastated. Rehab/physical therapy and a few cooperative staff were good, but overall cleanliness, staffing and management were unacceptable.
