    Good rehab, poor long-term care

My experience was mixed. The facility is clean with nice rooms, pleasant activities and a strong rehab team - some nurses were kind, caring and knowledgeable and helped improve mobility. However, nursing was inconsistent and often unresponsive (long call-light delays), communication and accountability were poor, medications were delayed, belongings went missing, and wound care/skin issues were neglected-my relative was even transferred to hospital and hospice. I might use them for short-term rehab, but I would not trust them for long-term nursing care.

    Summary review

Overall impression: Reviews for Port Orange Nursing And Rehab Center are highly mixed, with a clear split between strong praise for rehabilitation and certain frontline caregivers and severe criticism about nursing responsiveness, communication, and some aspects of clinical care. Many reviewers highlighted positive experiences around therapy, aides, and activities, while other reviewers described alarming lapses in basic nursing care, wound management, and resident respect. The result is a polarized picture in which families may find excellent rehab outcomes and compassionate support from certain staff but also risk encountering critical problems depending on timing, staffing, and unit.

Care quality and clinical concerns: A recurring pattern is that the rehabilitation department and several CNAs receive consistent praise—families noted mobility improvements after short stays, knowledgeable staff, and compassionate hands-on care. However, multiple reviews raise serious clinical concerns: pain reportedly went unaddressed, wound care was unclear or apparently not provided, and at least one patient was left unattended for extended periods and later transferred to hospital/hospice and passed away. These are significant safety-related issues. Several reviewers also reported sores, dried blood, and lack of notification about skin tears. Medication timing delays and slow call-light responses (reported as 20–30 minutes) further contribute to concerns about timely medical attention.

Staffing, communication, and accountability: Communication and staff accountability are prominent negative themes. Families complained about unanswered questions, poor communication with medical staff, and being yelled at when asking for help. There is a notable distinction between weekday and weekend experiences in reviews; weekend staff were described as especially poor in some accounts. Conversely, other reviews emphasize accommodating, kind, and knowledgeable nurses and staff who "do their best," indicating variability by shift, individual staff member, or unit. This inconsistency suggests issues with staffing levels, training, supervision, or culture that lead to uneven resident experiences.

Facilities, cleanliness, and environment: Multiple reviewers report that rooms are sufficient, nicely appointed, and that the facility is clean with no smell. Activities such as ice cream parties, singing, and arts and crafts were mentioned positively and contribute to a pleasant atmosphere for some residents. At the same time, at least one review described the facility as dirty, indicating inconsistent housekeeping or differing perceptions among visitors. Lost clothing and personal belongings were also reported, which points to problems with personal item management.

Dining, services, and logistics: Food quality is a frequent complaint; several reviewers called the food horrible or bad and said they did not like it. Practical coordination issues were raised as well: families said they were not helped setting up home care, were not notified about skin tears, and experienced delays in medication timing. Some reviewers acknowledged that COVID made tasks more difficult for staff, which may explain or partially mitigate some operational issues, but does not excuse reports of neglect or disrespect.

Overall patterns and implications: The dominant pattern is variability—strong, positive rehabilitation outcomes and highly praised CNAs/activities exist alongside reports of neglectful nursing care, communication failures, and serious clinical lapses. For prospective residents or families, this suggests the facility can deliver good rehab and has caring individuals on staff, but quality appears uneven and may depend heavily on unit, shift (weekday vs weekend), and particular caregivers. The most critical red flags are wound and pain management issues, slow response times, and reports of disrespect or neglect—these warrant careful inquiry by families.

Bottom line: Port Orange Nursing And Rehab Center demonstrates clear strengths in rehabilitation services, compassionate CNAs, cleanliness (per many reviewers), and social activities, which have led some families to recommend or reuse the facility for rehab. However, multiple serious and recurring complaints—poor nursing responsiveness, inadequate wound and pain care, poor communication, instances of neglect or disrespect, and inconsistent staffing—are significant. Families should weigh the positive rehab reports against the safety and communication concerns, ask specific questions about nursing coverage (especially weekends), wound-care protocols, call-light response times, and how the facility prevents and addresses lapses in care.
