    Spotless, friendly staff, excellent care

I'm very pleased with this facility - rooms are spotless, staff and nurses are friendly and attentive, and management is receptive and handles requests personally. The certified chef offers good meals and dietary options, activities and gathering spaces are active and welcoming, and rehab and social work teams went above and beyond to help residents.

    Summary of reviews

Overall impression: Reviews describe a facility with notable strengths in environment, activities, and certain frontline services, alongside operational inconsistencies that affect clinical care and family communication. Many families and residents highlight positive day-to-day experiencesโ€”clean rooms, spacious common areas, a lively activities calendar, and accessible spiritual programmingโ€”while others report variability tied to staffing and service continuity.

Care and staff: Multiple accounts praise individual staff members, rehabilitation teams, and social-work/case-management staff for being helpful, attentive, and focused on discharge goals. However, there is also a persistent pattern of staffing instability and turnover that reviewers link to slower response times, variable CNA performance, and uneven nursing engagement. These staffing patterns appear to produce inconsistent resident experiences: some describe prompt, compassionate care; others describe delays in assistance and concerns about medication handling or the tone of staff communication.

Dining and dietary services: Dining receives mixed feedback. Several reviewers commend a certified chef, dietary accommodations (including diabetic-friendly options), and a range of meal choices. Contrastively, other accounts indicate unreliable kitchen service and inconsistent meal quality. The cumulative pattern suggests that meal experience may depend on staffing, shift, or kitchen scheduling, making dining outcomes unpredictable for some residents.

Activities and facilities: The facilityโ€™s physical environment and programming are frequently cited as strengths. Large, open family-style gathering areas, active activity directors, plentiful social programming, an on-site chapel with music, and pleasant outdoor spaces are repeatedly noted. Room size and cleanliness are generally described positively, although housekeeping frequency and localized odor-control outcomes are uneven across reports.

Management and communication: Experiences with management are mixed. Several reviewers describe managers who are receptive and address concerns personally and promptly. Others describe difficulty reaching staff by phone, unanswered calls, and instances where complaints were not adequately followed up. This variability in leadership follow-through and communication responsiveness appears connected to broader staffing and operational challenges.

Notable patterns and guidance for families: The reviews suggest a facility that can provide a supportive, activity-rich environment with capable rehabilitation and social-work support, but whose performance is sensitive to staffing levels and shift-to-shift consistency. Prospective residents and families should consider an in-person tour, ask about current staffing ratios and turnover, request details on medication-administration safeguards and therapy schedules, inquire about housekeeping routines and odor-control protocols, and clarify the facilityโ€™s process for addressing family concerns and emergency communication. These checks will help assess whether the current operational state aligns with a prospective residentโ€™s care and quality-of-life priorities.

About Sandpiper Healthcare & Rehabilitation Center

Sandpiper Healthcare & Rehabilitation Center offers several care options like nursing home stays, rehabilitation, respite care, long-term skilled nursing, memory care, and services for Alzheimer's. The facility's recently renovated buildings have modern medical equipment, a therapy gym, comfortable lounges, and both private and semi-private rooms, though some reports mention holes in the walls and maintenance problems, so the place can feel new in some spots but worn in others. Residents get full medical attention, with attending doctors and registered nurses making unique care plans and staff on hand around the clock, while nurse turnover runs high at about 67.3% and total nursing hours per resident sits at 3.79 per day for an average of 73 residents out of 104 certified beds. There's a secured dementia wing for memory care, plus a safety system for those with cognitive issues, and transportation to appointments or outings happens regularly.

Sandpiper has a therapy gym and tailored orthopedic and rehabilitation programs, and the staff helps people recover from hospital stays with Transitional Care Units, aiming to send them home or to a more independent setting when possible. Rooms come with cable or satellite TV, Wi-Fi, air conditioning, and some have kitchenettes and private bathrooms; the staff supplies phones for resident use, and rooms are offered furnished if needed. Meals are made fresh by a professional chef, served restaurant-style with options for allergies or special diets, and residents can eat in the dining room or order room service, and guests are invited for meals too. There's a full calendar of activities, from religious services and arts and crafts to holiday parties, music programs, live shows, and movie nights in the facility's theater room, and people spend time in the activity room, computer room, game room, or outside on the patio, gardens, or walking paths.

Onsite, laundry and housekeeping are taken care of, mail and newspapers are delivered, and medication is managed by staff. The building is fully air-conditioned, with Wi-Fi and high-speed Internet throughout, and an emergency alert system is always working. Residents can use the salon, barber, fitness room, spa area, and community gathering rooms. The environment is pet-friendly, and the place does its best to accommodate individual needs, whether for dental, vision, or podiatry care, or making meals that meet each resident's nutrition plans created by dietitians and nutritionists. Family support and move-in coordination are offered, and the staff works to create a family-like setting where some caregivers have stayed for years.

Sandpiper's administration belongs to Midwest Recover Care LLC and aligns with Recover Care Healthcare, running all standard types of nursing facility programming. The site accepts both Medicare and Medicaid. Inspection history shows a total of 33 deficiencies, including three related to infections, and there are reports about pharmacy services deficiencies (F0761) and nutrition and dietary standards (F0812), so there's room for improvement in those areas. Staff assist with activities of daily living like dressing and bathing and provide help for folks who can't get around by themselves. Nursing coverage is strong with up to 16 hours daily care, and 24-hour supervision with a call system is standard. Many residents come for both short and long stays, and the center tries to focus on restoring independence and promoting a good quality of life by meeting each person's needs as well as possible.
