    Well cared for after surgery

I stayed here after open-heart surgery and felt genuinely well cared for - loving, professional staff, excellent PT and rehab, daily cleaning, good meals and plenty of activities. Overall a very good experience that gave me peace of mind; I would recommend it for recovery or longer stays.

    Summary of reviews

Overall impression: Reviews for Collinsville Nursing Home Inc are mixed but show clear patterns. Many families and residents praise the staffโ€™s compassion, the facilityโ€™s rehabilitation focus, and the general cleanliness and housekeeping. At the same time, a subset of accounts raise operational concerns about clinical oversight, responsiveness, and consistency of service.

Care and staff: Multiple reviewers describe caregivers with strong bedside manner who create a family-like atmosphere; therapists are frequently singled out as skilled and recovery-focused. These strengths appear strongest in the facilityโ€™s rehabilitation unit, where families report good therapy outcomes and attentive staff. However, other reviews describe variability in staff performance and communication, with issues such as slow responses to call lights and concerns about medication coordination. Those contrasts suggest the facility can deliver very good individualized care, but that consistency across shifts and units may vary.

Clinical management and safety: Positive statements about post-operative recovery and therapy are balanced by accounts that indicate gaps in clinical monitoring and follow-up for some residents. Specific operational themes include medication-management weaknesses and delays in attending to incontinence-care needs. These patterns can increase the risk of avoidable complications and, in at least one account, contributed to a hospital transfer; prospective families should inquire about current protocols for medication review, wound/incontinence care, and incident follow-up.

Dining and activities: Dining is generally well regarded, with several reviewers noting good meals. Activity programming is another consistent strength โ€” residents benefit from an active calendar, scheduled outings and field trips, and staff-run events that support engagement and socialization. This suite of programs supports a recovery-oriented environment, particularly for short-term rehabilitation stays.

Facilities and housekeeping: Many reviewers describe the facility as clean and well maintained, and daily room cleaning is cited as a consistent practice. Contrasting comments point to occasional housekeeping inconsistencies (sticky floors, missing personal items), and multiple reviewers note that decor and furnishings are dated. The physical plant appears functional and tidy overall, but families who prioritize modern rooms or upscale finishes should be aware of the facilityโ€™s more utilitarian presentation.

Management and limitations: A recurring operational theme is variability tied to staffing and reimbursement constraints; some reviewers attribute limitations in amenities or room updates to Medicaid funding realities. Given the mix of strong rehabilitation outcomes and uneven reports about responsiveness and clinical coordination, prospective residents and families should evaluate staffing patterns, therapy-minute expectations, medication oversight procedures, and mechanisms for escalating concerns during a tour.

Bottom line: Collinsville Nursing Home Inc displays notable strengths in compassionate caregiving, rehabilitation/therapy services, engagement programming, and generally solid housekeeping. At the same time, there are actionable concerns around responsiveness, medication and incontinence-care consistency, and upkeep of furnishings. Families considering this facility will benefit from targeted questions about call-light response times, medication-review processes, typical therapy schedules, and property-management procedures during an in-person visit or care-planning meeting.

    About Collinsville Nursing Home Inc

Collinsville Healthcare & Rehab is a large nursing facility located in Collinsville, Alabama, offering both short-term rehabilitation and long-term care services. With a capacity of 200 beds, it operates under for-profit, corporate ownership, and participates in both Medicare and Medicaid programs. While not part of a continuing care retirement community, this center provides comprehensive care for residents requiring various levels of medical and personal assistance.

Short-term rehabilitation services at Collinsville Healthcare & Rehab are designed to support patients who are recovering from hospital stays due to events such as strokes, heart attacks, infections, or accidental injuries. Approximately 51.9% of residents are able to return home after being discharged from short-term rehabilitation programs, highlighting the facilityโ€™s focus on recovery and transitional care. The staff strives to maintain a safe environment, with 0.5% of residents experiencing falls that result in major injuries and 5.2% of short-term rehabilitation stays leading to infections requiring hospitalization. During their stay, 14.7% of short-term residents have needed emergency room visits. Nurse staffing is an important aspect of care, averaging 4 hours and 8 minutes per resident per day to ensure that medical and daily living needs are consistently met.

Long-term care at Collinsville Healthcare & Rehab is structured to help residents maintain as much independence as possible while receiving daily assistance with essential tasks such as eating, dressing, bathing, and administering medications. A notable 87.7% of long-term residents have maintained the ability to move around, eat, and handle other common activities without assistance, reflecting the centerโ€™s commitment to supporting autonomy. The vast majorityโ€”99.3%โ€”have received the annual flu vaccination, underlining an emphasis on preventive health measures. However, weekend nursing staff levels are reported to be lower compared with weekday staffing, which is an important consideration for families planning long-term care.

Health, fire, and safety at Collinsville Healthcare & Rehab are subject to routine inspections. The most recent state inspection reports indicate that the facility has effectively met key standards related to food preparation and other resident safety factors. Past inspection findings have identified areas for improvement such as enhancing communication of information to residents and healthcare providers during planned discharges and implementing robust infection prevention and control programs. These issues were found to present low potential for harm and affected only a few residents, and corrective actions were taken in a timely manner.

Collinsville Healthcare & Rehab offers a supportive environment for those recovering from acute medical events as well as individuals who need ongoing assistance with daily living activities. By providing a blend of skilled nursing care, dedicated staff attention, and a focus on both rehabilitation and long-term support, the facility aims to address the evolving needs of seniors and individuals with complex medical requirements.
